Introduction to Summary of BenefitsThank you for your interest in Anthem Senior Advantage Basic (HMO). Our plan is offered by Community InsuranceCompany which is also called Anthem Blue Cross and Blue Shield, a Medicare Advantage Health MaintenanceOrganization (HMO) that contracts with the Federal government. This Summary of Benefits tells you some featuresof our plan. It doesn't list every service that we cover or list every limitation or exclusion. To get a complete list of ourbenefits, please call Anthem Senior Advantage Basic (HMO) and ask for the "Evidence of Coverage."

You have choices in your health careAs a Medicare beneficiary, you can choose from differentMedicare options. One option is the Original(Fee-for-Service) Medicare Plan. Another option is aMedicare health plan, like Anthem Senior AdvantageBasic (HMO). You may have other options too. Youmake the choice. No matter what you decide, you arestill in the Medicare Program.

You may join or leave a plan only at certain times. Pleasecall Anthem Senior Advantage Basic (HMO) at thetelephone number listed at the end of this introductionor 1-800-MEDICARE (1-800-633-4227) for moreinformation. TTY/TDD users should call1-877-486-2048. You can call this number 24 hours aday, 7 days a week.

How can I compare my options?You can compare Anthem Senior Advantage Basic(HMO) and the Original Medicare Plan using thisSummary of Benefits. The charts in this booklet list someimportant health benefits. For each benefit, you can seewhat our plan covers and what the Original MedicarePlan covers.

Our members receive all of the benefits that the OriginalMedicare Plan offers. We also offer more benefits, whichmay change from year to year.

Where is Anthem Senior AdvantageBasic (HMO) available?The service area for this plan includes:

Ashland, Clark, Clermont, Cuyahoga, Darke, Fairfield,Fulton, Geauga, Lake, Licking, Madison, Medina,Ottawa, and Warren Counties. OH.

You must live in one of these areas to join the plan.

Who is eligible to join Anthem SeniorAdvantage Basic (HMO)?You can join Anthem Senior Advantage Basic (HMO)if you are entitled to Medicare Part A and enrolled inMedicare Part B and live in the service area. However,individuals with End-Stage Renal Disease generally arenot eligible to enroll in Anthem Senior Advantage Basic(HMO) unless they are members of our organization andhave been since their dialysis began.

Can I choose my doctors?Anthem Senior Advantage Basic (HMO) has formed anetwork of doctors, specialists, and hospitals. You canonly use doctors who are part of our network. The healthproviders in our network can change at any time.

You can ask for a current provider directory. Foran updated list, visit us at http://www.anthem.com/medicare. Our customer service number is listed at theend of this introduction.

What happens if I go to a doctor who'snot in your network?If you choose to go to a doctor outside of our network,you must pay for these services yourself. Neither the plannor the Original Medicare Plan will pay for these servicesexcept in limited situations (for example, emergency care).

Where can I get my prescriptions if Ijoin this plan?Anthem Senior Advantage Basic (HMO) has formed anetwork of pharmacies. You must use a networkpharmacy to receive plan benefits. We may not pay foryour prescriptions if you use an out-of-networkpharmacy, except in certain cases. The pharmacies in ournetwork can change at any time. You can ask for apharmacy directory or visit us athttp://www.anthem.com/medicare. Our customerservice number is listed at the end of this introduction.

Anthem Senior Advantage Basic (HMO) has a list ofpreferred pharmacies. At these pharmacies, you may getyour drugs at a lower co-pay or co-insurance. You maygo to a non-preferred pharmacy, but you may have topay more for your prescription drugs.

What if my doctor prescribes less thana month's supply?In consultation with your doctor or pharmacist, you mayreceive less than a month's supply of certain drugs. Also,if you live in a long-term care facility, you will receiveless than a month's supply of certain brand and genericdrugs. Dispensing fewer drugs at a time can help reducecost and waste in the Medicare Part D program, whenthis is medically appropriate.

The amount you pay in these circumstances will dependon whether you are responsible for paying coinsurance(a percentage of the cost of the drug) or a copay (a flatdollar amount for the drug). If you are responsible forcoinsurance for the drug, you will continue to pay theapplicable percentage of the drug cost. If you areresponsible for a copay for the drug, a "daily cost-sharingrate" will be applied. If your doctor decides to continuethe drug after a trial period, you should not pay more fora month's supply than you otherwise would have paid.Contact your plan if you have questions aboutcost-sharing when less than a one-month supply isdispensed.

Does my plan cover Medicare Part Bor Part D drugs?Anthem Senior Advantage Basic (HMO) does cover bothMedicare Part B prescription drugs and Medicare PartD prescription drugs.

What is a prescription drug formulary?Anthem Senior Advantage Basic (HMO) uses aformulary. A formulary is a list of drugs covered by yourplan to meet patient needs. We may periodically add,remove, or make changes to coverage limitations oncertain drugs or change how much you pay for a drug.If we make any formulary change that limits ourmembers' ability to fill their prescriptions, we will notifythe affected members before the change is made. We willsend a formulary to you and you can see our completeformulary on our Web site at http://www.anthem.com/medicare.

If you are currently taking a drug that is not on ourformulary or subject to additional requirements or limits,you may be able to get a temporary supply of the drug.You can contact us to request an exception or switch toan alternative drug listed on our formulary with yourphysician's help. Call us to see if you can get a temporarysupply of the drug or for more details about our drugtransition policy.

How can I get extra help with myprescription drug plan costs or getextra help with other Medicare costs?You may be able to get extra help to pay for yourprescription drug premiums and costs as well as get helpwith other Medicare costs. To see if you qualify forgetting extra help, call:

What are my protections in this plan?All Medicare Advantage Plans agree to stay in theprogram for a full calendar year at a time. Plan benefitsand cost-sharing may change from calendar year tocalendar year. Each year, plans can decide whether tocontinue to participate with Medicare Advantage. A planmay continue in their entire service area (geographic areawhere the plan accepts members) or choose to continueonly in certain areas. Also, Medicare may decide to enda contract with a plan. Even if your Medicare AdvantagePlan leaves the program, you will not lose Medicarecoverage. If a plan decides not to continue for anadditional calendar year, it must send you a letter at least90 days before your coverage will end. The letter willexplain your options for Medicare coverage in your area.

As a member of Anthem Senior Advantage Basic (HMO),you have the right to request an organizationdetermination, which includes the right to file an appealif we deny coverage for an item or service, and the rightto file a grievance. You have the right to request anorganization determination if you want us to provide orpay for an item or service that you believe should becovered. If we deny coverage for your requested item orservice, you have the right to appeal and ask us to reviewour decision. You may ask us for an expedited (fast)coverage determination or appeal if you believe thatwaiting for a decision could seriously put your life orhealth at risk, or affect your ability to regain maximumfunction. If your doctor makes or supports the expeditedrequest, we must expedite our decision. Finally, you havethe right to file a grievance with us if you have any typeof problem with us or one of our network providers thatdoes not involve coverage for an item or service. If yourproblem involves quality of care, you also have the rightto file a grievance with the Quality ImprovementOrganization (QIO) for your state. Please refer to theEvidence of Coverage (EOC) for the QIO contactinformation.

As a member of Anthem Senior Advantage Basic (HMO),you have the right to request a coverage determination,which includes the right to request an exception, the rightto file an appeal if we deny coverage for a prescriptiondrug, and the right to file a grievance. You have the rightto request a coverage determination if you want us tocover a Part D drug that you believe should be covered.

An exception is a type of coverage determination. Youmay ask us for an exception if you believe you need adrug that is not on our list of covered drugs or believeyou should get a non-preferred drug at a lowerout-of-pocket cost. You can also ask for an exception tocost utilization rules, such as a limit on the quantity ofa drug. If you think you need an exception, you shouldcontact us before you try to fill your prescription at apharmacy. Your doctor must provide a statement tosupport your exception request. If we deny coverage foryour prescription drug(s), you have the right to appealand ask us to review our decision. Finally, you have theright to file a grievance if you have any type of problemwith us or one of our network pharmacies that does notinvolve coverage for a prescription drug. If your probleminvolves quality of care, you also have the right to file agrievance with the Quality Improvement Organization(QIO) for your state. Please refer to the Evidence ofCoverage (EOC) for the QIO contact information.

What is a Medication TherapyManagement (MTM) Program?A Medication Therapy Management (MTM) Programis a free service we offer. You may be invited to participatein a program designed for your specific health andpharmacy needs. You may decide not to participate butit is recommended that you take full advantage of thiscovered service if you are selected. Contact AnthemSenior Advantage Basic (HMO) for more details.

What types of drugs may be coveredunder Medicare Part B?Some outpatient prescription drugs may be covered underMedicare Part B. These may include, but are not limitedto, the following types of drugs. Contact Anthem SeniorAdvantage Basic (HMO) for more details.

Some Antigens: If they are prepared by a doctor andadministered by a properly instructed person (whocould be the patient) under doctor supervision.Osteoporosis Drugs: Injectable osteoporosis drugs forsome women.Erythropoietin: By injection if you have end-stagerenal disease (permanent kidney failure requiringeither dialysis or transplantation) and need this drugto treat anemia.

Hemophilia Clotting Factors: Self-administeredclotting factors if you have hemophilia.Injectable Drugs: Most injectable drugs administeredincident to a physician's service.Immunosuppressive Drugs: Immunosuppressive drugtherapy for transplant patients if the transplant tookplace in a Medicare-certified facility and was paid forby Medicare or by a private insurance company thatwas the primary payer for Medicare Part A coverage.Some Oral Cancer Drugs: If the same drug is availablein injectable form.Oral Anti-Nausea Drugs: If you are part of ananti-cancer chemotherapeutic regimen.Inhalation and Infusion Drugs administered throughDurable Medical Equipment.
