Director’s messageDear Friends,

We very much appreciate your interest in the Alzheimer’s & Dementia Alliance of Wisconsin (ADAW). This annual report covering the period of July 1, 2012 through June 30, 2013 highlights many of our accomplishments as we continue to grow and plan for the future.

One of the biggest challenges we face from year to year is helping connect people diagnosed with a dementia disease with the programs and services we offer. Changing existing systems is never an easy task but there are small inroads taking place.

Not long ago, two of our program staff members met with a physician faculty member from the University of Wisconsin-Madison who recently relocated from another major research institution. I was very proud to hear that this faculty member was thrilled with the wide array of programs and services that we offer persons with dementia and their family and friends. Soon student learners working on degrees in the health professions (physicians, physician assistants, pharmacists) will be attending our educational programs and observing our staff doing care consultations. This is just one example of how the people we serve help educate our staff and future health care providers so we can all work together to enhance important system changes needed to make life better for everyone impacted by dementia.

Program growth during this year includes the addition of two new Memory Cafés, Richland Center and a Pot-luck Café for persons under 65 that meets on Saturdays in our Madison office. We have a couple from Neshkoro who travel to Madison monthly for the Pot-luck Café. Memory Café’s provide a warm and welcoming environment for conversation, laughter and learning. We are proud to be Wisconsin’s leader in developing a network of Cafés so those impacted by memory loss can continue to enjoy life.

Our state-of-the-art dementia specialist training program is now being taught through Madison College so more students can take advantage of its unique curriculum. We continue to try to develop strategies to improve quality of institutional care through our training programs and this is one way to teach person-centered care as CNAs are developing their skills.

New staff members include Nichole Hawkins, Administrative Assistant who joined us in December 2012, Joy Schmidt, Community Education Specialist and Kira Stewart, National Family Caregiver Support Program (NFCSP) Coordinator, who both joined us in January 2013. Joy has been busy putting on community education programs everywhere, while Kira administers the federal caregiver program within Dane County’s Aging & Disability Resource Center. ADAW is the first community agency in Wisconsin to administer both the federal NFCSP program and the state Alzheimer’s Family Caregiver Support Program (AFCSP) within one entity, which is considered a “best practice” by Wisconsin’s Department of Health Services.

We continue to provide a wide array of critical programs and services for an agency our size. This includes 257 educational programs, 1200 visits at dozens of support groups, 2065 helpline calls and 720 in-depth care consultations. We are proud to be your partner along the journey, and our dedicated staff with decades of dementia experience is always willing to help. When you call us, you are talking to someone in your community and, more importantly, you can call back and talk to the same person so you don’t have to start all over.

Our local board of directors who have fiduciary responsibility for our agency has added members. This group of very committed volunteers forms overall agency policy while helping with critical fundraising responsibilities. Paul Pitas, Director of Public Relations and Communications for Culvers, joined us in November 2012. Beverly Simone, President Emeritus of Madison College, joined in March 2013 along with retired UW physician Dr. Perry Henderson in July 2013. We are fortunate to have such a dedicated group working so hard to make our programs and services, and thus our community, stronger and more welcoming to those impacted by dementia.

Highlights of FY 2012 - 2013

Centers of Excellence Awarded

The Alzheimer’s & Dementia Alliance of Wisconsin congratulates the staff of Sienna Meadows in Oregon, WI for their completion of the Center of Excellence in dementia care program. Sienna Meadows served as the mid-sized dementia care unit in the ADAW’s pilot program, which began in September 2012. ADAW is working hard to update the program for other facilities to participate.

Centers of Excellence is an intensive training designed to integrate the principles and tools from the ADAW’s successful Dementia Specialist Training Program into all levels of a long-term care setting, including buy-in of owners and administrators, staff, volunteers, outside providers, housekeeping, dietary, and family. After a tight training time-line, ADAW, the Dane County Department of Human Services and South Madison Coalition of the Elderly continue to support and mentor everyone in the care setting for approximately three months.

Memory Cafés

In 2012 ADAW launched two the Memory Cafés–the first ever in Wisconsin. This year has brought many more! ADAW was able to start Memory Cafés in both Richland County and Dane County (the second one!).

Based on a successful model in the Netherlands and United Kingdom, Memory Cafés address the tendency for people with early memory loss, mild cognitive impairment or dementia to isolate themselves, by providing a place to relax and socialize without fear.

Madison College Course Offered

ADAW’s popular training program is now offered at Madison (Area Technical) College! ADAW’s own Professional Training Specialist puts on the course. Recommended for direct-care staff, the Dementia Specialist: Best Practices for Direct Care Staff course provides comprehensive training on dementia, its impact on a person’s life, how to effectively communicate and build relationships with people with dementia and give them extraordinary care. The 15 hour program offers creative and interactive learning with many opportunities for skills application and team building.

The Alzheimer’s & Dementia Alliance of Wisconsin is a nonprofit organization dedicated to improving the lives of persons with Alzheimer’s disease or related dementias and their families. The Alliance is a guide to those coping with this disease as they learn about options and begin to plan for the future. Contact us for help and support. We are your partner along the journey.

For families

Trained professionals and volunteers provide invaluable support, resources and information to callers.

Our website provides a wealth of information on Alzheimer’s and other dementias, upcoming programs and events and community resources by county.

Family care consultations

Family support specialists provide short-term, professional consultation by appointment. They assist families with accessing resources, working with challenging behaviors, problem-solving issues and planning for future care needs.

Caregiver support groups

At support groups, family care partners come together to share information, experiences and support.

Alzheimer’s Family and Caregiver Support Program (AFCSP)

AFCSP offers support and limited funding to qualified families caring for a person with Alzheimer’s disease or other irreversible dementia. The funding can be used for a wide variety of goods and services and is available in each county.

Dementia and developmental disabilities

We provide training on how the dual diagnosis of dementia and developmental disabilities affects the person and specific issues that arise for the person and support staff.

Family education

Introduction to Alzheimer’s and dementia: Getting started

Learn about dementia and its warning signs, what to do if you are concerned about yourself or someone you know and how the Alliance can help.

ABC’s of Alzheimer’s and dementia

This program includes information on medical aspects of dementia, research, effects of dementia on the person, safety, communication and much more.

Legal and Financial Planning Seminar

A two-part seminar for families that provides a general overview of legal and financial issues to consider for loved ones with memory loss, Alzheimer’s disease or other dementia.

Monthly Family Caregiver Education Program

Learn about topics of interest to family

and friends of those with dementia such as activities, communication, help for caregivers, resources and more. Held monthly in Madison; topics presented in other areas as scheduled.

Savvy Caregiver Training

This 12-hour, multi-session training goes beyond introductory education programs and provides in-depth, practical training individualized for each family caregiver.

Powerful Tools for Caregiving

A six-part class for people who care for loved ones with stroke, Alzheimer’s, Parkinson’s disease or similar long-term conditions. Classes cover a variety of topics such as reducing stress, communicating effectively, dealing with emotions and making tough decisions.

Community education for groups and organizations

Maintain Brain Health

An interactive, scientifically-based workshop which features nutritional and lifestyle advice and strategies to keep your brain sharp and resilient.

We All Forget: Is it normal aging or should I be concerned?

This program is ideal for persons concerned about their own memories as they age or those of friends and loved ones.

Alzheimer’s and dementia: What is it? What can I do about it?

This presentation offers an overview of Alzheimer’s disease and dementia including common warning signs, risk factors and treatment options

Memory Cafés

A Memory Café is a social gathering place for persons challenged by memory loss, mild cognitive impairment or early Alzheimer’s/dementia and their family or friends. Partipants meet once a month in a relaxed environment to visit, share common interests, enjoy refreshments and a program or activity that’s stimulating and fun.

For professionals

Dementia Specialist: Best Practices for Direct Care Staff

A highly interactive, two-day training which is designed specifically for direct care staff working with people with Alzheimer’s. Participants learn together through role-plays, case studies and problem solving. Practical tips and tools are given to increase success and job satisfaction.

Professional workshops and in-service training

Dementia-specific training and in-service education (typically one to three hours) for service providers such as assisted living facilities, nursing homes, home health agencies, law enforcement and social service organizations.

Additional services

Advocacy

Our active Public Policy Committee and statewide network advocates for policies and programs to assist persons with Alzheimer’s and their care partners before county, state and federal governments. Anyone interested in participating in advocacy is encouraged to join the network.

Library services

Books, videos and brochures on a wide variety of topics related to Alzheimer’s, dementia and caregiving are available for families and professionals.

Newsletters (print and email)

Get information about local programs and services, the latest news about Alzheimer’s disease and related dementias, updates on research and resources and more. Contact us to be put on a mailing list for our free quarterly print newsletter. Subscribe to our email newsletter by visiting our website at www.alzwisc.org.

Resource guides

These guides provide information by county on local resources such as financial support, respite services, adult day programs, assisted living, nursing homes and more.

Conferences

Educational conferences are held regularly throughout our area to provide an opportunity for families, direct care workers and other professionals to hear speakers on a variety of topics related to Alzheimer’s disease and related dementias.

For those with mild memory loss or early stage Alzheimer’s

Crossing Bridges

Crossing Bridges is a four-week educational discussion group for people with mild memory loss (or in the early stages of dementia) and a primary care partner. The overall philosophy of the group is to promote a fulfilled life while living with cognitive change. An interview is required before participating in this group.

Learning Among Friends

After completing Crossing Bridges, this support group program is an opportunity for participants to continue to meet together regularly to discuss topics of their choice and share support.

Meeting of Minds

Meeting of Minds is a memory enhancement program for people with mild memory loss that includes stimulating activities, intellectual exercises and mild exercise. The main goal is to help people with memory loss positively impact their lives. Completion of Crossing Bridges is required before participating in this program.

Ways to donate

Donation

The most immediate way to show support is by donating securely online, in person, over the phone or by mail. Donations are received throughout the year from individuals, businesses and foundations. Gifts are tax-deductible.

Payroll giving

Contribute through payroll giving at work through Community Health Charities of Wisconsin or any United Way agency coast-to-coast. Simply indicate “Alzheimer’s & Dementia Alliance of Wisconsin” on your pledge form. We are United Way agency number 370. Payroll giving provides an opportunity for you to contribute on a regular basis while also receiving a convenient statement for your taxes.

Tribute and memorial gifts

Make a gift in honor or memory of a loved one or friend.

Circle of Honor Remembrance Wall

Located in a permanent area at the entrance to our office, this space provides another way for you to pay tribute to loved ones in your life. Names are grouped alphabetically within each gift category: $500-$999, $1,000-$1,999 and gifts of $2,000 and above. You may select an inscription of up to 30 characters.

Planned gifts

Please remember our organization in your will or trust outlining your legacy to the community. Here is sample

bequest language you can take to your professional advisor: “I give, devise and bequeath (insert amount of gift, or percentage of estate, or residuary of estate) to the Alzheimer’s & Dementia Alliance of Wisconsin, Inc., or its successor organization, located in Madison, Wisconsin, to support programs and services in southern Wisconsin. (EIN # 39-1679333).”

Life insurance and retirement plans

You also may name our organization as the beneficiary of a life insurance policy or retirement plan.

Charitable gift annuities

Alzheimer’s & Dementia Alliance charitable gift annuities are also available to individuals seeking regular income throughout their lives and/or the life of a loved one. Specific payment amounts depend on the age of the annuitant at the time the annuity is established.

Grants and corporate gifts

Grants and corporate gifts help us secure funding for many of the Alliance’s programs.

Matching gifts

Fundraising events

Wine and Roses Festival™

Guaranteed to be a fun-filled evening of wine tasting and good food, you don’t want to miss this annual event held in April. Celebrate spring with wine tasting, hors d’oeuvres, live music and live and silent auctions featuring exclusive items. Talk with winemakers from vineyards across the country as well as our own backyard and sample delicious wines, some of which may not be available in our area.

Million Dollar Shootout

Our annual golf tournament takes place in August. In addition to fun on the links, the event includes a silent auction with many popular sports-related items. There is an awards dinner following the tournament for both golfers and non-golfers. The event is co-sponsored by NBC 15 in Madison.

Alzheimer’s Walk

The Alzheimer’s & Dementia Alliance’s Alzheimer’s Walks are held in the fall and are our largest awareness and fundraising events. The walks help secure funds for our local programs and services and take place in seven counties in south central Wisconsin. All dollars raised stay in Wisconsin. We invite you to step forward and join your neighbors in supporting the cause.

To support individuals, families and professionals impacted by Alzheimer’s disease and other dementias through information, education, consultation and advocacy; and to promote the advancement of scientific research to better

diagnose, treat and ultimately eliminate Alzheimer’s disease.

About the Alzheimer’s & Dementia Alliance of Wisconsin

The Alzheimer’s & Dementia Alliance of Wisconsin is a nonprofit organization based in Madison that provides direct programs and services in south central Wisconsin and advocacy on dementia-related issues affecting the entire state.

The Alliance began providing services as a nonprofit organization in 1985. In 1990, it was incorporated as a chapter of the National Alzheimer’s Association. In 2009, the Alzheimer’s & Dementia Alliance of Wisconsin became an

independent nonprofit organization and is governed by a local board of directors.

We also maintain two regional offices – one serving Grant and Richland counties, and the other serving Columbia and Sauk counties.
