Dear donors and supporters,

Thanks to your generous donations and hard work – the numerous creative fundraisers at agencies around the nation, and work with local and national businesses in support of our cause – 2018 was a great year for the Hospice Promise Foundation. I continue to hear stories of patients and families who felt they had no hope, only to find an ally in a nurse, social worker, volunteer, or other staff member who cared enough to make a difference.

The clinicians, staff, and volunteers on the ground are making that difference – thanks to the continued generosity of people like you. More than ever, your kindness, your willingness to help, and your compassion for our patients and their families has touched the lives of so many.

In 2018, we provided 582 grants totaling $281,998 for hospice patients and families in need in cities, towns, and communities around the nation. We are reaching further, and with your help we will continue to expand our efforts and touch more lives as we help patients and families peacefully navigate life’s final journey.

As always, I want to thank our LHC Group employees for their continued generous donations through payroll deductions, and LHC Group for funding all of our administrative expenses so that 100 percent of our donations go directly to the patients and families we serve.

And, of course, I want to recognize our board members for donating their time and efforts to making the Hospice Promise Foundation possible as well as our Administrator in 2018, Annie Perret:

• Lori Stagg – Vice President• JoAnne Little – Secretary• Jody Soileau – Treasurer• Ginger Myers – Board Member• Paula Mills – Board Member• Dr. Ben Doga – Board Member

Our momentum is strong, and growing stronger. I look forward to what we will accomplish together in 2019. It is a genuine honor to play a role in this outstanding program.

Thank you.

Heart-Warming StoriesEasing the BurdenHospice of Central ArkansasHOT SPRINGS, AR

The family of a patient served by Hospice of Central Arkansas was struggling with the loss of their son and paying for his burial

expenses. The agency team reached out to the Hospice Promise Foundation for assistance and was able to provide the grieving

family with the funds they needed.

Weeks later, the hospice team received the following letter from the patient’s mother:

“To all who donate to the Hospice Promise Foundation, thank you all for your generosity of both spirit and money. Your assistance

allowed me to have my son cremated without incurring a debt that would have been overwhelming at this particular time in my

life. It helps to know that there are people who truly care.”

Home ImprovementsHeartlite HospiceELLIJAY, GA

A family on service with Heartlite Hospice had their home

burn down several years ago. They rebuilt a tiny home on

their property, but there was no money left for hot running

water. The family lived as best they could, taking baths in

a wading pool in the yard when the weather allowed and

heating water on the stove to wash dishes.

At her first meeting with the family, Wanda Brady, MSW, took

note of the way they were living and came back to discuss it

with her team members. She arranged for a plumber to visit

the family and got an estimate on what it would cost to get

hot running water in the kitchen and bath.

The cost was almost three times the amount of the average

grant. But because of the circumstances, their $1,700 grant

was approved and the work was completed in days. Without

a doubt, the grant greatly improved this family’s quality of life.

Helping Children GrieveHalcyon HospiceGREENVILLE , SC

For a third year, the Hospice Promise Foundation helped fund

Camp Sunburst – an overnight camping experience for children

(grades 1-6) who have lost a parent, sibling, or other relative.

From April 20-22, over 30 excited campers descended on the

Clemson Outdoor Lab to play, learn, remember, and grieve with

children who have had the same experience—and the mentors

who help them heal.

This year, Jeff Holland of Our World Festival involved campers

and staff in the use of drums and other percussion instruments

for exercises utilizing rhythm and music for healing, stress

reduction, and relaxation. Campers spent time together singing,

playing, releasing butterflies, and talking about emotions,

allowing them to learn about grief alongside others going

through similar experiences. Campers also created memory boxes to fill with hearts, touch stones, and other memory items.

Camp Sunburst was founded in 1996. It is the only grief camp in the South Carolina upstate area and is a combined effort of hospice

programs and other contributing agencies in five counties.

Give the Gift of Time, Family, and SupportHousecalls HospicePARKERSBURG, WV

A patient at Housecalls Hospice had a last wish – to celebrate

Christmas with her family before she passed. The staff reached out

to the Hospice Promise Foundation for a $700 grant to help fulfill this

wish. The patient was able to buy her family Christmas presents and

food for the holiday.

The Housecalls Hospice staff also donated ornaments so the patient’s

house could be decorated for the holiday. The patient had always

loved Christmas cards, so the staff helped spread the word. As a

result, the patient received over 115 kind and loving Christmas Cards!

Pure JoyHousecalls HospicePARKERSBURG, WV

With temperatures soaring into the mid-to-upper 90s, and a heat index

in the 100s, the team at Housecalls Hospice in Parkersburg, WV, helped

a patient and her family obtain a grant from the Hospice Promise

Foundation to purchase an air conditioner.

Diana Jones, SW, ordered the AC unit online and had it delivered to the

patient’s home. With the money left over, Diana and Lora Sams, CH, went

grocery shopping for the family. They bought several staple food items as

well as some “goodies.”

As evidenced in the accompanying photo of Mrs. Ambrose, those

“goodies” were a great delight!

She was excited by each of the items purchased, but two in particular

gave her great joy – a huge tub of peanut butter and a package of Nutter

Butter cookies. She was so happy, she wanted a copy of her picture taken

with the cookies.

A Forced Move Made EasierTennova HospiceCLARKSVILLE , TN

Due to poor home conditions, a patient and his spouse were forced to move out of their home by their landlord. Both the patient and

his spouse were on a fixed income and could not afford both moving expenses and their electric bill. With help from the team at

Tennova Hospice, the Hospice Promise Foundation provided the grant money to pay their electric bill, including past due and current

electric charges.

Baked with LoveWest Virginia HospiceBUCKHANNON, WV

West Virginia Hospice in Buckhannon, WV, recently had

a “Bake Sale and Raffle” at a local Walmart Store to

raise funds for the Hospice Promise Foundation. The

entire team baked goodies, worked the event, and sold

raffle tickets for some very special handmade wreaths

and baskets. Sherry Sellers, a new volunteer with the

Buckhannon team, made the wreaths and baskets for

this event.

This fundraiser earned $547 on the baked goods

and close to $300 on the raffle items. The event was

a HUGE success! The staff enjoyed spending time

together and getting a chance to talk to folks in their

community about hospice.

Celebrate Your HeroesLouisiana Hospice and Palliative CareMONROE, LA

Louisiana Hospice and Palliative Care in Monroe held their

first annual “Celebrate Your Heroes” 5K and one-mile run/

walk this year. The event drew a huge crowd of community

runners, walkers, and volunteers who were encouraged to

dress up as their favorite hero in celebration of our everyday

heroes who help make a difference in the lives of others.

Awards were given to teams with the best costume, the best

team name, and the largest team. All individual participants

were placed in a drawing for a door prize. Participants took

free photos with Batman and his Batmobile and enjoyed a

free McDonald’s breakfast the morning of the event. The

event raised close to $16,000 to help hospice patients and

families in need.

Keeping the PromiseHeartlite HospiceELLIJAY, GA

Keeping the Promise is an annual fundraiser held by Heartlite

Hospice of Ellijay. The purpose is to raise money for the Hospice

Promise Foundation while raising awareness of their hospice

services in the community. With the money raised through these

fundraisers, Heartlite Hospice has been able to assist families

with bills, moving expenses, final arrangements, necessary

home repairs, final wishes, and other non-hospice related items.

The most recent event was held at a local winery on a Friday

evening. Tickets sold for $25 each, which included five wine

tastings, a glass of your choice, lite fare, live music, and a silent

auction.

The generosity of the community was heartwarming. Several

businesses were Gold Sponsors, donating $400 or more. Many

smaller businesses couldn’t make a monetary donation, but did

donate items or services for the silent auction.

Some of the donated services included: guided fishing trips,

worth $400; a professional teeth whitening system donated by

a local dentist, worth $300; gift certificates donated by several

local restaurants; games from the local golf course; and ten

passes from the newly opened movie theater. They had local

jams and jellies, artwork, small appliances, grills, and many

more great items.

With close to 80 people attending, they raised over $9,000 for

the Hospice Promise Foundation.

A great deal of positive feedback was received from the

community and Heartlite Hospice was able to reach out and

financially help several of their families throughout the year.

South Louisiana Giving Day BBQLourdes HospiceLAFAYETTE , LA

In May, Lourdes Hospice held a South Louisiana Giving Day

(SOLA) BBQ fundraiser at LHC Group’s Lafayette Home Office

to support the Hospice Promise Foundation. SOLA – a 24-hour

online giving event – is organized by Community Foundation of

Acadiana and supports nonprofit organizations, churches, and

schools in our communities.

The hospice team spent weeks preparing for the fundraiser

and working with local organizations to obtain donations of

food items, supplies, and volunteers for the BBQ.

The event was a huge success and raised over $3,000 for the

foundation.

St. Patrick’s Day ParadeHospice of Central ArkansasHOT SPRINGS, AR

Every year, Hot Springs has a St. Patrick’s Day parade – known

as the shortest parade in the world. The city plans the parade

with celebrity Grand Marshall Joey Fatone, parade starter

Jon Heder, the Dallas Cowboy Cheerleaders, the Budweiser

Clydesdale Horses, and live performances from artists such as

Grand Funk Railroad and Morris Day.

This year’s event drew around 40,000 people and was held

outside the Hospice of Central Arkansas office building. The

hospice team set up a table outside of their office and sold

bottled water for $1 and handed out 1,400 beaded necklaces.

The event raised $810 for the Hospice Promise Foundation.

The Hospice Promise Foundation is a non-profit organization, and your donation is tax-deductible to the extent allowed by law. Donations collected will go to the general fund to support all hospice patients. Donors can also request their gift be earmarked for hospice patients in our care in specific cities, states, or regions. To make a donation or learn about benefits, visit us at LHCgroup.com/foundation.

Our purposeThe Hospice Promise Foundation assists patients and families with essential, non-hospice related expenses they are unable to afford themselves – such as rent and utility bills, emergency repairs, personal care items, and food assistance. The foundation also provides funding for patients’ last wishes, burial assistance, and community projects like bereavement camps for children and educational programs for end-of-life care.

Our promiseIn communities across the country, we’re working every day to ensure hospice patients live comfortably, with dignity, when a cure is no longer possible. But to continue our vital work, we need your support. In return, we pledge to promote our mission to provide comfort and care to the hospice patients and families who have placed their trust in us – and who we are so blessed to serve.

Become a hospice promise heroWhen you make a donation to The Hospice Promise Foundation, 100 percent of your donation goes directly to patients and families in need. Your generosity will make a difference in the life of another, and help ensure his or her final days are spent worry-free, and surrounded by loved ones in the peace and comfort of home.

Giving is caring. Become a hospice promise hero today.
