    REPORT FINDINGS

    The ALATAS SERVICE TEAM was invited to carry out proof load testing with water bags for mid ship Hose Handling cranes and also perform static load testing on provision cranes, pump room davit and engine room crane and the life boat davit & life boat, and also perform crane inspection of above mention cranes. However DNV-GL surveyor insisted on dynamic testing, so it took a lot of additional time time shifting 35t water bag (approx. 600kg) manually by men power from mid ship to behind accommodation and perform dynamic load testing with water bags. And then shift back water to mid ship again by boat crew assistance to perform load testing STBD Hose Handling Crane which was not allowed to perform load testing as bunkering activities was ongoing just right beside STBD Hose Handling Crane.

    A) LOAD TEST CRANES:------------------------------------

    - SWL dynamic Load Testing and Proof Load Testing with Water bag in both Hose Handling Cranes.

    - SWL dynamic load testing and Proof Load Testing with water bag in both Provision Crane.

    - Static Load Testing in Engine Room Overhead Crane - Static Load Testing in Steering Room Davit ( Not include in work scope as per

    correspondence) - Static Load Testing in Pump Room Davit

    B) LIFEBOAT DAVIT OVERLOAD TEST (PORT AND STBD):-------------------------------------------------------------------------------------

    - Perform overload test on the lifeboat davits using the empty lifeboat and water bags: SWL: 5,536 kg Overload: 6090kg

    Note: Lifeboat davit overload test witnessed by Class surveyor.

    C) CRANE CONDITION EVALUATION:------------------------------------------------------ As per original scope of work, also performed crane inspection prior to proof load testing so that we know for sure that crane are safe to be proof load tested. During crane inspection, the issues were found in the following individual cranes.

    PORT HHC: 1. Hoist winch drum heavily corroded. Hydraulic motor valve block heavily corroded.

    Winch gearbox oil pipe assembly heavily corroded. Recommend to overhaul the wholewinch drum assembly.

    2. Hydraulic tank sight glass heavily corroded. Recommend to change.

    3. Luff cylinder balance valve heavily corroded. Recommend to overhaul.4. Multiple slew bearing bolt and nuts (outside) heavily corroded. Recommend to change

    20% of the total number of bolts.

    STBD HHC: 1. Hoist winch drum heavily corroded. Hydraulic motor valve block heavily corroded.

    Winch gearbox oil pipe assembly heavily corroded. Recommend to overhaul the wholewinch drum assembly.

    2. Hydraulic tank sight glass heavily corroded. Recommend to change.3. Luff cylinder balance valve heavily corroded. Recommend to overhaul.

    STBD PROVISION CRANE: 1. It produces abnormal noise hydraulically from lowering brake valve whilst boom down

    motion both without load and with SWL load. And also observed that return hosesvibrate during boom lowering motion with SWL load. Possible cause of abnormal noiseissue is that malfunctioned lowering brake valve or incorrect adjustment of valve whichis attached to cylinder. Therefore, It recommends to troubleshoot and rectify the rootcause of abnormal noise produced from boom lowering motion in next available vesselschedule in Singapore.

    2. Hoist winch wire rope end point loose on the winch drum fix point. Recommend to redorope reeving and tighten wire rope end fix point.

    3. Control valve block heavily corroded. Minimess fitting heavily corroded, cannot connectpressure gage. Recommend to overhaul control valve block.

    4. Hoist upper limit switch not working, need to troubleshoot.5. Hook block heavily corroded. Need to de-rust and paint.6. Multiple slew bearing bolt and nut (outside) heavily corroded. Need to de-rust and

    paint.

    PORT PROVISION CRANE: 1. Hook block heavily corroded. Need to de-rust and paint.2. Hook swivel bearing worn out. Recommend to change bearing.3. Minimess fitting heavily corroded, cannot connect pressure gage. Control valve lever

    handle fell off. Recommend to overhaul control valve block.4. Hoist up function very slow. Possible hoist upper limit switch valve always activated.

    Suspect hoist limit switch valve have problem. Need to overhaul hoist limit switchvalve.

    5. Hydraulic tank breather heavily corroded. Recommend to change.6. Luff load holding valve leaking. Recommend to overhaul.7. Hoist winch drum wire rope guard roller seized and spring action not working.

    Recommend to overhaul.

    PUMP ROOM DAVIT: 1. Manual slew function not working. Can do slew function by pushing the davit arm.

    Recommend to overhaul slew gearbox.2. Recommend to change hoist gearbox oil.

    ENGINE ROOM CRANE: 1. Hoist wire rope grease not sufficient, recommend to apply more grease.2. Traveling gear track dry. Recomment to apply grease.
