Description:

On 24-May-2014 at 08.30 am, While daily site inspection it is foundthat the asphalt cutter was found using without fire extinguisher near

by or at the easy reach.

Immediate Cause

1. Unaware about the necessity of fire extinguisher.

2. Lack of Knowledge about the spark generation potential

3. Lack of supervision

4. Lack of HIRA communication by the supervisor

Corrective Action

1. DCP fire extinguisher wereplaced immediately at thepremises

2. Special In-House training given

to the operators Regarding theSafe use of Equipment

3. HIRA communication was doneby the HSEO.

4. Safety Inspection should becarried out at regular intervals

Description:

On 16-May-2014 at around 03.00 pm, during carpentry work ,it was found that

one of the worker was using with a defective hammer which had a small crackon the wooden handle which can result into breakage .

Immediate Cause

1. Carelessness of the worker and site supervisor

2. Hand tools were not inspected prior to work.3. Poor supervision

Corrective Action

1. The damaged tools wereimmediately removed from thesite

2. Supervisor and the workers werestrictly instructed to do dailyinspection on the tools they areusing

3. Tool Box Talk given on this topic

4. All workers should be aware of allhazards related to usage ofdefective tools
