California Community Choices Grant

California Health and Human Services (CHHS) requested proposals for their California Community Choices Grant project.

In 2008, CHHS awarded the California Community Choices Grant to CalOptima, in partnership with the County of Orange Office on Aging, and the Dayle McIntosh Center.

The purpose of the grant is to develop a long-term care infrastructure to increase access to home and community-based services and to help divert persons with disabilities and older adults from unnecessary institutionalization.
