I am very happy that all my action plans have kicked off very well. My dream project for this year is the Family Benefit Scheme (FBS) of IAP. More than 300 members have joined this Program. I request all the members of IAP to join this scheme and make it a grand success.

Regarding my programs for pediatric education, all my programs are going on very well. We conducted three Intensive Clinical Training Programs for postgraduates at IMCH Calicut, ICH Chennai and D.Y.Patil Medical College, Pune. All these programs were well attended by postgraduates. The workshops on 'Arts and Science of Paper Writing' was launched at Kottayam on April 28th and 29th under the able guidance of Dr. Piyush Gupta and Dr. Panna Choudhury. The second workshop will be conducted at Apollo Hospital, New Delhi on August 21st. The first Faculty Training Program will be launched at Mangalore on 20th and 21st of August. I thank Dr. Santosh Soans, Professor and HOD of Pediatrics, A. J. Shetty Medical College for the same.

Regarding Asthma Training Module, we conducted the first national level TOT on ATM on the 7th and 8th of May at Bangalore and trained more than 100 pediatricians and we are planning to conduct 35 workshops on ATM throughout the country. The Allergic Rhinitis Training Module is ready and we have conducted TOTS at Jaipur along with Pedicon-2011, at Hyderabad, Chennai, and Mumbai. We will be having TOTs at Delhi and Kolkata soon. Workshops on Newborn Hearing Screening Program was conducted at Thane, Pune, Kochi, Trichur, Palghat and Muscat by Dr. Abraham K. Paul. The workshops on Poor Scholastic Performance was launched at Bareilly and will be followed by four workshops at Kochi, Delhi and Kolkata.

From the Desk of Dr. Rohit C. Agrawal, IAP President 2012

Dear Fellow Academicians!

Seasion’s Greetings from President Elect!

It is indeed a great pleasure and honour for me as well, to

reach you all through this message in Academy Today. I

once again thank you one and all for getting me elected as

President Elect and giving me a wonderful opportunity to

serve our beloved Academy, the fellow pediatricians and

the children in general of the country.

The year 2011 as usual ushered with a big bang in the form

of an extravagant Pedicon 2011 at Jaipur. It is almost mid

way through, with a plethora of scientific activities, some

old revised and some new programs under Dr. Sukumaran’s

Presidential Action Plan which are becoming over popular

throughout the country.

I am grateful to the Executive Board for appointing me as

Chairperson of IAP Committee on Immunization (COI). The

COI team’s main focus would be to come out with evidence

based recommendations and develop indigenous diseases

surveillance data for VPDs, so that rational immunization

practices can be adopted, which is probably the only

modality to reduce child mortality should we strive to

achieve MDG-4 by the end of 2015.

Amongst many publications, 5th Edition of IAP Textbook

of Pediatrics and an innovative “Color Atlas of Pediatrics”

covering almost all the systems of pediatrics would be a

visual feast of International standards and should be the

land mark publications of 2012.

Preparation for Pedicon 2012 are getting hot; in their pursuit

to give “best of the best conference” but something different

by the local organizing team in the beginning of the “Golden

Year” of IAP which should be remembered for years to come

in true sense. I admire the dedication of the organizing

team under the dynamic leadership of young and versatile

Organizing Secretary, Dr. Mahaveer P. Jain.

As I promised you earlier, many new projects and modules

are already under development and should be released in

2012, which will be mainly focused on office practice for a

common pediatrician. Our association with AAP and RCPCH

would continue in the coming years.

I pray to almighty to give me strength to serve IAP selflessly

to achieve my goals mainly “Reduction of Child Mortality”,

“Reaching every pediatrician through academics” and

“strengthening IAP at government, national and international

arena”.

Jai IAP Jai Hind

Dear Fellow Academicians,

It gives me great pleasure to interact with you through this

Election issue of IAP bulletin. Activities under IAP Action

Plan 2010 still remain to be completed. I am happy to note

that members and branches are conducting those programs

alongside IAP Action Plan 2011 with sincere efforts. I

thank each and every member for helping me carrying out

activities planned during my tenure.

I have discharged my duties with the best of my competence

and will continue to work with same dedication for the

Academy. I wish my successors the very best in all

endeavors for better child health care and to take the

Academy to greater heights.

I wish all the best to all the contestants for the various posts

for which election are underway. I request all the eligible

voters first to cast their valuable vote (which is their right

as well as their duty) and to cast it in favor of the candidate

Dear friends,

It gives me great pleasure to write this message for the 2nd

issue of Academy Today for the year 2011.

The activities of the IAP under the Presidential Action plan

have moved very well in this year under the stewardship

of Prof. Sukumaran. Personally, I am involved in the

Asthma Training Module for which an comprehensive TOT

was organized at Bangalore. The new module has been

excellently drafted and is consistent with international

guidelines.

The second meeting of the office bearers and members of

the executive board took place in Cochin in June this year

and many innovative ideas were discussed. The summer

CME organized concurrently at Cochin was of a very high

academic standard and reflected the great talent among

members of our IAP.

I am delighted to note the style and content of our official

journal ‘Indian Pediatrics’. I must congratulate Dr. Piyush

Gupta for his untiring efforts which have taken our journal

to international heights.

Friends, IAP elections are round the corner. May I request

all of you to please actively participate in these elections

and cast your valuable vote.

Thanking you,

Dr. R. Dayal

IAP Committee on Immunization to expand its scope of activities

IAPCOI to expand its scope of activities for more comprehensive Infectious Diseases control

Many far-reaching decisions were taken at the first meeting of new team of IAPCOI held on July 16th 2011 at IAP office in Mumbai. They include the following:

1. Evidence based recommendations: In its efforts to develop a uniform approach of making explicit the evidence base for COI recommendations, a sub-committee is formed to devise and develop a model of grading system to grade available evidence to issue best evidence based recommendations. So all IAPCOI recommendations now will be issued after analyzing available strength of evidence for safety and efficacy of a particular vaccine. Even recommendations will be graded in to 2 or 3 categories on the basis of strength of evidence. This approach would ensure transparency and impart a scientific methodology behind recommendations.

2. COI VPD Surveillance initiative: In order to expand and energize the already launched innovative IDSurve Surveillance project (of IAPCOI and IAP Kutch branch), it was decided to form a national COI Surveillance Committee with the aim of having at least 2-3 reporting units in every city and district. COI will also explore other avenues to improve surveillance of vaccine-preventable diseases in the country. It was resolved to initiate new collaborative projects with other agencies, NGOs and vaccine manufacturers.

3. Immunize India Initiative- IAPCOI will soon launch an immunization reminder service for parents, delivered using Short Message Service (SMS or Text Message) and email all over the country to tackle one of the important barriers to achieve high routine immunization rates in the country.

4. Adverse Events Following Immunization (AEFI) reporting: The committee resolved to initiate its own AEFI reporting system. As a beginning, a trial project will soon be launched in New Delhi.

5. Misinformation against vaccination: The committee entrusted its convener the job of responding to media and other appropriate forum to issue statements on behalf of IAP/IAPCOI to tackle any irresponsible, misleading reports or campaign against immunization in the country. Members are requested to contact COI convener in an event of such an occurrence.

6. Postmarketing surveillance of new vaccines: COI decided to encourage post-marketing surveillance of any new vaccine launched by vaccine manufacturers in the country. It will instruct the company to present data periodically to the committee and also make it public. It will also collaborate with NRA to ensure conduction of these studies by every vaccine manufacturers/promoters in the country.

7. COI has decided to revise IAP Immunization Schedule on yearly basis and the schedule will now thereon be named according to the year it was launched, i.e.” IAP Immunization Schedule 2011”.

8. Many new projects like vaccine congresses, on-line vaccinology courses, position papers on individual vaccines, single theme consultative meetings, improvement of its website with Q &A format for parent education, etc were also planned for future.

9. It was also decided to make all the activities of the committee transparent with all information about the proceedings and recommendations of its meetings will be posted at its website.

Is Giving Sugar to Infants and Children Harmful?

Sugar, like fat, gets a lot flak in the print media, some

deserved, some not. Babies are born with a sweet tooth.

Human milk is quite sweet, so a child begins life making

the connection between eating, drinking, and pleasure.

Sugars are one form of carbohydrates and carbohydrates

are good for you, as long as you eat the right kinds in the

right amounts. Carbohydrates are your body’s main source

of energy. You couldn’t live or work without them. Your body

needs a lot of carbohydrates - around 60 to 70 percent of

your total calories should be in the form of carbohydrates.

But before you reach for the carbohydrates in a chocolate,

take some time to learn about sugar and other kinds of

carbohydrates. Over-processed, factory-made sugars and

starches have given carbohydrates a bad reputation. Once

you understand which carbohydrates are best for your body,

including which sugars are good for you, you can indulge

a sweet tooth and still enjoy sweet health.

Best complex carbohydrates

• legumes: beans, peas, chick peas, lentils

• nut butters - e.g peanut butter

• oatmeal (without added sugar)

• pasta

• soya

• sweet potatoes

• whole grains: whole wheat, brown rice

• whole-grain cereals

Carbohydrates contain carbon atoms that have been

hydrated by adding water molecules. Carbohydrates are

actually built of sugar molecules (saccharides) arranged

like beads on a necklace. Carbohydrates include sugars,

starches, and fiber. Both sugars and starches are broken

down by the body into the simple sugar, glucose. Glucose

molecules then circulate in the bloodstream, supplying cells

with fuel on an as-needed basis. Extra glucose is converted

into glycogen, which is stored in muscles and the liver. If

the body is already storing enough glycogen, glucose gets

changed into fat. Body prefers to burn glucose or glycogen

for energy, but when these reserves are depleted it draws on

fat, the reserve fuel. Carbohydrates are an important part

of the diet, since the body needs energy to grow, to work,

and to repair itself.

Simple carbohydrates are the disaccharides – sucrose

(table sugar - glucose + fructose) and lactose (milk –

glucose + galactose). Simple carbohydrates end in the

suffix ose. A simple carbohydrates containing one sugar

is a monosaccharide - glucose and fructose, the sugar in

fruits. In general, the simpler the sugar, the more sweet

it is. Fructose, the sugar found in fruit and honey, is the

sweetest.

Complex carbohydrates are polysaccharides. They are

made of long, complicated strings of simple sugars, and

there are many different kinds. As a general rule, complex

carbohydrates - what grandmother called "starches" - are

the ones that are the most nutritious, since they are usually

part of foods that contain a variety of other nutrients and not

a lot of fat. Starches, like simple sugars, are broken down

into glucose, but it takes longer to digest most starches, so

they don’t cause blood sugar fluctuations the way simple

sugars do. Fiber is also a complex carbohydrate, but human

intestines do not contain the enzymes necessary to break

down the fiber’s long carbohydrate chains into individual

sugar molecules so that it may be absorbed. Carbohydrates

don’t count as calories in the diet unless they are burned

for energy, so fiber is really a calorie-free food. Fiber in

food slows the digestion of other carbohydrates, especially

soluble fibers (citrus fruits, oats, and legumes). The extra

3435 - IAP Bulletin Academy Today Issue_July-Sept 11.indd 28 8/6/2011 3:22:58 PM

29IAP BULLETIN Volume 7 Issue 2 Jul-Sept 2011

fiber in whole grains also slows the digestion and absorption

of sugar, which explains why whole grains in cereal are

digested more slowly than high-carbohydrates pasta.

Sweetest sugars

Carbohydrates differ in their degree of sweetness, and

complex carbohydrates, such as starches, while being the

least sweet, are the best for the body. Carbohydrates ranked

from most sweet to least sweet are:

• Honey

• fructose sugar (fruits, fruit concentrate)

• table sugar (sucrose)

• complex carbohydrates (starches)

Complex carbohydrates taste less sweet because,

being a larger molecule; they don’t fit as easily into the

sweet receptors of the taste buds as do the more simple

sugars.

Harmful effects of sugars

The complex carbohydrates found in vegetables, grains,

and fruits are good; the simple sugars found in sodas,

chocolates, icings, and packaged treats can do harm, at

least when eaten in excess. It’s as simple as that. Here’s

why:

Excess sugar depresses immunity. Studies have shown

that downing 75 to 100 grams of a sugar solution (about

20 teaspoons of sugar), can suppress the body’s immune

responses. Simple sugars, including glucose, table sugar,

fructose, and honey caused a fifty- percent drop in the

ability of white blood cells to engulf bacteria. In contrast,

ingesting a complex carbohydrate solution (starch) did not

hamper this property of WBCs. The immune suppression

was most noticeable two hours post-ingestion, but the effect

was evident till five hours after ingestion. This has practical

implications, especially for teens and college students who

tend to overdose on sodas containing caffeine and sugar

while studying for exams or during periods of stress.

Stress also suppresses immunity, so these sugar-users

are setting themselves up to get sick at a time when they

need to be well.

Sugar adversely affects behavior, attention, and learning.

Studies of the effects of sugar on children’s behavior are

as wildly contradictory as a sugar-crazed four-year-old

after a birthday party, but the general consensus is that

some children and adults are sugar-sensitive, meaning their

behavior, attention span, and learning ability deteriorate in

proportion to the amount of junk sugar they consume.

Sugar promotes sugar highs. Some persons are more sugar

sensitive than others, and children may be more sensitive

than adults. This could be related to the fact that the brain

grows rapidly in the preschool years, exaggerating the

effects of sugar on behavior and learning. A study comparing

the sugar response in children and adults showed that the

adrenaline levels in children remained ten times higher than

normal for up to five hours after a test dose of sugar. Studies

have also shown that some children with Attention Deficit

Hyperactivity Disorder (ADHD) react to glucose intolerance

tests with a dip to low blood sugar levels. High adrenaline

levels or low blood sugar levels produce abnormal behavior,

decreased learning performance and more hyperactivity.

Some children are sugar addicts.

Children tagged with the ADHD label are often sugar-

sensitive. There may be several reasons for this. Hyperactive

kids are impulsive and need instant gratification. They

need more energy and they need it with instant effect!

Unable to curb their appetite, they overdose on junk foods.

Some studies of hyperactive children show a higher blood

sugar rise following a high sugar meal than one finds in

3435 - IAP Bulletin Academy Today Issue_July-Sept 11.indd 29 8/6/2011 3:22:58 PM

30 IAP BULLETIN Volume 7 Issue 2 Jul-Sept 2011

normally active children. Hyperactive children seem to

metabolize sugar differently. In response to a high sugar

meal, hyperactive kids increase their output of cortisol.

While the neurotransmitters in the brains of normally active

children signal the hormones to regulate blood sugar,

brains of hyperactive children do not seem to send the

same signals.

While studies show that activity levels go up in both

hyperactive and normal children on high- sugar diets, the

hyperactive children also become more aggressive. Adding

protein to a high- sugar meal mellows out the behavioral

and learning deterioration - another point in favour of eating

a balanced breakfast.

Sugar promotes cravings. The more sugar you eat, the more

sugar you want. A high sugar meal raises the blood glucose

level, which triggers the outpouring of insulin. This excess

insulin lingers in the system, triggering a craving for more

sugar, thus adding another hill to the roller coaster ride.

Sugar promotes obesity. People tend to eat and drink too

much foods and beverages that are sweetened with refined

sugar. Foods with a high glycemic index stimulate the

production of LPL (lipoprotein lipase), the enzymes that

encourage the body to store food in fat cells. Thus, low

fat diets that contain carbohydrates with a high glycemic

index can actually cause weight gain. It’s much easier to

excessively consume chocolates than fresh peaches or

apples. Healthier sugars usually come with a lot of fiber

that takes up room in the stomach. All those extra calories

have to go somewhere. Your body says, "Ah, extra energy.

I’ll pack that away as fat and save it in case there’s ever

a famine!" Refined starches, such as white flour, white

rice, white pasta, and corn starch are more likely to turn

into body fat than natural starches, such as whole grains

which, because they contain more fiber, are digested more

slowly and raise the blood sugar less drastically. Yes, fat

will make you fat, but so will sugar. Put them together in

soda and chips or high-fat baked foods, and you can expect

to put on some kilos. So, even though fat has gotten the

reputation as an unhealthful food, excess sugars deserve

a similar reputation.

Myth: Eating too much sugar causes diabetes.

Fact: No, it does not. Type 1 diabetes is caused by genetics

and unknown factors that trigger the onset of the disease;

type 2 diabetes is caused by genetics and lifestyle factors.

Being overweight does increase your risk for developing

type 2 diabetes, and a diet high in calories, whether from

sugar or from fat, can contribute to weight gain. If there

is a history of diabetes in the family, eating a healthy meal

plan and regular exercise are recommended to manage

ones weight.

Sugar promotes heart disease.

When bears are storing up body fat for their long winter

hibernation, they consume lots and lots of carbohydrates.

When you eat excess carbohydrates, your body turns

these sugars into fat. The body stores excesses of most

nutrients as a safeguard against starvation. If you eat more

carbohydrates than you can burn off, the excess is stored

as fats. People who eat too much sugar tend to have

higher blood tryglycerides, and this increases the risk of

cardiovascular disease.

Further, eating chewy, sticky, sugary foods may promote

tooth decay.

My consistent participation in IAP activities from grass root level with initial assignment in 1982 at state level to Chairperson of ID chapter 2010, is really a long evolving and fulfilling academic journey. My responsibilities as Chair Person Neonatology chapter/Vice Chairman Adolescent chapter and recently as Chair-Person of ID chapter have given me immense leadership training and team building. My long association in IAPCOI for more than 5 terms have taught me lessons on Immunisation needs of our children.

As CIAP executive member for several terms from 1995 to 2011 I have witnessed vibrant change in the philosophy and perception in the IAP. As the National Vice president IAP 2002 I have effectively discharged my duties and conducted the programmes and chaired many committees of decision taking. My devoted involvement and working for the four National conferences of IAP at Hyderabad from 1982 to 2010 and organising several Chapter conferences, I had gained real meaning of a worker and organiser.

With these well tested blend of abilities as teacher, researcher, organiser, administrator and well imbibed leadership qualities, I am sure i will deliver what IAP needs today.

I shall assure the transparency, direction and accountability in IAP affairs. I shall see that our programmes reach the peripheries where every member feels par ticipated. I will work with seniors and young members to give them opportunities for leadership and as Faculty, so Friends give me an opportunity and we shall make the difference and this is my Promise.

Aspirant for the Post of Secretary General IAP 2012-13

• DR SAILESH G GUPTA (MAHARASHTRA)

Dear friends,

As the future Hon. Secretary General of IAP, I wish to acquaint you with my background, as you are likely to interact closely with me, in 2012-13. I am an alumnus

of KEM Hospital, Mumbai and live in Malad, a suburb of Mumbai. I practice out of 2 nursing homes, which I solely own. My first administrative-academic association began with the Maharashtra Chapter of BPNI, as Hon. Secretary from 1999 to 2004. I worked hard to expand the objectives and activities of BPNI, making it an important partner with UNICEF and the Govt of Maharashtra in promotion, protection, and support of optimal IYCF. In 2003, I launched the inaugural conference of BPNI Maharashtra, the ‘IFCON’. IAP Mumbai invited me to take the Hon. Secretary’s position in 2001-2002, during which 2 years, the organization won the best branch awards in all categories, conducted a record breaking number of activities, and launched a quarterly magazine called ‘Impressions’, in addition of reaching a monthly bulletin to all its members. I was elected unopposed as EB member in 2001 and 2002. In the following 2 years I administered the IAP Respiratory Chapter as Hon. Secretary and in that period, participated in the creation of the very popular ‘Asthma Training Module’ of IAP, and organized the ‘Respicon 2003’. I also edited the quarterly ‘Pulmonology Update’. I have administered the IAP Neonatology Chapter in the capacities of Treasurer, Hon Secretary and Chairperson. I organized the inaugural ‘IAP Neocon’, the quarterly bulletin ‘Neospeak’, and the ‘fellowship program’ of the chapter during my tenures. I was the science coordinator of the CIAP President’s vision program (2007) called ‘Ar t and Science of Pediatric Practice’ and the National coordinator of the ‘Comprehensive Newborn Care’ module. I have also held various key positions in the IAP NRP FGM Project. As Treasurer of CIAP, in 2010-11, I streamlined the accounting system of IAP, held 3 meetings of the Finance committee, incorporated the system of transferring a part of Pedicon registration fee toward CIAP Corpus, and assisted the Hon Secretary and Presidents in all IAP Action Plans. As Hon. Secretary General, I envision myself as an impartial and apolitical administrator who will smarten up the functioning of CIAPO, improve the CIAP’s relations with various IAP limbs, branches and chapters, and pay heed to the sentiment of every member of IAP. In all my endeavors, I see your unstinted and kind support.

• DR N RAVI KUMAR

Dear Pediatrician

Evolution says it’s not all doom and gloom ahead and hence lets all climb the optimum fuelled bus to enrich the future. As a contestant for the central Executive member I reach you for your support and wishes which had been elemental in adding strengths to my innate desire to enliven the activities of IAP.As a member, an Executive member, TCB secretary, National Executive member, and presently as TCB president I contributed in my own way in placing the Twin city branch and our state branch in the national trajectory.

We all moved in unison to propel the academy to broader and brighter spheres. Being with the Academy for over two decades I am contentious about the accomplishment we had in academic, social, preventive and various subchapters of pediatrics. However the evolving scientific scenario calls for greater participation to reach the millennium development goals for which India had been a signatory.

I seek your guidance, help and support for this vision that possibly we all share to build a healthy nation

Please tender your valuable vote in my favor and enable me to render service as a central Executive member from our state.

Yours Dr. Ravi Kumar. N.

• DR NEELI RAMCHANDER

It has been a gentleman’s agreement in A.P to field two candidates (for elections to EBCIAP) from Twincities branch and one candidate from periphery on rotation. This time, Telangana gets the turn and 2012would be of Rayalaseema but unfortunately one candidate from Rayalaseema has entered the fray in haste. Efforts by the Leaders of A.P from all regions to avoid this have gone in vain.

I did justice in my last term as EB member in 2009 wherein I have taken up the cause of my colleagues from the state and was the only member who spoke vehemently for Dr.Binayak sen’s Innocence which was upheld by Supreme Court.

I led my district to Best Branch award at state and national level and organized one of Best state conferences. I appeal to Honorable members from A.P for another chance so as to pressurize the CIAP to take into confidence the entire District Presidents and Secretaries of the nation in formulating and implementing programmes aimed to strengthen the IAP. After all I am the only candidate in the fray from Non- teaching side and I have no personal ambition other than my own state.

MAHARASHTRA

• DR CHANDRASHEKHAR S DABHADKAR

I don’t want to blow my trumpet, but can humbly say, I’m very happy and satisfied with my contribution to childhealth upliftment, especially in the field of social/community pediatrics. I could achieve this without holding office of CIAP only because of constant help and encouragement from friends and well Wishers.

In journey of my work as a pediatrician & IAPmember, I realized poor representation of pediatricians from interior area of Maharashtra on apex body of pediatricians! Advocacy, planning and implementation of various child health programs can be improved once the voice of members from rural areas is heard and they are involved in decision making in the apex executive body.

Having worked with limited resources and studied the problems surrounding the child health for several years, I now wish to be representing you in the CIAP.

The health problems are very many in rural area. But even a small beginning may lead to substantial results in future. As Thomas Dooley said

“It is the beginning,

And if you light a candle in dark place,

It shines a long way”

May I request you to cast your vote in my favor and elect me as representative from the Maharashtra.
