The Fat Fighting JabA new anti-obesity jab has been hailed by researchers as being almost twice as effective as current fat loss pills in the battle of the bulge.

The new anti-fat jab, called liraglutide, can reportedly help overweight people to lose a stone in 5 months. The drug also cut blood pressure and improved glucose control in those with pre-diabetes – a condition which can lead to full-blown diabetes.

Liraglutide, also known by its brand name Victoza, has been licensed for use on obese patients by prescription in Britain since July. For the latest study, 564 patients were either given daily doses of liraglutide, a ‘dummy’ jab administered once a day, or orlistat pills – another fat-loss treatment.

All the volunteers, who were clinically obese or severely obese, also followed a calorie-restricted diet and increased their physical activity. However, it was the group given liraglutide who lost significantly more weight than those on the placebo and orlistat. Almost 75% lost more than 5 per cent of their body weight on liraglutide, compared with 30% on the placebo and 44% of those taking orlistat.

At the start of the study, carried out by Professor Arne Astrup and colleagues from the University of Copenhagen, around a third of patients in each group had prediabetes but using the drug cut this condition by at least 84 per cent.

“The new varicose vein procedure is called Endovenous Laser Treatment, or EVLT. It is a minimally invasive method, which is performed under local anaesthetic and is completed in about 45 minutes. This means that patients can return to normal activity almost straight away, and experience far less comfort and disruption, than with more traditional treatment methods”

Choosing the right practitioner is imperative when having a

Non-surgical or Surgical procedure.

Whether it’s “Botox”, “Dermal Fillers”, “Breast Enlargement” or a “Facelift” you are considering it’s highly recommended that you do your homework.

We have put together a few pointers to help and ensure you

are making the right decision.

• Ensure that your practitioner has the correct qualifications for the treatment being carried out. Check that the nurse, doctor or surgeon has been appropriately trained and is a current member of their governing body whether it be NMC/RCN for nurses or GMC for doctors.

• Make sure that the practitioner has the correct insurance to cover you and them if in the unlikely event, anything was to go wrong.

• Find out how long they have been performing the treatment, and how many they do a day/week/month.

• Be wary of any medical professional that doesn’t provide a full consultation and doesn’t offer you a consent form. You should make your practitioner aware of medications you are taking, or any recent surgeries or medical appointments.

• Ask to see before and after photos of treatments that the practitioner has performed.

• Confirm the price before proceeding with treatment and make sure that the price is in line with what you expect, too expensive and too cheap are to be thought about very carefully.

• Personal recommendation is the best form of advertising, so do listen to advice. But remember what is right for your friends and/or family, may not be right for you.

‘Botox®’, ‘Vistabel® ’, Dysport®’, ‘Xeomin®’, ‘Azzalure®’, ‘Myobloc®’.’Neurobloc ®’

‘Botox’ is a term that most people associate with words like freeze or paralyse, which I’m sure you would agree are scary words to associate with a cosmetic treatment used to improve the way we look!

The first thing to point out is ‘Botox’ is a trade name and there are many others you may hear of, Vistabel®, Dysport®, Xeomin®, Azzalure®,Myobloc®, Neurobloc®. Ultimately they all work in similar ways but patients, and practitioners, will still favour one over another.

It is important that your practitioner tells you which

one you are having as results may vary slightly.

The medical term for what’s now known commonly as Botox is Botulinum Toxin. This is a purified protein; it has been diluted many times so that the part you receive for treatment is not toxic to your body. It is administered in such minute quantities that your body will break it down very easily after it has had its desired effect.

Botulinum Toxin Type A (Botox) has been used in the NHS for the last 60yrs, and when doctors started using Botox in the 1990’s they started to understand its effectiveness in patients with overactive muscles and through intensive research found it to be highly effective in the treatment of children with cerebral palsy.

The cosmetic use of Botulinum Toxin was first discovered in 1987 when Dr. Jean Carruthers, a Canadian Ophthalmologist, an eye specialist, noticed that after she used the drug to treat eye muscle spasms, the lines around the eyes (known as crow’s feet) of her patients improved dramatically. When she mentioned this side effect to her husband, Dermatologist Alastair Carruthers, he decided to try it out on his receptionist. And that was how we discovered that the drug now known as Botox can reverse some of the signs of ageing on the face.

How does it work? Botulinum Toxin works by blocking the chemical (acetylcholine) message that the brain sends the muscle when telling it to contract. As

It is very hard to predict exactly how Botulinum Toxin injections will affect your muscles. You may therefore find that the results are not always what you intended. Very occasionally patients who receive Botulinum Toxin injections in the forehead experience a drooping (ptosis) of the eyelids or eyebrows, although this is temporary and may last a few weeks, it will almost certainly resolve when the effects of the treatment wear

off. Rarely double vision or blurred vision can occur and you should seek medical attention if this happens to you. The risk of side effects following treatment is reduced by going to an experienced practitioner.

One final thought Remember, you don’t always need to take 10yrs off your appearance; the aim is to look good for your age, and to look as natural as possible.

Most people can expect to pay around;• £150-£250 for ONE area eg,

crow’s feet or fore head or in between the eyes (glabellar)

• £200-£350 for TWO areas, which can be any combination of the three main areas for treatment.

• £250-£500 for THREE areas.

NB Extra areas like jaw line, chin or Neck treatments are at the discretion of the Practitioner/Dr and usually carry an extra charge at around £50-£100 per area.

Things to consider following a treatment• After your treatment there may

be some redness, swelling or tenderness at the injection site. Bruising can occur around the treated area. This is usually short term and will subside over the next few days.

• Avoid wearing make up post treatment for 12 hours.

• To avoid infection avoid touching the injection site and don’t massage unless instructed to do so.

• Until the initial swelling or redness has resolved, do not use sun beds, sauna, steam rooms and avoid extreme temperatures.

• If taking aspirin or similar

medication this could increase the risk of bruising and discoloration to the skin where you were injected.

• After your line relaxing treatment please refrain from drinking excessive amounts of alcohol, taking vigorous exercise or leaning the head forward for the first 6 hours after treatment and avoid sleeping on your front for the first 2 nights.

• To gain optimum results of your line relaxing treatment exercise the treated area by frowning and rising your eyebrows for a few hours after treatment

• Most people are pleasantly surprised at to how painless the treatment is. For most people they compare it to a slight scratch

• The fact that 90% of people who try this treatment repeat it is its own recommendation

• There are a lot of extremely good practitioners/Dr’s and clinics, just take time to find out which one is right for you.

• You must be able to trust the person performing the treatment. If you feel unsure at any time during consultation, don’t be embarrassed to say you may just think about treatment this time.

• Always fill out a consent form during a consultation.

How much does it cost?

the muscle is in a constant state of contraction, when it is relaxed by the treatment it is lifted achieving tighter, smoother and much softer appearance of fine lines and wrinkles.

That process usually takes around 10 days to take full effect. The effects of Botulinum Toxin injections generally last for about three to four months, although the results can last longer with repeated treatments. Botulinum Toxin may be less effective for people aged 60 and over because the muscles under the skin are less firm. There are three main areas for treatment on the face which are in between the eyes (Glabellar), crow’s feet (either side of the eyes) and forehead. Areas such as the lower jaw, chin and neck chords (rhytides) may be treated by more experienced practitioners.

Patient Profile – Meet Jane Clarke from Suffolk

Here is how he does it…… Surgical procedure with Professor Laurence Kirwan:Breast reduction using the inferior pedicle procedure – which means that the connection of the nipple to the breast is maintained to preserve sensation in the nipple. Kirwan used his anchor scar technique. This involves a scar around the nipple with a vertical scar from the areola to the crease under the breast, plus a horizontal scar in the crease itself.

Cost: £6-8,000 for Prof Kirwan’s fee. Additional costs of about £2,500 for the hospital charges and anaesthetist’s fee.

Duration of surgery: almost 4 hours. There was no necessity to have the stitches removed as they absorb by themselves. The patient returned to her normal life within 10 days. She runs a hat hire business, breeds Boxer dogs and has five grandchildren.

Primary factors contributing to wrinkled, spotted skin include normal aging, exposure to the sun (photoaging), and loss of subcutaneous support (fatty tissue between your skin and muscle). Other factors that contribute to aging of the skin include stress, gravity, daily facial movement, obesity, and even sleep position.

Skin Changes That Come With Age:• Skin becomes slack. The loss

of the elastic tissue (elastin) in the skin with age causes the skin to hang loosely.

• Skin becomes more transparent as we age. This is caused by thinning of the epidermis (surface layer of the skin).

• Skin becomes more fragile as we age. This is caused by a flattening of the area where the epidermis and dermis (layer of skin under

the epidermis) come together.

• Skin becomes more easily bruised. This is due to thinner blood vessel walls as we age.

Sun and Your SkinOver time, the sun’s ultraviolet (UV) light damages certain fibres in the skin called elastin. The breakdown of elastin fibres causes the skin to sag, stretch, and lose its ability to snap back after stretching. The skin also bruises and tears more easily and takes longer to heal. So while sun damage may not show when you’re young, it will later in life.Nothing can completely undo sun damage, although the skin can sometimes repair itself. So, it’s never too late to begin protecting yourself from sun exposure and skin cancer. You can delay changes associated with aging by staying out of the sun.

Changes Below the Skin• Loss of fat below the skin

in the cheeks, temples, chin, nose, and eye area may result in loosening skin, sunken eyes, and a “skeletal” appearance.

• Cartilage loss in the nose causes drooping of the nasal tip and accentuation of the bony structures in the nose.

Other Skin ChangesGravity, facial movement, and sleep position are the secondary factors that contribute to changes in the skin. When the skin loses its elasticity, gravity causes drooping of the eyebrows and eyelids, looseness and fullness under the cheeks and jaw (jowls and “double chin”), and longer ear lobes.Facial movement lines become more visible after the skin starts losing its elasticity (usually as people reach their 30s and 40s).

1. THE SKIN AGEING PROCESSThe processes by which the skin ages are complex, resulting from a number of different internal (genetic) and external (environmental) factors. Our internal aging is determined by our genetic heritage and there is little which can be done to change the genes handed down to us by our parents. However external aging factors mostly relate to largely environmental influences of which exposure to ultra violet radiation (UVA and UVB radiation from the sun) seems to have the most detrimental effect, resulting in wrinkling, sagging, loss of elasticity and a dehydrated skin appearance.

2. PHOTOAGEING AND SKIN DEGENERATIONWhat is photoageing? Photoageing is the premature ageing of the skin caused by sun damage as a result of repeated exposure to sunlight.

The sun’s ultraviolet A (UVA) and ultraviolet B (UVB) radiation that manage to break through the earth’s atmospheric ozone layer causes skin to age. UVB radiation increases skin cancer. UVA radiation causes premature skin ageing by directly breaking down the skin’s supporting structure of elastin and collagen. Healthy well supported skin has resilience and elasticity which allows movement as well as the ability to spring back to place. Photo ageing impairs the growth of healthy skin and destroys the skin’s elasticity.

Photoageing and the Skin Degeneration CycleThe combination of UVA and UVB radiation from the sun breaks down skin’s collagen and elastin. This process alters the distribution of natural hyaluronic acid (HA) in the skin and impairs the water binding properties of the skin. As a result the skin loses its ability to hydrate and it loses structural support. This loss in structure impairs the flow of nutrients to the skin cells which restricts the skin’s fibroblast activity which produces collagen, elastin and hyaluronic acid. This reduction in collagen and elastin causes the dermis of the skin to collapse and lines and wrinkles to form.

What is hyaluronic acid?Hyaluronic acid (HA) is a substance found naturally in our bodies, which binds water and hydrates the skin. HA is a polysaccharide (a sugar molecule) and is one of nature’s most versatile and fascinating molecules. As we age HA functions become impaired and our skin loses its natural elasticity and youthful volume as a result.

What are the functions of hyaluronic acid in the skin? Some of the functions of hyaluronic acid are to:

• Create volume and tone in the tissue by attracting water

• Influence the function of other skin constituents, such as collagen and elastin

• Cushion and lubricate tissue

• Transport essential nutrients from the bloodstream to skin cells

Plastic surgery is not something to be

entered into lightly, and your results could be

determined, in part, by the information you have at the very beginning as well as by your Surgeon

understanding you, what you are hoping to achieve and your

lifestyle.

Do your homework beforehand, and don’t be afraid to ask as many questions as you need. If you feel your Surgeon resents your inquisitiveness, or if you feel pushed into surgery, find someone else. Listen to your “gut reaction” and choose the Company/Surgeon you feel most comfortable with – go to several different Surgeons and ask the same questions and only choose the one you feel the most comfortable with, not just because they are the cheapest. After all, this is all about you and you will be the person living with the results! These are some of the questions you should ask before going ahead with any surgery, whether at home or abroad:

Choosing the Company/Surgeon• Is the company based in your

country? If not:- do they offer consultations

before surgery and follow up appointments in your country?

- what will happen if something goes wrong after surgery? Will they pay for you to fly back to the clinic/pay your hotel stay etc.,?

- do the surgeons and other medical staff speak your language?

- how long after surgery can you fly home? Patients should always stay in the country they have their surgery for AT LEAST 1 week post-surgery so that they can have their first follow up

appointment with the Surgeon. In addition, most incidences of infection occur within the first week, so you should be near the clinic should this occur. Also, the chances of getting a DVT (deep vein thrombosis) are increased when you have surgery, so you should not fly for at least 1 week afterwards.

• NEVER choose a company that does not offer a consultation prior to surgery. Companies who do the consultation on the day of surgery should be avoided at all costs.

• NEVER choose a company that offers consultations and/or follow ups on webcam.

• Expect to fill out a medical history questionnaire – the more information the Surgeon/medical staff have the better. If you aren’t asked even basic medical questions, choose another company.

• If the cost of surgery sounds too inexpensive, then the level of service and care you can expect is likely to be very basic, or non-existent.

• Beware of very cheap deals or companies that push you to have more than the procedure you are interested in.

• When you go for your consultation, who will you be meeting – a Sales Person/Co-ordinator/Nurse or the Surgeon? If you are not going to meet the Surgeon, don’t go along!

HyperhidrosisFirstly avoid soaps that irritate the skin. Use a moisturiser, such as aqueous cream, as a soap substitute which avoids unnecessary damage to the skin and is readily available from your pharmacist. I know it’s easier said than done but try to avoid things that you know make your sweating worse like stressful situations, getting hot and eating salty or spicy food. Wear clothes that are loose under the armpit and try to avoid man-made fibres such as nylon and lycra which are notorious for making you sweat more. Some clothing colours show up sweat marks more than others and I find that as a rule black or white clothes are best. You can even buy disposable underarm sweat

pads (Sweatex®) which are easily obtained on the internet and can be very helpful.

I suppose it’s too obvious to say use your antiperspirant regularly and not just rely on deodorants to stop you smelling. In fact fresh sweat doesn’t smell it’s only when bacteria build up in sweaty clothes that haven’t been changed and washed regularly that you start to smell. If your usual toiletries don’t help try the strong antiperspirant Ammonium chloride (Driclor®) which is also available as a roll-on from your local pharmacist. Driclor® works by blocking the sweat pores but can cause itching and skin irritation so please take expert advice on its use.

Botulinum toxin injections work well for sweaty armpits. The treatment is relatively painless, quick and simple to perform. You will need a number of superficial injections into both armpits and you usually see the benefits within 3-5 days. A recent large European clinical trial showed that well over 90% of patients responded well to the treatment. Some patients went 16 months between treatments though the average time was 7 months. During that time armpit sweat levels were reduced to virtually zero. The treatment was also shown to be extremely safe with no serious side effects. A small number of patients (less than 5%) got a mild flu-like illness

for a day or so. Your aesthetic practitioner will be able to give you further information on this and be able to answer any more questions you may have.

In severe cases that do not respond adequately to any of the above then surgery may be an option. This involves removing the sweat glands from the armpit and though very successful is not without significant risks. You would need to be referred to a surgeon who specialises in this procedure.

Maximising combination of intensive active ingredients, it is designed to have a deep moisturising effect, helping to plump and fill out wrinkles and lines from the inside. In this product, the microspheres of hyaluronic acid are combined with matrikines (which stimulate the neosynthesis of collagen, fibronectin and hyaluronic acid); Argireline (which acts on superficial wrinkles such as forehead lines and crow’s feet) and Vitamin C (which helps protect cells’ lipid structure and maintain hyaluronic acid integrity).
