A ONE-TIME OFFER.If you owe past occupational license taxes to the former City of Louisville or Jeerson County (currently Louisville Metro), you have just 65 days to pay up without interest, penalty or fear of
prosecution. Take advantage of this opportunity to come clean. This is the only time we will oer tax amnesty.
WHAT EXACTLY IS TAX AMNESTY?Occupational license taxes are fees imposed on employee wages and business net prots that are due each year from anyone working or doing business in Louisville Metro. This includes employees whose employers failed to withhold local license taxes from their paychecks and employers who withheld, but failed to remit the
employees local taxes. Also included are people who are self-employed, those who work as consultants and freelancers, and those who operate businesses on the side. Non-residents and businesses outside the city that perform work in the Louisville Metro area are also responsible for occupational license taxes.
Tax amnesty is an opportunity for delinquent taxpayers such as these to le and pay their occupational license tax liability for tax periods ending on or before December 31, 2003. In exchange for the full occupational license tax payment, we will waive any penalties and/or interest for taxes due from previous tax periods.
WHO QUALIFIES?There are three kinds of taxpayers who qualify:
Taxpayers who did not f ile a required return Taxpayers who are amending the tax liability on a previously
f iled tax return Taxpayers who have an outstanding tax liability as of
December 31, 2003
Taxpayers under criminal litigation or a criminal order of restitution are not eligible. To determine whether you are eligible to participate, calculate your liability for back taxes by preparing the appropriate returns and completing the amnesty application form. Amnesty applications and tax forms are available at www.louisvilletaxamnesty.com.
Or Pay the Price. DEADLINE.You must mail or deliver the completed application form and all returns, along with full payment for the occupational license taxes due from previous tax periods, by May 31, 2005.
PENALTIES AND INTEREST.
If you dont take advantage of our tax amnesty program, the Louisville Metro Revenue Commission will track you down. Our enforcement team has been strengthened to support compliance, audit, legal and collection eorts by sharing taxpayer data with the IRS and the Kentucky Revenue Cabinet.
Once a taxpayer is discovered to be in non-compliance, all applicable penalties and interest will be imposed on any tax liabilities due for periods prior to our tax amnesty oer. In addition, we will pursue all administrative, civil and criminal actions necessary to collect the full amount owed.
DO THE RIGHT THING.
Make sure you have paid any past occupational license taxes that you owe to the former City of Louisville or Jeerson County (currently Louisville Metro). If you do owe, come forward during tax amnesty it will end up costing you a lot less if you do.

Ive had the same physicians for over 23 years. Ive joked with Dr. Kanga to say that Im going to live to take care of him as an old man. Anne McMahan, Patient
she first entered the work world as a nurse in the early 1980s. Its where she now works in the UK HealthCare call centers pediatric triage service. Its where her husband works as a pharmacist. And its where, over the last four decades, she has received treatment for cystic fibrosis (CF).
Long-term relationshipsAccording to the National
Institutes of Health, CF is the most common fatal genetic disease in the U.S. It causes the body to produce a thick, sticky mucus that clogs the lungs and leads to infection. It also prevents digestive enzymes produced in the pancreas from reaching the intestines where they are required to digest food.
There is no cure for CF and the present life expectancy for people affected by it is usually their mid-30s. McMahan has beaten the averages. She is 48 years old and going strong.
McMahan credits her longevity and quality of life in large part to the aggressive medical care provided by her UK physicians: Dr. Jamshed Kanga and Dr. Michael Anstead. Ive had the same physicians for over 23 years. Ive joked with Dr. Kanga that Im going to live to take care of him as an old man, she said.
Because she has a less severe form of CF and her obvious symptoms
Since 1989 when the gene that causes cystic fibrosis (CF) was first discovered, the median lifespan for patients with the disease has increased by more than 10 years. And nearly half of that increase has come during just the last five years.
According to Dr. Michael Anstead, director of the adult CF Center, the median survival rate has jumped from 31.6 years in 2002 to 36.8 in 2004up from just 26.5 years in 1989. (When McMahan was born, patients rarely made it to puberty, often succumbing to the disease as young children.)
He attributes the increase in his own patients survival to three things: 1) a more uniform use of aerosolized antibiotics that fight the bacteria that invade the lungs of many patients; 2) use of antibiotics with special properties that inhibit the ability of the bacteria to produce toxins and are anti-inflammatory; and 3) an initiative undertaken by the Cystic Fibrosis Foundation to improve the
quality of care for CF patients.One of the great developments
in Kentucky is that we now do newborn screenings for cystic fibrosis, he said. Through the lobbying efforts of Drs. Anstead and Mabry, Annes father, CF was included in the newborn screening tests implemented by SB 25 passed during the last Kentucky legislative session. This screening can identify 99 percent of babies born with CF. If those patients can be identified and monitored through each lung infection, the bacteria might be prevented from growing in their lungs. Once it takes hold, it is extremely difficult to dislodge.
In Denmark, only 20 percent of teenagers affected with CF are infected with this bacteria, he said. In the U.S., 70 percent are infected with it. Denmarks median lifespan is about 45 years. So were starting to adopt some of the principles theyve learned over time. It will help the CF patients who are children today.
