Do you wish to pursue a professional course to become a classical ballet dancer on the stage?

YES NO

Many injuries can be sustained in the serious pursuit of classical ballet dancing. The New York Ballet Institute assumes NO liability or responsibility for any dancer's injuries either sustained while dancing or as a result of dancing. Beware! This is an intensive program, designed for dancers in excellent health and condition. If a dancer has any previous injuries, they should train wisely. If you have any medical or health conditions which the school should be made aware of please list them below and explain your care and treatment. Dancers are expected to be in good health upon beginning the program, and able to dance freely and for long periods of time every day.

We do recommend that students under the age of 16 be accompanied by a parent. We also know that parents and students having experience as pre-professional or advanced dancers have traveled to Summer Intensives and understand that every effort is always made to insure children's and student's safety and well-being. Students must act responsibly. Students are expected to go to and from the school independently, feed themselves, shop, do laundry, and make mature decisions about their actions while left to their own devices. Arrangements will also be made to make sure students are are getting the things they need, at least on a weekly basis. If the student has any problem, emergency, or issue, they need to inform the school and their parent immediately. Parents should feel free to call the school about any matters relevant to their child. We have rules, which differ for each student based on their living situation and other things. Students are expected to follow this advice and act in a mature fashion. Part of a Summer Intensive anywhere is a certain amount of autonomy expected and learned. We are happy to help with that, but it does not take the place of the student/parent relationship. If students are behaving willfully, staying out past a curfew, or doing other things which distract from the nature of the program, interfere or threaten other people, or do harm, criminal mischief or injury, they will be expelled. Drugs and alcohol, including cigarettes, are prohibited, in the school and any residences. Any students found to be be breaking these rules will be sent home immediately. Adults in the program must show every courtesy and respect to other students who may not be adults, and we ask that they follow these rules, too, particularly on the premises of the school and in the housing.

Parents and GuardiansIf you are entering the program as a minor, then it is required that parents read and sign everything to do with the application and registration process. They are responsible for you and you are responsible to the school and for your classes. Please make sure that parents read and sign all materials as required and fully understand the course on which you are embarking, and agree to the terms, risks posed, and the results you desire. We ask that parents provide an email, and emergency contact information THIS IS NOT THE STUDENT'S RESPONSIBILITY, so they may be kept aware on each item or event as necessary. Permissions to participate in the activities and performances, travel and arrangements are agreed to and accepted by this signed application and parents should arrange all personal travel, housing and other details themselves as the school and the students will be too busy. All parents of students 18 and under are required to agree to these terms.

Is the student over the age of 18?

YES NO

Do you accept these responsibilities for your child?

Choreography and the teacher observation course fees are listed on the Tuition page. Costs for those programs are subject to deposit. Please calculate tuition and course fees together before calculating deposit amount, then add Registration and Final Performance Fees to the total. No refunds of deposits will be made without the approval of the Administrator, and are on a case-by-case basis. Money awarded is to offset, not replace, other costs and expenses, such as housing and food. Please contact the school with any questions about this program.
