Operation Unit : Oil & Gas RefineryDate:Occurrence : Explosion of Oil Vessel Separator at HCC caused by Over Pressure Time: I. Event ScenarioHCC Fractination Unit operated normally but the suddenly surprised by hardly explosion sound from Oil Vessel Separator 211 V102 (vertical Drum) followed by fire. Oil Separator 211 V102 is a new additional installation for optimalizing release pressure from 211 V08, where within initial operation (Start Up) in 90oC, the PSV Blank of vessel oil separator 211 V102 has not yet removed thus causes uncontrollable Over pressure and at the end vessel explodes and get fire. The emergency is not resulted casualties but the explosion is so hard and making the window glasses of building around 150m break so badly.

Emergency Reported by On Duty Control Room Officer to fire station by Handy Talky and forwarded to the On Scene Commander as well as Incident Commander and by approval of Emergency Commander, Incident Commander declares as emergency conditon by sounding the emergency siren.

All the teams involve within the Emergency Response Organization along with Fire Brigade conducting the task thoroughly and rapidly as meet procedure thus the emergency response accomplish.

2) Evacuation and Security Operation


1Strengthen the control at the Main gate of Refinery towards the enter/exit vehicles by prioritizing the vehicles and officers related with the emergency response and prohibit the unauthorized enter to the scene.Security Team

4) Fire Fighting Operation


1. Fire on Oil Vessel Separator 211 V102 at HCC Fractination caused by over pressure as a result of unremoved PSV blank therefore it explodes and proceeds to fire.

2. After receiving the report of explosion and fire on oil vessel separator 211 V102 at HC Fractination, On Scene Commander and HSE Supervisor at Fire Station immediately dispatches Fire Brigade completed with Foam Crash Tender to the scene.

4. By considering the size and height of fire that can endanger the other installations so the On Scene Commander decided to condut fire fighting by using Foam Monitor/Branch Pipes that installed on Foam Crash Tender.

5. Fire Brigade conduct fire fighting by using Foam Monitor/ Branch Pipes heading to fired Oil Vessel Separator and fire can be extinguished rapidly and followed by cooling until the condition is safe.
