4 Synthes 4.5 mm and 6.5 mm Headless Compression Screws Technique Guide
1Insert screw
Thread the head of the screw into thetip of the compression sleeve. Insert thescrew into the bone using the compres-sion sleeve construct.
2Compress
The tip of the compression sleeve acts as a conventional lag screw head.When the tip of the compression sleeve contacts the bone, the fracturegap is closed and compressed by further turning of the sleeve.
3Countersink
Once the desired degree of compres-sion is reached, hold the compressionsleeve stationary and use the screw-driver to advance the screwhead intothe bone. During countersinking, no additional compression is generated.
Indications
Synthes 4.5 mm and 6.5 mm Headless Compression Screwsare indicated for fracture fixation, reconstruction, osteotomy,and arthrodesis of various bones and bone fragmentsincluding joint fusion (arthrodeses) in the foot and fixation of intra-articular fractures of the humerus, femur and tibia.

Make a stab incision, and advance the sleeve assembly (protection sleeve, drill sleeves and trocar) through the softtissues to the bone.
To reduce fragments, remove the trocar and advance theguide wire through the sleeve assembly into the bone untilthe thread tip is anchored in the far cortex, using image intensification.
Note: Do not forcefully insert the guide wire. This may causeit to bend.

Remove the drill sleeves and slide the narrow end of themeasuring device through the protection sleeve and over the guide wire to the bone.
The measurement on the measuring device shows the depthof the guide wire in the bone in millimeters and indicates theappropriate screw length.
Note: Use the correct length guide wire to ensure correctmeasurement.
Important: If the screw is to be countersunk below the surface, or a large fracture gap needs to be closed, subtractthe appropriate length.

Predrilling makes it substantially easier to insert the screw indense bone.
Insert the drill sleeve in the protection sleeve and slide thecannulated drill bit over the guide wire and predrill to the desired depth, using image intensification if necessary.
Remove the drill sleeve and verify the appropriate drillingdepth with the image intensifier.
Note: Do not drill beyond the tip of the guide wire. Slowlypull the drill bit straight out while running the power toolforward to ensure that the guide wire stays in place.

To facilitate head insertion in dense bone and to prevent thebone from cracking, it is recommended to predrill the nearcortex using the appropriate drill bit.
Slide the drill bit with the protection sleeve over the guidewire and carefully predrill the cortex.
Note: The depth of predrilling depends on the inclination of the guide wire to the bone. It is necessary to predrill onlyto the first marking in cases where angulation is +_90°. An angulation of +_45° requires a deeper predrilling to the second marking.

Slide the compression sleeve handle into the compressionsleeve. Insert the screw assembly over the guide wire. Insertthe screw into the bone by turning the compression sleeveuntil the fracture or osteotomy is closed and compressed.
Optional: Screw insertion can also be performed with theuse of a power drill. Slide the compression sleeve attachmentinto the compression sleeve and connect it to the power drill.Insert the screw into the bone and stop before the compres-sion sleeve touches the bone. Compress manually using thecompression sleeve handle.

In osteoporotic bone, an additional sleeve should be placedover the compression sleeve before compressing the fractureor osteotomy to prevent the compression sleeve from piercingthe cortex.
Note: Verify the correct position of the shaft thread in thedistal fragment, using the image intensifier. If the thread liesover the fracture gap or the osteotomy, the gap cannot becompressed.
Carefully tighten the screw with the compression sleeve.Forceful tightening could cause stripping of the shaft.
If the thread strips, some or all of the compression will belost. If the screw is then countersunk correctly, the thread willregain purchase, reducing the danger of postoperative screwloosening.
If loss of compression makes screw removal necessary, followthe Screw Removal instructions on page 16.

Remove the compression sleeve handle and slide the cannulated screwdriver through the compression sleeve into the recess of the screw.
Countersink the screw by turning the screwdriver clockwisewhile simultaneously holding the compression sleeve stationary.
Verify the screw position with the image intensifier. Ensurethat the screw tip does not penetrate the distal cortex. Remove and dispose of the guide wire.

Color markingsThe color markings on the screwdrivershaft indicate the position of the screw-driver tip and the head thread of the screw in the bone.
■ When the green mark is flush withthe top of the compression sleeve,the screw is fully threaded into the compression sleeve and thescrewdriver tip is seated in the recess of the screw.
■ When the yellow mark is flush withthe top of the compression sleeve,the top of the screw is flush with the bone surface.
Note: If the screw is inserted at an angle, it must be countersunkfurther than the yellow mark so that it does not project from the surface.
■ When the red mark is flush with the top of the compression sleeve,the screw is 2 mm below the bone surface.

For the removal of the 4.5 mm headless compression screws,use a T15 StarDrive screwdriver, and for the 6.5 mm headless compression screws, use a 4.0 mm hexagonalscrewdriver.
If the screw strips, use the following procedure:
Thread the compression sleeve over the screwhead threadand insert the screwdriver through the compression sleeveinto the recess of the screw.
Remove the screw by simultaneously pulling on the compres-sion sleeve and turning both the screwdriver and compressionsleeve counterclockwise.
Note: If necessary, expose the screw recess and part of thehead thread with a hollow reamer or preferred method.
