Prior to being named interim dean of the College of Nursing, I was keenlyaware of the many talents of our students. Since assuming the role, I ameven more aware of their capabilities and motivation to learn. The dedicatedfaculty, staff, preceptors and others who contribute to their education areequally impressive. For these reasons, we are excited about featuring some ofour students who are balancing multiple demands while enrolled in rigorousprograms of study, yet still find time to do even more!
When you read the “balancing act” article, I believe you will be impressed — and perhaps amazed— about the way DNP student Autumn Roque balances her personal and professional life whilecoordinating a commute from her home in northern Virginia to her work in Washington, andclasses five times a semester at the College of Nursing in Lexington! There is no doubt that usinginformation technology for a variety of curriculum strategies and methods eases her study burdensomewhat. Still, it takes resilience, dedication and love for knowledge to maintain oneself whetherin the BSN, DNP or PhD program.
Not long ago, I asked two of our BSN students why it was important for them to seekemployment in a hospital setting while enrolled in school. Kate McNulty said she planned onapplying for a nursing externship during the summer before her last semester in order to haveadditional nursing experience. Similarly, Ryanne Haga spoke about how grateful she was to havethe opportunity to participate in the Norton Extern Program. Ryanne said, “I think this is goingto be an optimal time to use everything I have learned here at the UK College of Nursing andpractice in real time in a very hands-on manner.”
Finally, when you read about the research grants that faculty members have received since thelast issue of Opportunities, you will have a better understanding about why students come toUK for their PhD studies. They get very involved in the research activities of our gifted, talentedfaculty. Our PhD graduates leave here prepared to be scientists with the capability of addressingclinical problems that our DNP graduates will then incorporate into their evidence-based practiceleadership roles.
In sum, the payback of students’ investment of time and resources is certainly well worththe outlay. Thank you for supporting them in their efforts!

Efforts to Maximize Perinatal Outcomesin Women-at-Risk (EMPOWR)Kristin Ashford, women’s health nurse practitioner andassociate professor, received a four-year, $587,038 Start for Mothersand Newborns grant from the Centers for Medicare & Medicaid Services(CMS). The purpose of this award is to test whether or not an enhancedpregnancy centering model will reduce the incidence of adverse perinataloutcomes. EMPOWR will empower women via self-management andpeer support.
A Dietary Antioxidant Interventionfor Secondary Prevention ofCardiovascular DiseaseMartha Biddle, assistant professor, received a University ofKentucky Faculty Research grant for $20,000 to test the effect of a dietaryantioxidant intervention on biomarkers of inflammation in patients whohave experienced myocardial infarction.
Managing Nicotine Withdrawal in Medicaland Surgical PatientsAudrey Darville, assistant professor and certified tobaccotreatment specialist for UK HealthCare, received a two-year,$199,082 investigator-initiated grant from the Pfizer Medical EducationGroup. With this award, Dr. Darville and her team will develop andpromote participation in web-based training modules to provideinformation for managing nicotine withdrawal and tobacco use cessation,provide a discussion forum for providers and clinical experts, and monitorwithdrawal management practices of providers by specialty at UKHealthCare pre- and post-intervention.
FRESH: Dual Home Screening forLung Cancer PreventionEllen Hahn, professor and director, Kentucky Center for Smoke-free Policy and Tobacco Policy Research Program and MarciaA. Dake Professor in Nursing, received a $2,100,351 grant from theNational Institute of Environmental Health Sciences. The goals of thisproject are to test a dual home screening and tailored environmentalfeedback intervention designed to reduce environmental risks for lungcancer, and to identify factors associated with monetary incentives forradon mitigation.
Promoting Responsive Smoke- and Tobacco-free Policy Adoption and ImplementationEllen Hahn, professor and director, Kentucky Center for Smoke-free Policy and Tobacco Policy Research Program and Marcia A.Dake Professor in Nursing, received $50,000 for year one of what isanticipated to be a five-year initiative from the Foundation for a Healthy
Kentucky. The goal of the study is to promote smoke- and tobacco-freepolicy adoption and effective implementation through advocacy, policyanalysis and monitoring, and stakeholder engagement.
Community Partnerships for TobaccoPrevention and CessationEllen Hahn, professor and director, Kentucky Center for Smoke-free Policy and Tobacco Policy Research Program and MarciaA. Dake Professor in Nursing, received $114,263 from the KentuckyHealth Services to monitor progress in reducing tobacco use in Kentuckyand meeting CDC tobacco prevention and cessation goals.
Reducing Health Disparities inAppalachians with Multiple CardiovascularDisease Risk FactorsDebra Moser, professor and Linda C. Gill Chair in Nursing,received a $2,092,473 contract from the Patient Centered OutcomesResearch Institute (PCORI) to determine the effectiveness of a patient-centered CVD risk reduction intervention in a major at-risk populationliving in an environment where CVD risk reduction is difficult.
Early Detection and Prevention ofMild Cognitive Impairment Due toCerebrovascular DiseaseDebra Moser, professor and Linda C. Gill Chair in Nursing, is amultiple principal investigator with Gregory Jicah, associate professor,Department of Neurology and Sanders-Brown Center on Aging, ona $1,859,780 National Institute of Nursing Research funded study todirectly increase the understanding of and impact of the major cause ofcognitive decline in the elderly population.
Aging, Neuromuscular Behavior,and Risk of Occupational Low Back PainDeborah Reed, professor, is collaborating with Babak Bazrgari,assistant professor, College of Engineering, (principal investigator) on agrant funded for $413,233 from the National Institute for OccupationalSafety and Health (NIOSH). The purpose of the project is to explorerelationships between aging and spine biomechanics.
A Model of Decision-Making inRheumatoid ArthritisElizabeth Salt, assistant professor and rheumatology nursepractitioner, received a $225,000 grant from the American College ofRheumatology to empirically evaluate a process of decision-making usedby rheumatoid arthritis patients as they decide to take medications for thisdisease and to determine if medication adherence impacts disease activity.

Kristin AshfordKristin Ashford,PhD, RN, APRN,has been promotedto associate professorwith tenure. Shereceived her PhD innursing from UK
in 2007. She began her research career as anNIH BIRCWH (Building InterdisciplinaryResearch Careers in Women’s Health)Scholar. She currently serves as PI on anNIH COBRE (Centers of BiomedicalResearch Excellence) study in which sheleads a multi-center trial across two states.She is administrator of the Kentucky GIFTS(Giving Infants and Families Tobacco-freeStarts) program, working jointly with theKentucky Department of Public Healthto provide smoking cessation and wellnessservices to prenatal and postpartum womenacross Appalachia. Dr. Ashford was namedthe 2010 Nurse Researcher of the Year bythe Kentucky Nurses Association NursingEducation and Research Cabinet. She alsoreceived the Outstanding Early CareerCollege/University Teacher Award from theKentucky Academy of Science.
Patricia BurkhartPatricia Burkhart,PhD, RN, hasbeen promotedto professor withtenure. She is theassociate dean ofthe undergraduate
program in the College of Nursing, whereshe teaches undergraduate parent-childnursing and mentors undergraduateresearch interns. She co-directs the Deans’Interprofessional Healthcare HonorsColloquium at UK. Dr. Burkhart’s researchis an expansion of her dissertation study,which focused on testing an interventionto improve children’s adherence to asthmaself-management. Her work has beenpublished in several nursing and medicaljournals. Her award-winning presentationshave been given at numerous national and
international conferences. She has receivedresearch funding from the National Instituteof Nursing Research/National Institutesof Health and the National Institute forOccupational Safety and Health. Dr.Burkhart served on the board of directorsof the Lexington-Fayette County HealthDepartment from 2003-2012. In 2008 shereceived the Society of Pediatric NursesExcellence in Nursing Research Award. Shealso received the Excellent UndergraduateResearch Mentor Award at the 2008 UKShowcase of Undergraduate Scholars.
Karen ButlerKaren Butler,DNP, RN, hasbeen promoted toassociate professorwith tenure. Sheearned both herMSN and DNP
degrees from UK, finishing her DNPin 2006. Dr. Butler is a clinical coursecoordinator in the undergraduate programand teaches in the DNP Program as well.In 2006 she joined the Tobacco ResearchPolicy Program as a faculty associate. She haswon awards for her work in both scholarshipand education, including the Louise ZegeerExcellence in Undergraduate TeachingAward in 2008. Dr. Butler has clinical andresearch interests in adult health promotion,including tobacco dependence preventionand treatment. Dr. Butler’s work related tonursing education, leadership, evidence-based nursing practice, college student healthand tobacco has been presented in local,state, national and international venues,and has been published in a variety ofjournals. She is a member of five professionalorganizations and serves on multiplemanuscript review panels.
Claudia DieboldClaudia Diebold,MSN, RN, CNE,retired in June 2013,after teaching inthe undergraduateprogram at thecollege since
1994. She was promoted to senior lecturerin 2009. She was the course coordinator ofProfessional Nursing Care Across the Lifespan,an eight-credit-hour fundamentals course,Career Management, a senior-level coursepreparing students for their first position; andthe NCLEX. She developed the Skills LabInternship Program, an enhancement coursefor undergraduate students. She also servedas the faculty co-coordinator of the NurseResidency Program since 2002, when UK wasone of the original pilot sites. She served on theoriginal curriculum development task force andthe recent curriculum revision task force. Shewas also the site evaluator for the Commissionon Collegiate Nursing Education NurseResidency Accreditation process.
Jan FindlayJan Findlay, PhD,RN, APRN, assistantprofessor, has joinedthe faculty and UKHealthCare in acollaborative positionwith the college and
with UK HealthCare Nursing. In addition toservice and scholarship, Dr. Findlay’s teachingrole will primarily be in the DNP Program, inthe Psychiatric/Mental Health Track BSN-DNP Option. Her clinical practice will focuson evidence-based practice endeavors at EasternState Hospital.
Dr. Findlay is a 2012 graduate of the college’sPhD Program. Her dissertation was titled“Decision-Making Processes and HealthBehaviors among Adults Diagnosed withSchizophrenia.” Her practice has focused onpsychiatric/mental health since 1991. In 2013she completed the post-masters certificate,Psychiatric Mental Health Nurse Practitioner,at Eastern Kentucky University, Richmond,Ky. She earned her BSN at Capstone College ofNursing, University of Alabama, Tuscaloosa,and her MSN at the University of Alabama atBirmingham

Melanie Hardin-PierceMelanie Hardin-Pierce, DNP, RN,APRN-BC, ACNP,has been promotedto associate professorwith tenure. She
has an extensive background in adultcritical care. Her research interests includeimproving outcomes in the mechanicallyventilated adult critically ill patientpopulation, prevention of ventilator-associated pneumonia, and positionaltherapy as an adjunct treatment for acuterespiratory distress syndrome. She haswritten numerous textbook chapters in thearea of high acuity nursing and is a co-editorof an award-winning, nationally recognizedcritical care textbook, High Acuity Nursing(fifth edition). Dr. Hardin-Pierce teaches inthe graduate program, practices as an acutecare nurse practitioner at Baptist HealthLexington and is also involved in researchwith the college’s RICH Heart Group.
Sharon LockSharon Lock,PhD, RN, APRN,associate professorand coordinator forthe Primary CareTrack in the DNPProgram, is serving
as interim associate dean for MSN and DNPStudies. Dr. Lock is in the position whilePatricia Howard is serving as interim deanof the college. In addition to her teaching,research and administrative duties, Dr. Lockcontinues to practice at the UK Women’sHealth and Rheumatology Clinic.
Leslie ScottLeslie Scott, PhD,PNP-BC, CDE, hasbeen promoted toassociate professorwith tenure. Dr.Scott is a 2004 PhDgraduate of UK. She
is the coordinator for the Pediatric NursePractitioner BSN-DNP Track and a board-certified pediatric nurse practitioner withmore than 20 years’ experience as a certified
diabetes educator. Her clinical practice is inthe pediatric endocrinology clinic at UK. Shehas provided care to children with diabetesfor more than 18 years. Dr. Scott received theExcellence in Clinical Practice Award fromthe College of Nursing in 2007. She receivedthe Outstanding Community ServiceAward-Camp Hendon from the BluegrassLeadership Council in 2006 and she receivedthe Diabetes Educator of the Year Award in1999 and 2003 from the Kentucky Chapterof the American Diabetes Association andKentucky Lion’s Club.
Darlene WelshDarlene Welsh,PhD, RN, hasbeen promoted toassociate professorwith tenure. Dr.Welsh receivedher MSN from
UK, specializing in adult critical care. HerPhD is in Educational Psychology fromthe UK College of Education. She teachescritical care nursing and coordinates asenior practicum in the College of Nursingundergraduate program. Her researchinterests include occupational stressamong hospital nurses, nursing care andintervention for heart failure patients, andpatient and student education.

Carol DonnellyCarol Donnelly, BA, retired in January
2013 after serving asthe college’s grantsfacilitator sinceJuly 2000. In thiscapacity, she assistedresearch faculty andPhD students withgrant applications,
renewals and manuscript editing. She alsoedited and published Inquiring Minds, anewsletter highlighting the research andscholarship accomplishments of facultyand students in the college, and served aswebmaster for the research section of thecollege’s website. Prior to coming to theCollege of Nursing, she served for six years ina similar position at the Southeast Center for
Agricultural Health and Injury Prevention.Post-retirement, she does consulting andfreelance editing.
In Memory
Gwen LeeDr. Gwen Lee, 77,professor emeritaof the College ofNursing, passed awaydue to Alzheimer’sDisease in July 2013.Dr. Lee retired
in 2002. She specialized in parent-childnursing, nurse-midwifery and maternal-childhealth early intervention. She also conductedresearch, taught and mentored many nursingstudents during her tenure. Dr. Lee earnedher diploma in nursing from the Schoolof Nursing, Nashville General Hospital,Nashville, Tenn. (1955); a BS from StetsonUniversity, Deland, Fla. (1959); an MS fromthe University of Colorado, Boulder, Colo.(1969); and a Doctor of Education fromUniversity of Tennessee, Knoxville, Tenn.(1973). Prior to coming to the University ofKentucky, she served on the faculties of theUniversity of North Carolina Chapel Hill,Clemson University, University of Tennessee(Memphis) and the University of Washington(Seattle). In the dark morning hours of 4:30, 5:00, 6:00, a chorus of alarms goes off.Some people start the day with yoga, prayer, or some other meditativeactivity that centers and grounds the mind and spirit for the day ahead.
Others just hit the ground running.Driving with one hand on the wheel and the other grasping a travel mug oflukewarm coffee, they drop their children off at school and daycare beforesearching for a parking space at work or school. And so the day begins ...

“Self-development is very much a natural part of what anurse does,” says Patricia B. Howard, PhD, RN, NEA-BC,FAAN, professor and interim dean, College of Nursing.
However, formally going back to school takes learning to the next level. Whether they are goingback for a second degree in nursing, enrolling in the RN-BSN Option, or are students in theDoctor of Nursing Practice (DNP) or PhD program, nurses juggling school, life and a job performa balancing act that requires a lot of flexibility and time management skills to be successful.
All of this adds up to a tremendous number of people returning to nursing school whilesimultaneously working and supporting a family. Fortunately most nursing programs assume thatstudents are working full time and offer options for part-time study. When it comes to balancingwork and school, it actually helps that nursing is a 24-7 job. Since nurses can work days or nights,weekdays, weekends and holidays, a resourceful student nurse can work around the demands ofwork, classes and personal life.
That’s exactly what Sonia Yaksich, BSN, BS, RN, UK Birthing Center staff nurse,UK Albert B. Chandler Hospital, needed to finish her bachelor’s degree in nursingby enrolling in the RN-BSN Option. She already had earned an associate’s in nursingand a bachelor’s degree in psychology.
In one sense, nurses are always students: always studying, always keeping upwith the latest in health care research related to patient care.
In the RN-BSN Option, says Gina Lowry,PhD, RN, senior lecturer and coordinator ofthe RN-BSN Option, students meet for classsix times in the fall semester, four times in thespring and three times in the summer to limitthe amount of time they will need to requesttime off from work. “The RN-BSN Option isdesigned around a distributed learning format,which means the classes are a mix of online,in-class and independent study,” says Dr. Lowry.Additionally, most classes are held on Saturdays.Dr. Lowry adds, “You just figure it out,” whenshe recounts the scheduling gymnastics she didduring that time of her life.
Mrs. Yaksich’s example convinced her husbandto go back for a second degree in nursing aswell. They were both in school until Mrs.Yaksich graduated from the RN-BSN Optionin May 2013.
“The end was pretty difficult,” says Mrs. Yaksich.“I have three children and a husband who is in
school full time so it was quite a juggling act.My final semester I worked three night shifts perweek and then I had a public health class thathad a clinical every Tuesday, and then everyThursday I had a 12-hour Synthesis class. I wasdoing four 12-hour days and one five-hour dayper week just of school and work, and I also hadpapers to write. All that and my children as well,including one who was born at the beginningof my last semester. So life was pretty crazy.”
Despite the challenging finish, Mrs. Yaksichsays the pace of life overall while working towardher BSN “… was really not that bad. At UK,you can choose to go full time for 12 monthsor you can do part time for two years. I did thepart-time option, and as a UK employee mytuition was paid for. Also, the hospital was alwaysaccommodating and made sure not to scheduleme the nights before I had class or clinical. I thinkif I can do it working full time with three kids,anybody could do it.”

Ms. Coles earned a degree in biomedicalscience and worked in clinical research for afew pharmaceutical companies before realizingnursing was her calling. “It just wasn’t fulfillingfor me because my passion is to work withand serve people,” says Ms. Coles. “It provokedme to go ahead and pursue my dream ofbecoming a nurse.”
With three children at the tender ages of 9,7 and 2 and working one 12-hour shift a weekas a student nurse tech at Baptist Healthcarein Lexington, she has learned to take the timemanagement skills she is being taught fornursing and apply them to the rest of her life.
“As I’ve been mentored by registered nurses,I’ve learned that time management is crucial,”says Ms. Coles. “I hope to be an ICU nurse andcould have up to two critically ill patients tocare for at the same time. It all becomes aboutprioritizing. One of the skills that UK has reallydone a great job teaching us is how to prioritize.In my day-to-day life, I work off a grid, literally.I block off my time.”
Her day usually starts at 4:30 a.m. with prayerand doesn’t end until around 11:30 p.m., whichmakes Ms. Coles’ daily pilgrimage to Starbuckseven more understandable. She keeps a realisticyet positive attitude toward the demands placedon her by school, work and family.
“It’s a continual balancing act that requires support fromfamily and faculty. I have by no means mastered it. I rely onmy faith, and I understand I have to be able to adjust. And
that’s something about being a nurse, period.You just have tobe flexible. And you have to be willing to roll with the punches.Being at UK, I’ve really honed in and learned how to do that.”
“I don’t know many nursing students who don’t have at least one pit stop to somecoffee shop throughout the day,” says second degree BSN student Joy Coles, BS,whose personal favorite pick-me-up is a caramel macchiato from Starbucks. “Ifcoffee isn’t your thing, it’s tea, or some sort of snack, something to get you throughthe second half of the day.”

While a BSN equips nurses with a broad-based education, a DNP will put Ms. Guiliani at alevel where she and her colleagues will apply research findings to practice and lead evidence-based change. “We are preparing leaders in advanced practice to go out and be the oneswho actually change practice,” says Interim Associate Dean for MSN and DNP Studies andAssociate Professor Sharon Lock, PhD, RN, APRN.
“Our families are very supportive, but the support wegive each other is really our saving grace. There are timeswhen each of us becomes so overwhelmed that we want
to quit. But our motto is ‘no child left behind.’ We’re all goingto do this together. Just knowing that you’re not the only
one who’s feeling overwhelmed helps put a differentperspective on things.”
While she is going in a million differentdirections throughout the week, Ms. Guilianiconnects with her group via hundreds oftext messages discussing class assignments,offering general encouragement and makingarrangements to find a narrow window oftime when they can all get together anddecompress. Their ability to burn through thedata on their iPhones is legendary. “One timewe had a group project that was due and wewere all texting back and forth. One of myfriends hopped in the shower and by the timeshe got out there were 50 new messages!”laughs Ms. Guiliani.
“I always tell students that it’s helpful ifthey can go through graduate school on
the buddy system. That way students canbounce ideas off each other and providesupport as needed. Ashley and her friendshave developed an extensive buddy systemthat is helping them be successful in theprogram,” says Sharon Lock, PhD, RN,APRN, associate professor and interimassociate dean for MSN and DNP studies.
“My manager is also very encouraging — notonly to me but to others on my unit — as wefurther our careers by going back to school,”says Ms. Guiliani of other factors critical toher success. “There have been some timeswhen I’ve been able to do some weird thingswith my schedule at work to accommodatesome things with school.”
More than caffeine, successful working students tend to have extensive networks ofsupport systems in place that make managing school, work and life possible. Theyinclude managers willing to be flexible with their work schedules as well as spousesand families willing to shoulder extra family responsibilities. And in the case of AshleyGuiliani, BSN, RN, ICU staff nurse, UK Albert B. Chandler Hospital, the most criticalcomponent is a group of friends and co-workers going through the program together.

The BSN-DNP Option is also set up to accommodate a wide spectrum of schedulesincluding people from out of state. Autumn Roque, BSN, RN, clinical care facilitator,MedStar Washington Hospital Center, Washington, is a third-year BSN-DNP studentat the college.
Roughly once a month during the semester,she flies in from her northern Virginia hometo attend classes. Years back when Ms. Roquewas working toward a BSN at the Universityof Louisville (U of L), her mentor, PeggyEl-Mallakh, PhD, RN, then at U of L butnow the track coordinator for the Psychiatric/Mental Health Track at the college, set up ameeting between Ms. Roque and then-associatedean of DNP studies Dr. Howard to talk toher about UK’s BSN-DNP Option, which hadopened in 2009.
“That kind of dedication and their vision fornurses pursuing graduate education, whatUK’s programs have to offer, and Dr. Howardtaking the time out of her schedule to cometo Louisville and talk to someone going toLouisville about her program really said a lot,”recalls Ms. Roque. “When I decided graduateeducation was something I wanted to pursue,it made a lot of sense to be at a program wherepeople were already invested in my success.”
The former Marine Corp reservist is a seconddegree nurse who used the life skills she learned
being in the military, living abroad, andeven working at Tiffany & Co. to advancefrom being a bedside nurse to nurse managerin less than two years.
Her decision to begin work toward a DNPwas made with long-term goals in mind.
“I decided I really need to go ahead and dothe work to make myself more marketable,”says Ms. Roque. “This degree is an importanttool to be a leader on a management team oradministratively and to be someone who is goingto move nursing forward. Certainly with theDNP I think we are on the pointy end of thespear on this movement. For me, I’m not old butI’m 37, I don’t have 10 years to figure it out. Ineed to start making things happen and makingmy mark now.” Ms. Roque’s day starts with a 45-mile, one-way commute in and out of DC fourdays a week that can take anywhere from an hourand a half to more than two hours, dependingon the time of day. With her job, commute andflying back and forth to Kentucky, Ms. Roquehas a punishing schedule that wouldn’t bepossible without the support of her family.
“My husband is tremendous,” says Ms. Roque. “I can’t tell you the last time I’ve seen the inside of agrocery store. He has taken on all the domestic duties. He knows how to do ponytails well, and he canbraid our daughter’s hair now, too. He has been in the Marine Corp for 22 years and is in a place inhis career where if the kids are sick or if there is a snow day, he can stay home with them. On the otherhand, I’m essential personnel so I don’t get a pass because of the weather or family obligations. He hasreally stepped up and has embraced moving our focus to where my career has to be the one gettingforward momentum.”
On weekends when she comes to Kentucky, Ms. Roque’s mother picks her up at the airport inLouisville and feeds her dinner while she’s in town. With her husband, mother and the support of herunit director, Ms. Roque is making it work.
“Many people are looking out for my well-being andpicking up a lot of things for me so that I can be successful,”says Ms. Roque. “I’m the one who will have to do the work
involved to succeed, but they take off so much stress.”

“Kidogo kidogo hujaza kibaba,” says Lori Mutiso, MSN, APRN-BC, a private practiceadult psychiatric/mental health nurse practitioner and PhD candidate, when asked howshe manages her life. “It’s a Swahili phrase that means ‘little by little fills the pot.’ ”
Mrs. Mutiso would know, as she has been in school more or less continuously since 2001. First anRN-BSN, then MSN, post-MSN and now expects to defend her dissertation soon to complete herPhD in nursing. She knows that having a PhD will give her the knowledge and skill set to developinterventions for improving patient care outcomes.
“There is a big push for translational research and nurses are now among the best suited to conductit,” says Terry Lennie, PhD, RN, FAHA, FAAN, professor and associate dean for PhD Studies. “It’stesting to see how it works in real life situations. It’s helping translate that research into practice.”
The “kidogo kidogo” mantra works in allareas of her life — academic, professionaland personal.
With her native Kenyan spouse, Mrs. Mutisofounded and coordinates a scholarshipfoundation for Kenyan young people whohave demonstrated a clear vision and an activeplan for a better Africa. Over the years, severalof the students have come to live with themin Lexington until they became acclimatedto American culture. Mrs. Mutiso is also aboard member and resettlement facilitator ofKentucky Refugee Ministries in Lexington, Ky.
“Education is very important in our home,”says Mrs. Mutiso. “But you cannot just putfamily relationships on hold. You have to do alittle school every day and a little family everyday. Life is going on all the time and if I hadignored it, I would have missed out on over adecade of relationships!”
One of the key factors Mrs. Mutiso has learnedover her years in school is that her “zone time”
is when she gets the most accomplished.Mrs. Mutiso isn’t naturally a morning personand sleeps in compared to some of her fellowstudents. This gives her the energy she needs toserve her patients, focus on her dissertationand enjoy life with her family as fully aspossible. Mrs. Mutiso sees patients in her privatepsychiatric practice from 10 a.m.-8 p.m. Thisworks well with her unconventional homelife and benefits her patients because she canaccommodate late day appointments.
Sometimes life just happens with a vengeance,however. Mrs. Mutiso’s husband and father bothrecently suffered major health events on the sameday. “The UK faculty has been so supportive ofme as a person,” says Mrs. Mutiso. “To themI’ve always been a respected colleague as well as astudent. With my father’s and husband’s healthdifficulties, they have been very compassionate.Deadlines don’t rule everything. I strive forexcellence and do what I can, but when crisiscomes, they’ve been incredibly supportive of me.”
“Make sure it’s absolutely your passion,” Ms. Coles advises. “Make sure there isa sense of calling and duty. You can’t decide ‘I want to be a nurse because the fieldis exploding and it has a good job outlook.’ Nursing is a heartfelt profession. Yourheart has to be in it.”
While their schedules can seem overwhelming, students tend to share the perspective that thelong-term benefits outweigh the short-term challenge. “There is, as they say, a light at the endof the tunnel,” laughs Ms. Guiliani who is expecting to graduate with a DNP in May 2014.“Though from here it’s just a glow, but the glow is getting brighter!” It’s more than just enduringthe journey through school, it’s also enjoying the process. “I thought I would plow throughand then finish and enjoy life, but I found that the process of taking classes and writing thedissertation was really fascinating and enjoyable.”

Nearly 40 percent of employed nurseswork outside a medical center setting.Many work for private practices, public health agencies, primary careclinics, insurance companies, nursing homes, schools, hospices, themilitary and industry. Others choose academic careers as college professorsor research scientists. Across Lexington, UK College of Nursing professorsand alumni can be found in myriad non-hospital-based environments.
For many students, their introduction to non-hospital-based health careoccurs during their undergraduate community and public health course.Hartley Feld, MSN, RN, PCHNS, lecturer and clinical instructor, connectsstudents with organizations in Lexington that expose nurses to populationsmost likely to develop chronic health conditions in the future. Theseorganizations serve low-income individuals in both rural and urban areas,undocumented immigrants, refugees, racial and ethnic minorities, peoplewith disabilities, the homeless and the uninsured. Her goal? To transformnursing students’ perceptions about the origins of health, wellness anddisease. After taking classes that teach how to care for individuals, studentsattend her class to learn the larger issues related to population health. Shehopes to inspire empathetic and passionate nurse advocates for vulnerablepopulations, not only for that semester but for a lifetime.
“I have the best job. I really do,” says Ms. Feld. “I am able to see students,who may be out of their comfort zones at first, have transformationalmoments. After a couple of weeks they rise to the occasion, build rapport,become great listeners and begin to positively influence their population’shealth decisions. Most of our students feel they are making a difference inthe community, which is very empowering.”
Current and former community partner organizations include the BluegrassDomestic Violence center, AVOL (HIV prevention), Maxwell StreetPresbyterian Church (homelessness), Girls on the Run (teen confidence-building), the Lexington Farmer’s Market (SNAP and WIC promotion),Bluegrass Pride (environmental health concerns), the East 7th Street Center(after school program/free meals for children) and even the LexingtonAcademy of Barbering.
“The barbers make sense,” she smiles, “when you consider how many at-riskindividuals visit their barber more often than their health care provider.”

Ms. Feld has seen firsthand thatbarbers can be strong partners forcommunity health nurses. Thenursing students collaborate with thebarber students and provide healthinformation for their clients, largelyurban male populations that may beat risk for poor health outcomes. Thenursing students share knowledgeand resources for smoking cessation,HIV/STD prevention, and lifestylechanges that can lower bloodpressure, prevent diabetes, and reducethe risk of developing cancer.
For the nurses, an increased awarenessof public health issues often bringsan increased compassion for theaffected populations. The studentscan be deeply moved when listeningto circumstances that lead people topoor health, such as poverty, abuseor discrimination. Ms. Feld recalls arecent student journal entry followinga spa and wellness day at theBLU EGR A SS DOM ESTICV IOLENCE FACILIT Y. Asthe student massaged and paintedthe fingernails of an abuse victimwho was afraid to return home, thewoman began to cry because she hadnot been touched in a positive way
by another human being for a longtime. However, the young nurse hadto smile later on, says Ms. Feld, as thewoman “laughed with her friends,pointed at the male genital model onthe condom demonstration table, andsaid ‘I never want to see one of thoseever again.’”
The mission of the U K CEN T EROF EXCELLENCE INRU R A L HE A LT H is toimprove the health of ruralKentuckians through education,research, service and communityengagement. Here providers aretrained to work in rural areas,and there is a focus on workforceneeds, research and the preventionof chronic rural health issues. Thecenter’s director, Frances Feltner,DNP, RN, has known her career pathsince early childhood.
When she was 9 years old, Dr. Feltnerhad a medical condition that forcedher to spend time at CincinnatiChildren’s Hospital. “I met a nursenamed Miss Richter,” she says,“and she was very kind, gentle andtalented. She became my example, myrole model, and from that momenton, there was never a doubt that I’dbecome a nurse.”
However, the hospital setting wasonly a stepping stone on her careerpath. Dr. Feltner has more than 30years of nursing experience, includingacute care, but prefers the varietyof responsibilities that her currentposition provides.
“As the center director, I get to wear alot of hats,” she says. “I’m in chargeof budgets, goal-setting, missionalignment, grant writing, strategicplanning, academic administrationand research. I’m also the chief cookand bottle washer.”
In leading a team dedicated toaddressing Kentucky’s health caredisparities, Dr. Feltner often relieson her medical center backgroundto give her the “ability to understandthe entire spectrum of organizationstrying to improve rural communityhealth.”
“The high point of my day is when Ican see multiple government andgrassroots groups working togetherto reach out and touch peoplethroughout the Commonwealth,”she says.
As an advocate for rural health,she is often involved in legislativeand legal endeavors, helping peoplenavigate health care systems, dealingwith a shortage of rural providers,and improving the transportation,communication and follow-throughfor patients living in remote areas.
“You don’t just walk into a communityand say ‘this is what you need,’” sheexplains. “You have to listen to theindividuals and get feedback. Youneed to do your research, but thenmake sure to report your findingsback to that community. Healthpromotion is a two-way street.”
Dr. Feltner believes that today’shospital-based nurses should try tosee patients more holistically. “I get tosee them at home, where sometimesthey don’t have the right medicationor can’t find a ride to a health carefacility. It can be heartbreaking. Weas a society must take responsibilityand do everything within our powerto help these folks.”
“In a hospital setting, you’re focused onthe crisis at hand,” says DNP studentSusan R. Noel, MSN, APRN, RN,clinic director at the T HOM A SH . PIN K STA FF M EDIC A LHOM E CLINIC . “You’re tryingto preserve life or maintain a bodyfunction. In the outpatient world,it’s more about the long-term healthjourney.”
Ms. Noel works in a Lexington-basedclinic that is a joint venture amongthe Kentucky Cabinet for Health andFamily Services, the Commissionfor Children with Special HealthCare Needs and the UK College ofMedicine Department of Pediatrics.It’s dedicated to bringing primarycare to children in foster care andthose with special health needs.

She is working to complete herDNP at the UK College ofNursing, where she is enrolled inthe Psychiatric/Mental HealthTrack. She did her clinicalsupervision at the UK Center onTrauma and Children.
“I found a lot of satisfactionworking with individual childrenat the center,” she explains. “But Ilearned that there is no quick fixfor these kids. You have to be inthis for the long haul.”
Ms. Noel often sees fosterchildren at the clinic who maybe visiting for a physical illnessbut are exhibiting signs ofpsychological distress as well.Sometimes the children appearsad or have behavioral problems.She applies her clinical experiencefrom the Center on Trauma andChildren to help her patients healboth physically and emotionally.
Ginny Sprang, PhD, a professorin the College of MedicineDepartment of Psychiatry andthe executive director of theCEN T ER ON T R AU M AA N D CHILDR EN ,appreciates the skills necessaryto lead teams in a non-hospitalsetting. In a single day, she mayact as an advocate, legal expert,clinician, researcher and businessmanager to promote a betterunderstanding of how stress andtrauma can affect children.
“Early in my career, I spenttime working with a child whohad witnessed the murder ofhis parents and was sufferingfrom severe anxiety and troublewith his foster parents,” she
remembers. “I learned that byteaching the child and the fosterparents to communicate better,we could decrease the anxiety andimprove the situation. It was anincredible experience.”
Although she now works for auniversity, Dr. Sprang believesthat while some ideas fortherapeutic psychology can beinformed by validated research,“the best ideas don’t come fromthe ivory tower. They come fromthe people doing the work in thetrenches.”
Ms. Noel agrees. She relies onthree main strategies that sheexplored as a student at theCenter on Trauma and Childrenand has now applied to her newposition.
First, she says, the communityhas a responsibility to identifychildren suffering from traumaas quickly as possible, becauseproblems can be deterred ordiminished if they’re recognizedearly. Second, cognitivebehavioral therapy teachespatients healthy ways to deal withthe memories and thoughts ofwhat they’ve been through. “Likedeep-breathing exercises whenthey get that panicky feeling,” shesuggests.
Finally, Ms. Noel firmly believesin helping children developstronger communication skillswith their parents or guardians,a process that works best in anextended, non-hospital setting.For example, she was surprisedand delighted when, after monthsof therapy with a particularly

frustrated foster child, his seasonedfoster parent grew more confidentand successful. “The mom was reallygrateful,” remembers Ms. Noel. “Shesaid, ‘I wish I’d used these techniquesto raise my own children!’”
Dr. Sprang appreciates her evolutionfrom working with individualchildren to providing community-sized solutions for larger populations.“We really must give attention to themost vulnerable, the children withoutaccess to quality care,” she warns.“We must care for and defend theweak and defenseless.”
Samantha Todd, MSN, RN, APRN,is also concerned with helpingKentucky’s most vulnerable citizens.She is a family nurse practitionerat the SA IN T JOSEPHCON TIN UING C A R ECLINIC , part of the Saint JosephPhysician Enterprise in Lexington.Her clinic provides care to low-income, uninsured patients throughthe organization’s financial assistanceprogram. New patients for the clinicwith no designated primary care
provider are identified upon hospitaladmission to Saint Joseph Hospitaland are often uninsured, but someMedicare/Medicaid patients are alsoreferred for services.
Ms. Todd provides her patients withbasic health care screening and helpsthem manage chronic illnesses such asdiabetes and high blood pressure.“I’ve had patients who say to me,‘I wouldn’t be alive without you,’”she says. “And that can be a prettyhumbling experience.”
She serves about 500 patients, mostlylow-income adults and many withhigh-acuity health problems. “I seea lot of chronic illness,” she explains,“I don’t see a lot of sniffles and sorethroats.”
“I recently lost a young mother in her30s who had a cocaine addiction, heartcondition and high blood pressure,”she recalls. The patient was on a bloodthinner following heart surgery andhad been though rehabilitation, buther continued cocaine abuse causedher to avoid the Saint Joseph clinic.
“We couldn’t find her for six weeks,and then she was referred again afterhaving a stroke. It was devastatingwhen the hospital called to inform usthat she had died several weeks afterthe first stroke, from a second brainhemorrhage,” she remembers sadly.
However, Ms. Todd uses experienceslike this one to fuel her inner fire bysharing her wisdom with incomingnursing students. As a preceptor forthe College of Nursing, she takes onone student per semester and gives thestudent a safe environment in whichto hone skills.
“I had the most knowledgeable, patientpreceptors when I was a student,”she says. “I just want to show myadmiration and respect for thisincredible profession. I love what I do,and I love my students.”
Eric Afuseh, DNP, RN, served as apreceptee under Ms. Todd duringhis family nurse practitioner DNPstudies at the College of Nursing. Hebelieves his time at the Saint JosephContinuing Care Clinic allowedhim time to explore the differencesbetween hospital-based and familycare settings and better steer his owncareer compass.
“I’ve always wanted to work as a familypractitioner, because most of the issuesyou see at a hospital intensive care unitare preventable,” he explains. “Youhave to get to the root lifestyle cause ofproblems before a crisis hits. WatchingMs. Todd in action truly reinforcedmy determination to be a familyclinician.”
While studying at UK, Dr.Afuseh also did a rotation at theFAY ET T E COU N T YDET EN T ION CEN T ERCLINIC , where he worked withmany patients who suffer fromalcohol and prescription medicationaddictions.
“Most of them are non-functional.They drink for a living, up to half agallon of vodka per day. There arehuge public health ramifications fortreating this population,” he says.
The nurse practitioner at the FayetteCounty Detention Center, DonnaSchwartz, MSN, RN, APRN, agrees.“The low point is seeing a 19-year-old incarcerated with an intravenousheroin addiction who has beendrinking since the age of 14,” shesays. “When they come down, theycome down hard. Hallucinations,seizures, sometimes lasting for a weekor more.”
But after inmates have detoxed forseveral days and “have come back to

earth,” Ms. Schwartz appreciates theirrenewed sense of self. “You can see itin their eyes, the clarity,” she explains.“And although the relapse rate is 90percent, it really pleases me whenyou can tell by their stance, by theconfidence in their voice, that they’regoing to make it this time.”
She recalls a gentleman, a severealcoholic, who although always brightand joyful, had been a repeat visitorto the clinic that she had counseledmany times. When a friend of his wascommitted, Ms. Schwartz inquiredabout the man and learned that hehad gone to rehab and had been cleanfor eight months. “I was overjoyed,”says Ms. Schwartz. “Eight months ofsobriety is fantastic!”
Ms. Schwartz began her health carepath as a 13-year-old candy striperin a hospital. She later became a
paramedic, a nurse and eventuallyan advanced practice family nursepractitioner. “But nothing beatsthe fulfillment I get from working atthe detention center clinic,” she said.“It’s where I’m most needed. It’swhere I belong.”
Whether they care for patients inrural or urban areas, these nursingspecialists in non-hospital-basedsettings have taken the road lesstraveled. But what about the future?Do they see more or less dependenceon acute-care hospitals and medicalcenters during the coming years?
Dr. Afuseh is amazed at the amountof suffering that people will endureto avoid making lifestyle changes.“Family care and preventive medicinewill become our main focus,” hebelieves. “It simply has to.”
Dr. Sprang argues that greatercentralized decision making andbureaucracy could harm the intimatebond of a nurse-patient relationship.She is excited to see “young nursesputting their patients first, takingresponsibility and advocating for theirbest interests.”
Meanwhile, Ms. Feld recently receiveda text from one of her former students,now employed at a hospital, asking forthe Refuge for Women phone number.The former public and communityhealth student wanted to refer a patient,a possible victim of human trafficking,from her hospital unit.
“If you were a poor, victimized girl,what would you want your nurse todo? I was proud of her for stepping into make that referral,” says Ms. Feld.

the UK College of Nursing doubledundergraduate enrollment to producemore quality, BSN-prepared nursesfor Kentucky and the nation. Itwas a bold and strategic move forthe college. Not only did it bringin additional talented, new nursingstudents each semester, but also agrowing number of faculty membersto teach them, including early careerPhD faculty on the threshold ofpromising research careers.
“We have several now who have beenwith us for three to five years, andall have major teaching roles in ourundergraduate program,” says Collegeof Nursing Interim Dean and ProfessorPatricia B. Howard, PhD, RN, NEA-BC, FAAN.
“All have completed a dissertation in anarea that supports nursing research ona major new front,” says Dr. Howard.Dissertation topics included a cost-effective intervention to improveoutcomes for post-discharge surgicalpatients and interventions based onnew knowledge in genetics to improvethe health of entire communities.“These undergraduate facultymembers are at the beginning of veryimportant research programs,” saysDr. Howard. “All have demonstratedthe talent and ability to follow inthe footsteps of our senior researchfaculty and are exemplars of the nextgeneration of nursing leaders at thecollege.”
The new associate dean for researchat the college is Thomas Kelly, PhD,Robert Straus Professor and vice chair
of the Department of BehavioralScience in the UK College ofMedicine; professor of psychiatry andpsychology, and director of ResearchEducation, Training and CareerDevelopment for the Center forClinical and Translational Science.
“I’ve been at the university since 1992and have had the good fortune towork on research collaborations andon PhD committees with a numberof faculty at the college over theyears,” says Dr. Kelly. “Senior facultymembers have worked very hard andvery effectively to impact the health ofKentucky as well as the nation.”
Like Dr. Kelly, Dr. Howard believestoday’s new research-focused facultynot only has the ability to impactthe future of evidence-based nursingpractice but also to impact the
undergraduate nursing studentsthey’re teaching.
“They’re energetic researchers who areforerunners in several areas of nursingresearch — and our undergraduatesare the recipients of their expertisein the didactic courses and throughour Undergraduate Nursing ResearchInterns program,” said Dr. Howard.

and associate dean for PhD studies.Both senior faculty members havebeen nationally recognized for theirseminal research in cardiovascularhealth and serve as co-directors ofthe college’s RICH Heart researchcollaborative, where youngerresearchers such as Dr. Dekkerdiscovered her own calling whileserving as a research assistant.
“Dr. Lennie encouraged me tocontinue my education and go intoresearch while I was still in the MSNProgram,” says Dr. Dekker. “I’d nevereven considered it before.” She tookher mentor’s advice. By the time she
had finished her PhD, Dr. Dekkerhad already been published in anumber of peer-reviewed journals,participated in national conferencesand had been recognized with aprestigious award as a rising younginvestigator.
“Dr. Lennie and Dr. Moser sawsomething in me that even I didn’tsee,” she says. “That’s what greatmentors can do.” Today, Dr. Dekkeris teaching pathopharmacology tofirst-semester junior BSN studentswhile pursuing her own line ofresearch related to the use ofcognitive behavioral therapy to treat
depression in heart failure patients.Her current study is supported by aNational Institutes of Health (NIH)career development grant throughthe National Institutes of NursingResearch, which is supporting hertraining in cognitive behavioraltherapy training at the renowned BeckInstitute and her training in clinicaltrial research through the Universityof Kentucky Center for Clinical andTranslational Science.“There’s a pushtoday for evidence-based nursingbecause it’s so needed,” she says.“And we need to keep pushing fordissemination and translation of thescience so we can implement what
we know works to improve patientoutcomes.” Every semester Dr. Dekkeraddresses senior nursing students andurges them not to wait to continuetheir education. “The future ofnursing depends on nurse researchershaving long career trajectories.”
“Nursing is fairly unique in that manywill assume their roles as facultymembers fairly early after completingtheir PhD programs,” says Dr. Kelly,noting that most research faculty inother health disciplines have spentyears in post-doctoral training andare well-established in their researchprograms before taking on teaching

Ana Maria Linares, DNS,RN, assistant professor,College of Nursing, receivedher degree in midwifery from theUniversity of Chile and her Doctorof Nursing Science degree fromthe University of Nuevo Leon inMonterrey, Mexico. She came toKentucky in 2009 and joined theundergraduate faculty to teachobstetric nursing and work on her
research interest in the health ofwomen in vulnerable populations.During the summer of 2013, Dr.Linares completed a qualitativeresearch study on the exclusivebreastfeeding decision of Hispanicwomen. An earlier study, funded bythe university, found that despite well-documented evidence on the healthbenefits of exclusive breastfeeding forinfants and for mothers, Hispanicwomen in Central Kentucky wereroutinely choosing to supplementwith formula. Her current studyon decisions surrounding exclusivebreastfeeding in the population is
supported by funds granted by theCollege of Nursing.
“These initial start-up financialresources can get research programsoff the ground,” says Dr. Kelly. “Itgives early career faculty the trainingand the freedom to begin generatingthe preliminary data required to becompetitive for independent awardsand funding.”

For Jan Odom-Forren,PhD, RN, CPAN, FAAN,assistant professor, Collegeof Nursing, a passion to improveoutcomes for perioperative patientswas influenced by her years in clinicalpractice working with recoveringsurgical patients. Dr. Odom-Forrenwas already a recognized authority inher field when she decided to pursueher PhD and a career in research.She had lectured nationally andinternationally to perioperative nurses,
contributed chapters to books andeven authored a book, a nationallyrecognized text on the care ofperianesthesia patients.
Today, Dr. Odom-Forren is writingwhat she hopes will be anotheraward-winner — her first majorgrant proposal since completingher PhD program. She’s seekingsupport from the Patient-CenteredOutcomes Research Institute(PCORI) for an investigation intothe self-management of symptoms forpost-discharge ambulatory surgicalpatients. It’s a complex study thatwill involve qualitative research, the
development of an intervention andrandomized controlled trials to testthe effectiveness of the intervention.
She’s grateful to Dr. Kelly for thehelp he’s given her in writing theproposal and for the support of herfaculty colleagues. Once a week,research faculty meet to talk and shareinformation. “I can get comments onan abstract for a poster I’m workingon, or I can send out a piece of thegrant I’m writing ahead of time andget feedback at the meeting.” Her workis being supported by collaboratorsfrom within the college and from theuniversity at large.
“UK has some unique researchstrengths as an institution,” saysDr. Kelly. “We have geographicallylocalized expertise in nursing, healthsciences, pharmacy, medicine,public health, dentistry … andthe university has been really goodabout setting up policy that hasbeen supportive and encouraging ofinterdisciplinary kinds of activities.”

Dr. Mudd-Martin’s background isin public health, but she found theinfluence of new genetic informationon health practice and its potentialfor impacting the health of vulnerablepopulations intriguing. “In thecommunity setting where I worked,people were asking me about geneticinformation they were receivingand the health implications of theirfamily history.”
Dr. Mudd-Martin entered doctoralstudies after completing her master’sdegrees in nursing and public health.She finished her doctoral program in2007 under a faculty mentor whosebackground is in genetics.
She says the community-basedparticipatory research she’s pursuingtoday in Kentucky stemmed from herearlier experiences in the field. That’snot uncommon in nursing research,says Dr. Kelly, and in fact, offers adistinct advantage. “Nurses engaged inresearch have had professional trainingand often their clinical experiences givethem a unique awareness of where thegaps are in patient care and where thechallenges are in health management.”
Currently, Dr. Mudd-Martin isprincipal investigator on a studyunder the umbrella of a larger grantawarded to the College of Dentistryby the Centers of Biomedical ResearchExcellence (COBRE). COBREgrants are designed to encouragemultidisciplinary research and arestructured to give mentored juniorinvestigators a chance to lead smaller,stand-alone studies within the largergrant’s theme and scientific focus.Earlier this year, Dr. Mudd-Martin’stime was dedicated to completing theCOBRE study and to her work as a co-investigator on a major PCORI-fundedHeart Health study led by Dr. Moser.It was the first time since joining thefaculty that Dr. Mudd-Martin didn’tteach the undergraduate course inpublic health. While she was gladto have the time for her research, shemissed the students and the energy ofthe classroom. “To have a student whograduated two years ago and is nowworking in an emergency departmentemail me and say, ‘now I understandthe importance of prevention and whatwe talked about in class. If only I couldkeep people from having to come to theER in the first place … how do I goback and study for public health?’ That,for me, is very exciting.”

“I know that I’m prejudiced, but I thinkwe turn out the best quality nurses inthe country—no question,” says BrendaGhaelian, director of instructionaltechnology at the College of Nursing. “We’retough and it’s a rigorous curriculum, but ifour graduates run into a heart attack or abirth their first day on the job, they’ll knowexactly what to do.”
“The information technology revolutionthat shook Fortune 500 companies is justbeginning to push health care and education,”says Jason Harris, director of informationtechnology at the college. Before coming toUK in April 2012, he managed systemwideprojects for large U.S. corporations, includingthe management of the centralized help deskthat supports Toyota manufacturing across allof North America.
Mr. Harris believes that students have cometo expect cutting-edge technology as anintegral part of their education experience.“There are so many great reasons to fallin love with this stuff,” he says. “Thecost savings for students, time savingfor professors, amazing cross-disciplinecollaboration opportunities, the breakingdown of the status quo … it’s truly anexciting time to be in a university setting.”
LIGHTS, CAMERA,ACTION!The ability to digitally record, upload andreview the results of a nurse practitionerstudent interacting with a standardizedpatient is just one example of how thecollege is employing new technologies.
“With the digital video, we can giveimmediate feedback,” explains Lynne Jensen,PhD, RN, APRN-BC. “And by filming ourstudents interacting with actors portrayingpatients, we dramatically improve ourstudents’ clinical and interpersonal skills.”
To give students the opportunity to testtheir skills, actors are briefed on a wide-ranging scope of health issues, includingdiabetes, headaches, bronchitis, high bloodpressure, heart disease, abdominal painand a host of other concerns. As Dr. Jensenelaborates, “I can tell my DNP students thatwhen they begin to study health assessmentsand then in their specialty courses atUK, that they’ll be exposed to the top 50diagnoses in their practice. Our standardizedpatients will get them ready for what they’llsee in the real world.”
Standardized patients also can help teachstudents how to navigate the fine linebetween being clinically focused and beingemotionally connected and present withtheir patients. Dr. Jensen recalls a recentevaluation scenario where the actress woreheavy black makeup over one eye to simulatea domestic violence injury.
“It was so moving,” says Dr. Jensen, “becauseyou wanted to applaud the nurse practitionerstudents for being compassionate andconcerned about their patient’s safety. Theywere even helping her to make escape plansto avoid further violence.”
But it was a powerful lesson in compassionversus concentration when the studentsreviewed themselves on tape and noticedthat in their effort to connect with andprotect their patient, they had forgotten to
touch her face to check for broken bones. “Itwas a tough lesson to learn,” says Dr. Jensen,“but better here than in the real world.”
MANNEQUINS BRINGPATIENTS TO LIFEAs the clinical simulation facility manager,nursing instructor Mary Jayne Miller, BSN,RN, is in charge of the next-generationversion of patient assessment—high-fidelity simulation. “Communication is soimportant,” she explains. “Our simulationsteach students how to get the correctinformation without upsetting their patients,in real time.”
These life-sized, computerized mannequinscan respond vocally and physiologically tointerventions, allowing nursing studentsto experience incredibly dynamic scenarios.The simulations can be programmed to bevery lifelike, says Ms. Miller. “The studentsmight be working with a catheter andsuddenly urine will be running everywhere,and I think to myself, Surprise! Welcometo nursing!”
“A cup of coffee with some yellow foodcoloring can make a realistic batch of urine,”says Greg Williams, MBA, the simulationtechnology specialist who programs the high-fidelity health assessment simulators. “Weaim for realism as much as possible.”
Currently, the college uses seven computerizedpatients: two men, two birthing mothers,two babies and an adolescent. The “patients”offer a variety of programmed vocal andphysical responses including pulse, bloodpressure, heart, lung and bowel sounds,

blink rate, pupil dilation and the ability tohave artificial blood drawn via injection. Thebirthing mother simulators can hemorrhage,and the babies can be delivered breech, oftenproviding dramatic results.
Naomi Sloan, an undergraduate nursingstudent who has worked extensively with thehigh-fidelity simulators, was initially caughtoff guard by their realistic nature. During arecent emergency simulation of a birth with aprolapsed umbilical cord, the “baby” began todescend while the simulator mother groanedaloud, “I feel something between my legs.”
“I thought, well, yes, you’re having a baby,”says Ms. Sloan. “And then matter-of-factlyyou have to insert your hand into the vaginaand push the cord back up.” The strangething about the simulation process, saysMs. Sloan, is that after the baby is born,“the baby just glides back into its motherautomatically, like it’s no big deal. Justwaiting for another lab class.”
The College of Nursing simulation labsare identical to rooms in the new patientcare pavilion at UK Albert B. ChandlerHospital. “These labs were originally thedemonstration patient rooms before theybuilt the new health care pavilion,” explainsMr. Williams, “so our students can practicewith simulated patients in environmentsidentical to the real thing.”
After graduating from the college in 2010,Nick Welker, MSN, RN, almostimmediately encountered several medicalcases in the field that directly replicated casestudies that he had seen in the simulationlab. “That kind of realism, all of that detail,truly prepared me for my current role as
an acute care provider,” he says. “I like theICU and critical care simulations, becauseas students we’re exposed to a broad scopeof complex and basic care challenges,”agrees Ms. Sloan. “It helps with our criticalthinking skills to be in a realistic setting.”
However, Ms. Miller observes that thereis a limit to the amount of realism somenon-health care professionals wish toexperience. In the past, her team wouldplan a simulation exercise for touringgroups visiting the facility that wouldinclude a dramatic cardiac arrest and CPRscenario to show how the technologyworks. Unfortunately, they had to stopthe code simulations because the visitorswere disturbed when the patient died.“We thought it was an exciting script,” sheexplains, “but a non-nurse audience doesn’tappreciate that we make our simulations asrealistic as possible.”
IT HARNESSESTHE DATACertainly, the goal of using technology inthe clinical setting is to create more positiveoutcomes. Another way that technologybenefits the field of nursing is when itfacilitates advanced research projects.Mary Kay Rayens, PhD, professor andbiostatistician, is dedicated to making thatdream come true for students and facultyalike. She primarily uses technology toanalyze research data. However, whenthe sheer number of annual researchprojects became cumbersome to track, thebiostatistics group turned to Mr. Harris andthe IT team to provide a solution.
“We have a biostatistics team at the UKCollege of Nursing that is dedicated tohelping faculty and students design andconduct research studies, analyze the data,and develop reports, presentations andpublications,” says Dr. Rayens. “Our three-person team works on more than a dozenlong-term funded studies and at least 50 pre-funded faculty or student projects every year.”
Once the data have been collected, Dr.Rayens uses statistics software to determinethe answers to the research questions posedat the beginning of every project. Shebelieves that biostatistics is an important toolin allowing health care professionals to makeobjective, evidence-based decisions.
The importance of evidence-based decision-making is stressed with doctoral students.“When a new kind of medicine or therapycomes out, I want my students to beskeptical,” she says. “I want them to go tothe literature, read the study and judge forthemselves if it’s worthwhile.”
But recently, as the number of researchprojects within the UK College of Nursinghas continued to grow, a system to organizeand maintain the information became anecessity, so the biostatistics team called onthe IT department for help.
“When Mary Kay asked us if there was asimple way to use technology to track herteam’s workload and deadlines, we had tothink about it for a minute,” remembers Mr.Harris. “Then, we simply gave her a projectmanagement system identical to the systemwe use in our IT department. Sometimes thesimple solution is the best solution,” he sayswith a smile.

THE ELECTRONICBLACKBOARDAnother simple solution to the challenge ofincreasing collaboration and communicationis Blackboard, a web-based classroomsoftware. A cloud-based learning application,Blackboard offers students a personalizedapproach to education, with integratedteaching tools such as discussion boards,embedded multimedia, social networking,interactive rubrics, assessments and testing.
“Blackboard was an integral part of myMSN and even more so with my DNP,” saysMr. Welker. “It’s also the central hub forthe undergraduate high-acuity nursing classthat I teach.”
“I’m only 35, but I’m a digital immigrant,”he continues. “These 21-year-olds havenever even seen an Encyclopedia Britannica,let alone used one. I had to change myperspective in order to reach them.”
Mr. Welker believes that not only does it savehim time and energy as the instructor, but italso prompts students to be active learners,allows them to watch lectures ahead of labtime and encourages them to synthesizeinformation and act dynamically rather thansimply regurgitate notes from a textbook.
Mr. Harris notes how easy and intuitive thenew technology has become. He recentlywatched as a room of 60 students registeredand logged into their accounts for the firsttime. “Afterward, I asked the students toraise their hands if they thought the processwent smoothly. I was elated to see 59 handsshoot up — only one had an account issue.”
TECHNOLOGY WITHA LARGER PURPOSEThe UK College of Nursing is combiningeach of these technologies: digital cameras,simulated patients, biostatistics and web-based education, to create a 21st centuryclassroom. Now, a typical lab class willwatch a PowerPoint presentation withan online voiceover as a pre-lab lecture,then work on the assessment skills in thesimulated environment and finish with areview of the video footage.
Ms. Sloan appreciates how these newmethods combine to allow nursing studentsto “put all of the theories into practice whilepreparing us for real patients in the future.”
Mr. Welker predicts an increase in morehandheld tools and applications (apps) forfuture nurses. He notes that patients witha history of heart problems can already usea smartphone app to perform an EKG thatwill immediately notify their health careprovider of the results. “Soon we will beprescribing apps for patients as readily as weprescribe medications,” he says.
Meanwhile, Mr. Williams thinks that thenext level of simulated clinical assessmentwill include more complicated verbalinteraction and more delicate simulated tests.“Imagine what will happen when you cantrain with a simulated patient that’s gettingangry and frustrated with you,” he wonders.“Now that’s real life.”
“I’m looking forward to more remoteeducation,” says Ms. Ghaelian. “You can’texpect people to give up their jobs and moveacross the country to go to school anymore.We need to connect.”
Ms. Miller is looking forward to moreinter-professional simulations amongdoctors, nurses and pharmacologists. “It’svery rewarding and the students love it,” saysMs. Miller. “They are very, very comfortablewith this technology.”
After watching thousands of hours ofdigital footage of students interacting withstandardized patients, Dr. Jensen prefers tofocus on the big picture when consideringthe future of technology and nursing.She believes that the high-tech tools helpstudents and faculty work better and moreefficiently. But in the real world, computersand simulations are only part of the story.
“All of this new technology is great,”says Dr. Jensen, “but only in the handsof a compassionate, well-prepared nursewho understands and knows how to usethe technology.”

The promotion to head nurse meant only 10 cents morean hour, but she still loved being a nurse.
Now she gives back to the place that got her started.Giving back has been part of Vicki Beekman Gorman(BSN ’66) and her husband F. Chris Gorman’s (A&S’65 and Law ’67) lives. Mrs. Gorman says, “I alwaysknew I wanted to be a nurse. I had been a candy striperbefore going to college and loved that work and beingat the hospital — helping people.”
Mr. Gorman, a former attorney general of Kentucky,adds, “We believe you should always give back when youbecome successful, especially to the places that gave youthe tools for that success like UK did for us.”
Mrs. Gorman began her nursing career at UK’s AlbertB. Chandler Hospital in the Clinical Research CenterUnit after receiving her BSN degree. She and herhusband moved to Louisville after he finished his UKlaw degree, and Mrs. Gorman took a position in JewishHospital’s ICU. She was quickly promoted to headnurse — a position at the time that only earned her anextra dime per hour.
Mrs. Gorman accepted a director’s position withCaretenders in 1987 in their Private Duty Department.“The Private Duty Department was made up of staffwho went into homes, mostly seniors, and helped withpersonal care, light cooking and cleaning, and somedriving for errands. I saw firsthand how difficult andfrustrating it often was for these folks to get home afterbeing cared for in the hospital only to find that theycouldn’t get into their kitchen because of a wheelchair orwere no longer able to drive themselves to appointments.That’s where my group stepped in to help themtransition to new ways of taking care of everyday tasksso they could remain in their homes longer.”
Caretenders merged with Almost Family, and Mrs.Gorman continued to work for Almost Family, servingin several other administrative leadership positionsduring her 23-year career there.
For several years, the Gormans helped to support thecollege through phonathon gifts but then decided tomake a firmer, more personal commitment — onethat would benefit one selected student each year. Mr.Gorman said, “Vicki supported my political careerall along. I wanted to make sure she was recognizedin her own right.” The result was their establishingthe Vicki Beekman Gorman Undergraduate NursingScholarship in 2010.
Now they get to know the student who benefits fromtheir philanthropy. The Gormans met their firstrecipient at the college’s annual scholarship brunch.“It was wonderful to meet our first recipient andlearn more about her. She had so much energy andenthusiasm. It felt good to know we were helping herachieve her dream to be a nurse,” Mrs. Gorman said.
Mrs. Gorman eagerly told her Almost Family co-workers how much she enjoyed giving the scholarshipand helping the student she had met. She spoke sohighly of the experience that when she retired fromAlmost Family they honored her and her careerthrough a generous gift to the Gormans’ scholarshipfund. Since then, the Vicki Beekman GormanUndergraduate Nursing Scholarship has helped fivestudents pursue their dreams of becoming nurses.
Recently, the Gormans helped in a new way. Theyhosted a dinner in their home and invited UK nursingstudents, Louisville health care professionals and evenUK Board of Trustee member Jim Stuckert and hiswife, Diane, for the first “Dinner and Dialogue.”
“The evening was a wonderful success,” said InterimDean Patricia B. Howard, PhD, RN, NEA-BC, FAAN.“The students were able to meet and network with a varietyof health care professionals in a comfortable setting.”
Mrs. Gorman also sits on the College of Nursing’sDean’s Advisory Council. “It feels good to be involvedwith the college and to see all the changes in hownurses are educated today. It really is all about students!And we are hooked on helping them succeed.”

Membership is given to deserving undergraduate and graduate students, and professionalnurse leaders. This year we inducted 15 undergraduate students, six graduate studentsand one nurse leader. Delta Psi is always open to nurses who want to join and learn howthey can enhance their own learning and network with other nurses who share the samevision of enhancing the profession. 2012-2013 has been a great year for the Delta Psi chapter! Not onlyhave members continued to pursue excellence in nursing through their research, teachingand staff development, the chapter received the 2011 Chapter Key Award from Internationalto reward the chapter for the excellent work by our members to advance nursing throughresearch, excellent programs, membership recruitment and retention, collaboration withother nurses at the local, national and international level and professional/leadershipdevelopment. We also had the privilege of having Dr. Suzanne Prevost as a Delta Psi memberwhile she served as the STTI international president for 2011-2013. She represented ourchapter very well and did a fantastic job as the international president. We are so proud!
The Out-of-the-Gate Event was held in August 2012 to help orient new members andencourage networking among current members. The chapter funded this meeting, which washeld at Shillito Park and we had a good turnout of members. This is being considered as anannual event to be held the weekend after the fall semester begins.
The Fall Scholarship Meeting was held in October 2012 at the SpringHill Suites near UK’scampus. Dr. Lisa Turner, a past research award recipient, gave a great presentation on obesityand long-distance truck drivers.
Our annual Christmas Dinner and Silent Auction was held in December 2012 at the HilaryJ. Boone Center on UK’s campus and many members attended. More than $1,700 was raisedthrough the auction, of which 100 percent went to funding nursing research and scholarship.Members continue to be generous, donating items for the auction and bidding on variousitems. The Scholarship and Award Committee gave about $7,500 in awards between the falland spring applications. All members are encouraged to apply for awards and scholarships.
The Spring Business Meeting was held February 2013. Dr. Kristin Ashford gave awonderful presentation on her nursing research focusing on the effects of tobacco smokeon pregnancy and preterm labor. We had a Theta Nu member, as well as several studentsalso in attendance.
The year ended with the Induction Dinner and Ceremony, held April 2013, at the CentralBaptist Education Center. Mendy Blair, current Leadership Succession Chair and 2009-2011Delta Psi president, was the keynote speaker. She shared the story of her nursing journey, atruly inspiring one for both new and current members.
Members were also very generous by participating in several service projects this year,benefitting the Humane Society, the Simon House, Water.org, Lexington’s VA Hospital andthe Down Syndrome Association of Central Kentucky. Members donated money, books, babysupplies, personal care items and pet food/supplies. Delta Psi typically supports a service projectat each Delta Psi event throughout the year. We welcome any future service project ideas!
