SECTION 1 – IntroductionThis directory provides a list of Inter Valley Health Plan’s network providers.

You will have to choose a network provider, that is listed under Primary Care in this directory, to be your Primary Care Provider (PCP). The acronym “PCP” will be used throughout this directory. A PCP is a physician who meets state requirements and is trained to provide routine or basic medical care. Please note, when you choose your PCP, you are also choosing a medical group, and its’ contracted hospital(s) and specialist(s).

Your PCP will coordinate all your health care needs. There may be times when your PCP feels you need specialized treatment and in those cases, you must receive a written referral from your PCP to see a specialist. Once your PCP’s referral request is approved, an appointment can be made with the specialist. Be careful not to receive care from a specialist without a written referral from your PCP, as you may be responsible for payment of those services.

It is important to know which providers are

in our network because with limited exceptions, you must use network providers to obtain your medical care and services. Some of our provider groups use sub-networks which may require you to use certain specialists. Not all specialists in your physician’s provider network may be available for referral. To learn which specialists your PCP utilizes, you will need to contact your PCP directly. The only exceptions are emergencies, urgently needed services when a network provider is not available (generally, when you are out of the area), out-of-area dialysis services, and cases in which Inter Valley Health Plan authorizes the use of out-of-network providers.

If you get a bill for covered service from a contracted provider (and in some cases a non-contracted provider), you may submit the bill to us for reimbursement. We will pay for our share of the bill and will let you know if you must pay any cost- sharing. However, if you have already paid for the covered services, we will reimburse you for our share of the cost.

Emergency Care: Our Plan covers emergency services, whether or not they are provided by a contracted network provider. If you have a medical emergency, get help as quickly as possible. Call “911” for help or go to the nearest emergency room, hospital, or urgent care center. Call for an ambulance if you need it. You do not need to get approval or a referral first from your PCP.

As soon as possible, make sure that your PCP is informed of the emergency visit. He or she will need to follow up on the emergency care you received. In addition, you or someone else should call our Member Care Team and inform us about the emergency care you received, and if possible within 48 hours.

After the emergency is over, you are allowed follow-up care to monitor your condition and that it continues to be stable. Your follow-up care will be covered by our plan.

If your emergency care is provided by out-of-network providers, we will try to arrange for network providers to take over your care as soon as your medical condition and the circumstances allow.

Emergency care can always be obtained in or out of the service area from the nearest available provider. You do not need to get approval or a referral from your PCP.

Urgently Needed Care: Urgent Care is a non-emergency situation when you need medical care right away because of an illness, injury, or condition that you did not expect or anticipate, but your health is not in serious danger.

Our plan covers urgently needed care as long as the care you receive meets the definition of “urgently needed care” given above.

Urgent Care: When in the service area, you must use plan providers for urgent care. When outside of the service area, urgent care may be obtained from the first available provider.

Dialysis Care: In addition, when outside the service area, you can obtain dialysis treatment from any qualified provider.

SECCIÓN 1 – IntroducciónEste directorio provee una lista de los proveedores de la red de Inter Valley Health Plan.

Tendrá que elegir uno de nuestros proveedores de la red que aparecen en este directorio para que sea su PCP o médico de atención primaria. En este directorio se utilizará el término “PCP”. Por lo general, usted debe obtener sus servicios de atención médica de su PCP. Su PCP es un médico que cumple con los requisitos estatales y está capacitado para proporcionar una atención médica básica. Usted obtendrá la mayor parte de su atención rutinaria o cuidados básicos de su PCP.

Su PCP será su médico personal que coordinará también el resto de los servicios cubiertos que usted obtenga como miembro de nuestro Plan. Por ejemplo, le recomendamos que consulte con su médico de atención primaria si usted cree que necesita atención especializada. Aunque su PCP coordinará todas sus necesidades de salud, quizás haya ocasiones cuando él o ella crea que usted necesita tratamiento más especializado. En tal caso, usted debe recibir de su PCP una referencia por escrito para poder ver a un especialista. Una vez aprobada la solicitud de referencia de su PCP, podrá solicitar una cita con el especialista. Tenga cuidado de no recibir atención

de un especialista sin una referencia escrita de su PCP, porque de hacer eso, usted será responsable del pago de esos servicios.

Es importante saber qué proveedores forman parte de nuestra red, ya que, con excepciones limitadas, mientras sea miembro de nuestro plan, usted debe utilizar los proveedores de la red para obtener atención y servicios médicos. Nuestro plan cubre la atención de urgencia, siempre y cuando la atención que reciba

cumple con la definición de “atención de urgencia” ya mencionada.

Atención de urgencia: Cuando esté en el área de servicio, usted debe utilizar los proveedores del plan para la atención de urgencia. Cuando esté fuera

del área de servicio, la atención de urgencia puede obtenerse del

primer proveedor que esté disponible.

Atención de diálisis: Además, si está fuera del área de servicio, puede obtener tratamiento de diálisis de cualquier proveedor calificado.
