We've filled our wellness program with resources and new incentives in 2021 to support you and your family's health journey. This voluntary program gives you the tools to get active, stay healthy and live better every day. Employees and spouses/domestic partners enrolled in a Mosaic health plan can participate in this free program. Each of you can earn $500, an activity tracker and more!

For employees and spouses/domestic partners enrolled in a Mosaic health planYou have an opportunity to earn up to $500 each by completing any combination of the activities listed below. Incentives earned will be added to the employee's paycheck approximately 4-6 weeks following completion of the activity. Online Health AssessmentThis short, confidential survey assesses your health across seven factors. Get a personalized report and actions to take. Complete the assessment each year to earn your incentive. Incentive can be earned one time per calendar year.

Personal Health ScreeningGet a snapshot of your cholesterol, blood pressure, blood sugar level and BMI to help you track progress toward your goals. Screenings can be completed with a personal physician, convenience care clinic, Quest lab or at home using a Quest Lab Kit. Visit benefits.mosaicco.com for a step-by-step guide and deadlines. For an in-network location near you, call Cigna at 800-244-6224. Incentive can be earned one time per calendar year.

Track Steps or Active MinutesHave you ever wondered how many steps you take a day or month? Download the Virgin Pulse app and follow the steps to connect your activity tracker. Once connected, track activity for 20 days a month and earn $10 in payroll credit per month. Incentive can be earned 12 times per calendar year.

Preventive Care VisitsPreventive care is one of the easiest ways you can help yourself stay healthy. Complete any combination of the following exams in 2021 and earn $100. Incentive can be earned two times per calendar year:• Annual Physical• Prostate Screening• Colonoscopy• Preventive OB/GYN Exam• Mammogram• Dental Cleaning• Vision Exam

Cigna Coaching ProgramWork with Cigna's Personal Health team to make progress toward your personal health goals (i.e., nutritional counseling, tobacco cessation, weight management). Call Cigna at 800-244-6224 or visit www.myCigna.com to get started. Incentive can be earned one time per calendar year.

Community Wellness Events Participate in organized community wellness events (i.e., Relay for Life, American Heart Association Walk, 5K). Once you’re done, sign in to your Virgin Pulse account and navigate to the Benefits page to find the Community Wellness Event attestation form. Complete the form to earn your $50 Pulse Cash! Reward can be earned four times per calendar year for a total of $200 in Pulse Cash.

Company Step Tracking ChallengeLooking for a little friendly competition? Watch for details on step challenges later this year. Participate in a fun step tracking challenge where you can earn prizes! Reward can be earned up to two times per calendar year for a total of $200 in Pulse Cash.

New! Whil Video TrainingsWhil is an activity-based learning system to help reduce stress, increase resilience and improve mental wellbeing through a series of self-paced, on demand mini-courses. Complete a Whil video session at least 10 days in a month to earn $10 in Pulse Cash per month. Reward can be earned 12 times per calendar year for a total of $120 in Pulse Cash.

Although we may use aggregate information collected to design a program based on identified health risks in the workplace, the wellness program will never disclose any of your personal information either publicly or to Mosaic, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information may never be used to make decisions regarding your employment. You will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements.

You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.
