ORDERING INFORMATION ­ Help make this process easier! Please read the following!

• Only FOP members can order tickets/submit payment. (no order forms from non-members)• Current FOP membership will be verified prior to order processing. (dues up-to-date…no lapsed membership)• Tickets for non-member (guests) must be tallied into the members’ order form (one order form from the member)• Payment for any non-member’s tickets must be made through the member’s check (no non-member checks)• Tickets will only be mailed to the members’ address as listed on the order form (no secondary address mailings)• Ticket Order deadline….Received by Thursday, July 7th, 2016 (have to give the park a final guarantee) • All mail orders will be sent out within 2 days of receipt….no tickets mailed out after July 9th, 2016. • Returned checks must be replaced with cash/money orders (including bank fees) prior to ticket distribution• The NYSFOP / Foundation / Great Adventure are not responsible for any tickets that are: lost, stolen,

delayed/misdirected/returned in the mail, misplaced, destroyed, detached, voided, defaced, not received or left athome and unfortunately cannot replace / refund any tickets.

• If any of the above occur, you must purchase a replacement ticket (if available)• Since tickets are on consignment to the NYSFOP / Foundation, we are financially responsible for them.• All payments are considered to be an unrestricted donation to the NYSFOP Foundation.• Want to avoid the postage fee?
