HSA Bank Transition On October 24, your HSA banking will transition from JPMorgan
Chase Bank to HSA Bank Replacement debit cards will be sent to your homes mid-
October All balances will automatically be transferred to your new card –
there are no blackout dates Begin using the new card on October 24. The Chase card will
be deactivated at the same time You can still access all account information on mycigna.com If you currently have automatic claim payments with Chase
Bank, you will need to re-enroll after HSA bank transition
Existing Wellness Resources Telephonic Coaching – call 1-800-Cigna24 at anytime to work
with a coach to achieve your health and wellness goals. Includes chronic condition management Weight, Tobacco, Stress management Wellness, exercise, nutrition, etc.
Mycigna.com resources: Health Assessment Online Coaching – My health Assistant Apps and activities on mycigna.com Look up benefits, find a doctor/dentist, cost transparency
tools, manage claims, etc.
New Programs
Healthy Pregnancies, Healthy Babies – Voluntary program for pregnant women that provides incentives for following certain guidelines.
Cancer Support Program – Provides care and support for employees and families touched by cancer.
Personal Health Solutions Plus – Ensures that employees and their dependents receive the most appropriate and cost effective inpatient and outpatient care. Precertification required for certain outpatient procedures and services

All eligible employees must sign-in to confirm their elections
Elections go into effect on October 1, 2015
