2014-15 Annual Report

We like to express a heartfelt thank you to all who contributed to Oak-Leyden’s successful 2014-2015 fiscal year. Formed as the Oak Community School in 1956 by a group of concerned parents, Oak-Leyden continues to advance our founders’ mission. We provide those with developmental disabilities with the training, therapies, and education they need to help meet life’s challenges and reach their highest potential.

Through the talents of our staff and the support of our families and community, Oak-Leyden has grown significantly over the past 60 years. We now assist more than 700 children and adults annually in western Cook County with a variety of services that promote a sense of inclusion within a larger community. This report illustrates the impact Oak-Leyden and its partners have made on our programs and services over the past year.

Our Adult Services Program achieved Department of Human Services licensure at 100% and successfully completed the Bureau of Quality survey. Our Children’s Services Program received 100% by the Department of Human Services’ Early Intervention Monitoring team. This year, an impressive 84% of the financial contributions we received went to directly fund our programs – far exceeding the Better Business Bureau’s Wise Giving Alliance’s minimum recommendation of 65%.

Like many of the non-profit agencies in the social impact sector this year, we experienced challenges with the availability of state funding. Now more than ever, your support is critical to ensuring Oak-Leyden continues to make an impact for our families and community.

Children's Services

Oak-Leyden’s Music Together® program instructed dozens of newborns to five-year-olds of all abilities and encouraged each child to participate at their own level for an enlightening experience. Our popular drop-in programs celebrated holidays, hosted readers, offered can-do craft projects, and modeled healthy social skills.

Families of children with developmental delays or disabilities receive coaching, support, and services to help them thrive.

Helping children with developmental disabilities or delays is a team effort. We worked with families and siblings to overcome physical, mental, and social hurdles through activities they learned in our sessions. Family members actively participated in our therapies with professional guidance and took away new skills they can use every day.

• In May, Oak-Leyden awarded 98 diplomas to our Children’s Program’s graduates. Their families and therapists were proud of their accomplishments and our graduates are now better prepared to take their next steps with confidence.

• Our Children’s Services Program enjoyed the opportunity to expand its therapy services during our first full season in our new outdoor play space.

• Therapists in our program retrofitted four GoBabyGo ride-on, battery-operated cars for the use of our children with limited mobility.

• We hosted a variety of offsite outings for our program’s families. Children made new observations and built confidence through field trips to the Brookfield Zoo, WonderWorks, and the Trailside Museum.

• Grants awarded to our program provided for the purchase of new instruments for Music Together and a terrific climbing apparatus. Private donations funded our new ball pit and a wide range of creative toys and craft supplies.

In FY2015, our Children’s Services Program assisted 366 children in 30 communities. Oak-Leyden’s unique, multi-disciplinary team approach encouraged collaboration among developmental, physical, speech-language, and occupational therapists to assist with the holistic development of our program’s participants.

Community Living ServicesOur 12 group homes created safe and supportive environments for each of our residents. With low residence turnovers and a caring 24/7 staff, our clients developed friendships, respect, and independence. Our residents learned new skills through cooking, housekeeping, personal upkeep, and connecting with their neighborhoods.

• Our Commission of Accreditation of Rehabilitation Facilities (CARF) Accreditation Survey received an excellent rating, with zero recommendations for improvements to our residential services.

• Oak-Leyden filled three open residential beds and ended the fiscal year with only one open bed.

• We converted our Oak Park group home from an eight-bed to a seven-bed residence, providing each resident with a private bedroom.

• The adult programs contracted with Applied Behavior Analysis of IL for licensed Behavior Analysts to develop Individual Behavior Support Plans for our group home and Lifelong Learning Center participants, train staff on the programs, and track and report progress.

• We held holiday parties for all residents at Villa Brunetti and Bear Manor in Oak Brook.

The unique treatment strategy of our Multidisciplinary Clinic ensured the successful care of our clients by linking together members of each client’s medical team. This facilitated smooth communication, effective problem solving, and less intrusive treatment.

• Our CARF Accreditation Survey received an excellent rating, with zero recommendations for improvements to our clinical services.

• Oak-Leyden performed annual safety training with all division staff and a licensed dietician trained staff on our residents’ special diets.

• We worked with Walgreens to provide onsite flu shots to all residents.

• A Department of Human Services Nurse Trainer held several Medication Administration Training (MAT) classes to educate all group home staff on administering medications.

EXCEEDING EXPECTATIONS

Our Adult Services Program empowers participants to build confidence, develop new skills, and

integrate with the broader community.

• The clinic team presented at a parent night at Oak Park/River Forest High School for Oak-Leyden's residential and clinical services.

• The clinic contracted with Therapy Providers to offer evaluations and access to physical therapy, occupational therapy, and speech therapy for our CILA residents.

Lifelong Learning

Our program’s staff worked one-on-one to help our clients identify skills they would need to reach their goals and dreams. From field trips and cooking instruction to technology training and healthy eating habits, Oak-Leyden ensured our day program was welcoming, educational, and engaging for each participant.

• Our day program participants averaged 2.5 community activities per month.  Visits to the beach, park, museums, festivals, concerts, ice skating shows, zoo, and barber as well as shopping trips, lunch outings, architectural tours, and gardening, connected our clients to their communities and provided social and educational opportunities.

• 90% of our program participants met or exceeded their goals of daily living.  Skills such as memorizing their addresses and phone numbers, carrying Identification, learning to count change, following a budget, looking for a job, reading a newspaper, listening to and reporting the news, and learning to do laundry and dishes, were among the many steps they took toward independence.

• Health and safety was a program emphasis this year.  Each day started with exercise.  Many of the daily classes were devoted to topics of healthy eating, community safety, and relaxation/meditation. Day service buildings added alarmed doors and video cameras.  The Office of Inspector General gave us a clean bill of health and Licensure found us in 100% compliance with all safety requirements.  All staff carried valid first aid and CPR cards and the number of incidents requiring more than simple first aid was at an all-time low.

• Our program was managed with exceptional efficiency.  Budget targets were reached. All who applied for service found easy access to the program, and no one lost service that desired it. A survey of our participants and family members indicated that Oak-Leyden “exceeded expectations.” 

Oak-Leyden’s Supported Employment Program identified and developed skills for our participants who are ready and eager to join the workforce, and mentored those who are currently employed. Working hand-in-hand with our SEP participants, their families, and their employers, ensured they made smooth transitions, found the ideal fit, met employer expectations, and developed satisfying working relationships.

Event Highlights for FY2014-2015 27th Annual Benefit: Oaktoberfest

• A capacity crowd of 200 attended the event, hosted by Presenting Sponsor, Hofbräuhaus Chicago.

• More than $70,000 was raised for our programs and services.

• The Oak-Leyden Adult Choir wowed the crowd on the Hofbräuhaus’ main stage.

The following acknowledgements were presented:President’s Award – IFF for providing financial assistance, and construction and project management consultation for our Elmwood Park Lifelong Learning Center.

Volunteers of the Year – ACH Food Companies, Inc. for their annual holiday gift collection on behalf of Children’s Services families in need.

Mighty Oak Award – Alexis Colter

Special Honoree – Chris Caponigri, a long-time friend of Oak-Leyden for his donations ranging from vehicles to office furniture.

Dine for a CauseA special “Chef and Brewer’s Tasting” dinner at Hofbräuhaus Chicago raised nearly $10,000 for our Children’s Services Early Intervention Program.

Community EventsOak-Leyden had the pleasure of participating in a number of promotional events with our local businesses – thanks to Starship Restaurant, Ten Thousand Villages, and Carson’s for helping raise funds and awareness for our programs!

Every donation makes a difference in the lives of our participants. Our financial contributors help bridge the gap between state, federal, and insurance funding and enable us to continually improve the types and number of services we provide.

Donation OpportunitiesFrom online donations and matching gifts to real estate, charitable trusts, and memorial funds, 84% of every monetary gift goes directly to our programs that enrich Oak-Leyden’s clients’ lives.

Corporate ContributionsOak-Leyden’s Annual Corporate Partner Fund presents local businesses in our neighboring communities an opportunity to make yearly contributions to help sustain our programs. We also seek Corporate Sponsors for our fundraising events. Such contributions offer high-profile recognition and help us keep costs to a minimum.

Wish ListWe have a wide range of needs for new or gently-used items. Our wish list is updated throughout the year at www.oak-leyden.org for items such as art and craft supplies, toys, housewares, toiletries, office supplies, electronics, and gift cards.
