Dear Student-Athlete,

On behalf of the Department of Athletics, I would like to welcome you to Weber State University - the home of the Wildcats. We are very excited to welcome our new student-athletes as well as welcome back our returners. Each and every one of you is an important part of this University and Deparment of Athletics.

We are looking forward to a year of outstanding achievement by our student-athletes in the classroom and on the field/court of competition. Our staff of coaches and administration is committed to the overall vision and mission of the University. The Department strives to have its student-athletes reach their full potential in all areas of the collegiate experience and focuses on ensuring you receive the support necessary to be the very best student and athlete possible.

Competing at the highest level of intercollegiate athletics and experiencing Wildcat Pride is something you will develop throughout your time at Weber State. Your primary purpose for being here is to receive a quality education. There are many people counting on you to work hard, study diligently, and give your utmost to achieve your full potential, but the most important motivator in your life must be yourself.

As a student-athlete at Weber State, you have the serious responsibility of assuming a leadership role as a member of your athletic team. Because of your high visibility as a student-athlete, you are expected to uphold our University’s guiding values of integrity and honesty and exhibit sportsmanship, self-discipline, and personal responsibility.

Dear WSU Student-Athletes,

As President of the 2011-2012 Student-Athlete Advisory Council (SAAC), I am greatly privileged to welcome you as a Wildcat to Weber State University. It takes hard work and dedication to your respective sports on and off the field to achieve the great success all of you have displayed. With these achievements, I am delighted that you will be a part of our athletics department and want you to know that each and every one of you will play a vital role in proudly representing Weber State and our athletics programs in the Big Sky Conference and on the national level.

As student-athletes at Weber State, we are given so many incredible opportunities to grow and learn academically, athletically, and socially. Our administration, coaches, faculty, and staff really take the time to put our best interests first. With that being said, it is important that you know about and use your campus and athletics department resources and, if you have any questions, just know that there are many great mentors and peers you can reach out to.

SAAC is one of the many resources you can reach out to. Just so you know a little about SAAC, we are made up of many of your peers/teammates whose purpose is to serve as your liaison and voice as students-athletes on campus as well as with athletics administration. Our mission is to enhance the total student-athlete experience by promoting opportunity, protecting student-athlete welfare, and fostering a positive student-athlete image. I hope you have the opportunity at the beginning of the school year to get to know your SAAC representative(s) as they will be the voice for your team and can bring the committee your issues and needs to be addressed. As a collective group, the SAAC members and I encourage you to get involved in SAAC and campus activities. It is our hope that we as a council can provide you with fun and exciting opportunities for you to get to know other student-athletes, coaches, the wonderful staff of the athletics department, and our supporting community.

As your fellow student-athlete, I encourage you to set your goals high and strive to reach for the stars with success in all your endeavors here at Weber State, whether they are on or off the field of play. Always keep your head up and be proud of the things you do to represent yourself, Weber State University (and now as a Wildcat). Lastly, as student-athletes, we must know what our priorities are, and we must strive to accomplish the little things every day. Again, I congratulate you and wish you the best of luck in your studies and respective sports this upcoming year.

Intercollegiate Department of AthleticsMission Statement

The mission of the Department of Intercollegiate Athletics is to support the greater mission of Weber State University, which is to meet the educational needs of Utah. This is accomplished by stimulating and improving athletics programs for students that are designed to develop and promote skills that assure an excellent chance of success in athletics participation, college, and career. The Department of Intercollegiate Athletics enhances the development and success of student-athletes within a diverse and inclusive environment by supporting equitable opportunities for all, including women and minorities, and by acting affirmatively in the recruitment and selection of athletes, coaches, and staff who are representative of society.

The goals of the Department of Intercollegiate Athletics are to:

• Support student-athletes’ intellectual development and graduation;

• Prepare student-athletes for a lifetime of productive achievement pursuing excellence through personal development and teamwork;

• Enhance the health and safety of student-athletes through compliance with sports medicine guidelines and development of physical fitness and athletics skills;

Student-Athlete Code of Conduct

The Department of Athletics at Weber State University (WSU) expects all student-athletes to conduct themselves in a manner that creates a positive image for the Department of Athletics, the University, and its teams. Participating as a student-athlete at WSU is not a right, but a privilege. The University and Department of Athletics remind you that, as a member of the WSU Intercollegiate Department of Athletics, you are a representative of the University and are always in the public eye. The following Code of Conduct clarifies the behavior that is expected of WSU student-athletes.

I. Standards of Conduct

A. Compliance with Laws, Rules and Regulations – Student-athletes who participate on the University’s NCAA athletic teams are required to comply with WSU’s Student Code, regulations related to student housing, other rules, and regulations pertaining to student life, along with all state and federal laws. This policy is supplemental to and does not supersede these other laws, rules, and regulations. The Department of Athletics reserves the right to impose sanctions as outlined in this policy that are additional to those imposed under the laws, rules, and regulations referenced above.

B. Specific Prohibitions – Student-athletes who participate on the University’s NCAA athletics teams are prohibited from engagin in behavior listed below. This is not a comprehensive list, and the Department of Athletics reserves the right to sanction student-athletes for behavior not specified here, but deemed to be of equivalent seriousness.

1. Academic Dishonesty – The Department of Athletics will not tolerate acts of academic misconduct, which include, but are not limited to: cheating, falsification/fabrication, tampering, plagiarism, facilitating academic misconduct, multiple submissions, and other academic misconduct.

2. Unexcused Class Absences and Poor Academic Performance – Student-athletes are expected to attend class regularly and punctually, and to fulfill course load and academic performance requirements. Failure to meet such obligations may affect eligibility to participate in practices and/or competitions, scholarship eligibility, and academic standing.

3. Alcohol – Prohibited acts include any and all of the following: a) possessing or consuming alcohol if under the age of 21; b) operating a motor vehicle while under the influence of alcohol;

c) misrepresenting one’s age for the purpose of purchasing, possessing, or consuming alcohol; d) purchasing, furnishing, or serving alcohol to or for an underage person; e) public intoxication; f ) violations of additional team rules regarding alcohol use; or g) consumption of alcohol during team travel or team functions.

4. Other Drugs – Prohibited acts include a) possessing or using illegal substances, as defined by law; b) possessing or using controlled substances, as defined by law; c) distributing, selling, or possessing with the intent to distribute illegal or controlled substances, as defined by law; or d) the use of tobacco products by a student-athlete during practice, competition, or other team functions.

5. Hazing – Hazing, as defined by Utah law, is strictly prohibited.

6. Web-based Social Networking – Student-athletes are permitted to have profiles on social networking websites provided that a) no offensive or inappropriate pictures are posted; b) no offensive or inappropriate comments are posted; and c) any information placed on the website(s) does not violate the ethics and intent behind the WSU Student-Athlete Code of Conduct. Materials posted on the social networking sites of WSU student-athletes cannot contain language or images that are damaging to the individual’s or Weber State’s reputation.

7. Violence – Student-athletes are prohibited from engaging in violent or aggressive acts, including assault on a person or property, harassment, or stalking.

8. Sexual Assault – Student-athletes are prohibited from engaging in sexual harassment or intimidation, rape, domestic violence, or other related violations.

9. Disrespect/Poor Sportsmanship – At all times, student-athletes are required to conduct themselves in a manner respectful of themselves, their team, the University, the Department of Athletics, game officials, opponents, and property.

10. Gambling and Bribery – Student-athletes shall not knowinglya) provide information to individuals involved in organized gambling activities concerning intercollegiate athletics competition; b) solicit a bet on any intercollegiate team; c) accept a bet on any team representing the institution; d) solicit or accept a bet on any intercollegiate competition for any item that has tangible value (e.g. cash, clothing, equipment, meals); or e) participate in any gambling activity that involves intercollegiate athletics or professional athletics through a

bookmaker, a parlay card, or any other method employed by organized gambling. Violations of this policy also break NCAA rules and will affect the student-athlete’s eligibility.

C. Team Rules and Obligations – Student-athletes must comply with team-specific rules as communicated by team coaches, complete training requirements, arrive promptly for team practices, meetings, and departures for away games and other team events attend team functions, and otherwise fulfill their responsibilities as team members. The head coach must excuse absences in advance unless an unforeseeable emer-gency arises. Sanctions for violating this section shall be determined by the head coach.

II. Reporting

A. Each student-athlete is obligated to report his or her own violations of this code of conduct to the team’s head coach and/or the Director of Athletics within 24 hours of such violation. This obligation is in effect year-round, including semester, summer and vacation breaks.

B. Student-athletes who do not self-report their code of conduct violation to their head coach and/or the Director of Athletics will receive stricter sanctions.

C. Student-athletes, Department staff, and coaches have an obligation to report any violations or potential violations of the Code of Conduct to their head coach and/or the Director of Athletics.

D. The Director of Athletics or any head coach that is made aware of a violation of this policy that may constitute criminal activity is obligated to report the violation to the WSU Police Department.

III. Sanctions

The Department of Athletics’ disciplinary process is independent of, and supplements the University judicial process. Sanctions levied by the WSU Department of Athletics may occur over and above those levied by the University. Sanctions will vary depending on the type and circumstance of the violation.

A. Sanctions for violations of sections IA or IB of this policy may include, but are not limited to, warning, probation, community service, required educational or counseling sessions, drug testing, restitution, temporary suspension, suspension, expulsion, and/or non-renewal of financial aid as provided by the NCAA.

B. Violations of sections IA or IB of this policy not involving criminal charges will result in sanctions that are to be determined by the head coach in consultation with the Director of Athletics.

C. Violations of sections IA or IB of this policy involving criminal charges will result in sanctions according to the following criteria:

1. Student-athletes charged with a felony or serious misdemeanor (usually Class A) will be suspended from all team activities until the charges have been dismissed, changed, or a plea or judgment has been determined.

Once the charges have been dismissed, changed, or a plea or judgment has been determined, a committee made up of the Director of Athletics, Faculty Athletics Representative, and Dean of Students will conduct a hearing and determine the appropriate sanctions to be administered. As part of the hearing, the committee may meet with the student-athlete and other individuals (as available) who have relevant information about the incident(s).

2. Student-athletes charged with a less serious misdemeanor (usually Class B or C) will be subject to sanctions as determined by the Director of Athletics in consultation with the responsible University Vice President.

D. Student-athletes subject to sanctions according to Section IIIC of this policy have the right to appeal such sanctions to the Vice President for Administrative Services.

The Intercollegiate Athletics Student-Athlete Code of Conduct is applicable to all student-athletes on the current team squad list for actions that occur while matriculated at Weber State University or that are prosecuted while enrolled as a student-athlete at the University.

Athletics Study Hall & Accountability Policy

Elizabeth Hall rooms 213 & 215Monday through Thursday, 6:30 pm to 8:30 pm

The Study Hall program is designed so that all student-athletes have the opportunity for a supervised atmosphere in which to study. Mandatory participation in the Study Hall program will be determined by the student’s coaches and the Academic Services staff. The Study Hall atmosphere is the same as a library. Come prepared to STUDY and LEARN. The Study Hall is available for ALL student-athletes. If students would like a quiet, structured time to study, or if they desire to work with a tutor to improve their understanding of a class or increase their grade level (even if they are not struggling), students are encouraged to take advantage of the resources available through this program.

Student-athletes will be required to attend the Study Hall under the following conditions:

• The student has below a 2.75 cumulative, H.S., or transfer GPA;• The student has below a 3.0 Weber State cumulative GPA;• The student is required by an advisor, staff member, or coach to be there;• The student still needs, or is enrolled in any, developmental math or English class(es), which includes any 900 level class and Math 1010; OR• All student-athletes attending a developmental class are required to meet with a tutor twice a week during the ENTIRE term to maintain progress in the class.

RULES

• Arrive on time so as not to disrupt other individuals.• Turn cell phones and other electronic devices on silent or vibrate, or use head phones (making sure the volume can be heard only by the one wearing the headphones) • Sign in and note which subject(s) you will be working on. The monitor has the authority to check work completed at the end of the evening to see if student-athletes accomplished their stated goals.• Bring all necessary items, including textbooks, pens, paper, laptops, etc., for academic use only.• No tobacco products or other NCAA banned substances are allowed in Study Hall.

• Food and drink must be eaten quietly and cleaned up.• Pick up all garbage and keep rooms neat and clean.• Be courteous and respectful to other students, staff, and tutors.• No loud talking or inappropriate language (i.e. swearing, vulgar comments or gestures, ethnic/racial or gender-based name calling, comments, or insults, etc).

Contact your Academic Advisor in Athletics to let them know if you are unable to attend Study Hall due to illness or injury. The coaches are not in charge of the Study Hall; it is an Academic Services resource.

Student-athletes must obtain PRIOR permission to work in small groups, and/or to attend the computer lab during these study periods. Access to the computer lab during these study periods is ONLY for academic purposes. This is not a time to surf the internet or chat on Facebook. Student-athletes who are caught doing something other than class work will NO LONGER BE ALLOWED to utilize the computer lab during these study periods.

Any student-athletes who break Study Hall rules will be held to the Accountability Policy (see below). All student-athletes, especially those required to attend Study Hall, are required to accept this Policy. Academic Services personnel, especially those directly involved in Study Hall, and/or other staff or campus personnel have the right to ask student-athletes to leave the building and/or the program if:

• The student-athlete is disruptive or disrespectful to other students, tutors, staff or campus personnel;• The student-athlete breaks ANY of the Study Hall rules; OR• Academic Services personnel, especially those directly in charge of this program, feel that it is in the best interest of the offending student-athlete, other students, and/or the Study Hall program.

Study Hall Accountability Policy

• First Violation – verbal warning, head coach notified.• Second Violation – written warning, head coach notified. Two-day suspension from Study Hall. Required study hours will be made up at another time and monitored by Academic Services personnel.• Third Violation – Second written warning, head coach notified. One-week suspension from Study Hall. Required study hours will be made up at another time and monitored by Academic Services personnel.

• Fourth Violation – Dismissal from the Study Hall Program. Head coach notified. Other study arrangements will need to be made between the coach and Academic Services personnel.

Tobacco Product Use Sanctions

• First Violation – The student-athlete will be required to meet individually with Hanalee Hawkins, Prevention Specialist/Drug & Alcohol Program, for a drug education session.• Second Violation – The student-athlete will be suspended from the NEXT competition.

Tutoring ServicesWriting Center (Elizabeth Hall room 210)

Students in developmental math (math 1010 or lower) are required to go to The Hub (Lampros Hall) for math tutoring services. Time spent in The Hub above and beyond what is required by the math class will be counted towards study hall totals for each week.

On Tuesday and Thursday evenings, a tutor is available for math 1030 and above.

Tutoring for English is available at the Writing Center during Study Hall through the Athletic Academic Services Program. The Writing Center is also open during the following hours to assist students outside of study hall times. (Monday –Thursday 9:00 am-8:00 pm, Friday 9:00 am - 3:00 pm, Saturday 10:00 am - 1:00 pm)

There are several tutoring services available on campus as well. Many of these services are free to students at Weber State (see CAMPUS SERVICES – Academic Support Centers and Programs, page 48-49 in this book for a detailed list). Likewise, each department provides tutoring services for many of their classes. You can ask your instructor or the department secretary if you wish to take advantage of these services.

If tutoring needs are not met through the campus services provided, the Department of Athletics will attempt to provide tutors. It is important that student-athletes arrange all of their tutoring needs through Athletic Academic Services personnel. A Tutor Request Form can be obtained from the Athletic

Coordinator if needed.

It is important to remember that the use of a tutor is to help in understanding course materials. The tutor is not meant to, nor should he/she be asked to, do the work for student-athletes. Any improper use of the tutoring services provided is a violation of NCAA rules and will be dealt with immediately according to set policies and procedures. If questions or concerns arise about the use of tutors, please contact Academic Services personnel.

Weber State provides certified tutors, on campus and through the Athletics Study Hall Program. Only certified tutors will be allowed during the Study Hall’s hours of operation.

Rules for Tutoring Services:

• Tutors will assist with homework questions. They will NOT give the answers.• Tutors will help student-athletes study for exams. They will NOT give out old exams or answers.• Tutors will help student-athletes explore ideas for papers and projects. They will NOT give out old papers to use.• Tutors will help student-athletes edit/format papers. They will NOT write or type papers for students.

Should a tutor violate the Department of Athletics Academic Services’ rules regarding reasonable assistance, they will be fired immediately and the student -athletes will be declared ineligible. Should a tutor be suspected of assisting a peer too much, the coach and/or the Academic Services personnel should be notified immediately.

Should a student-athlete violate the Department of Athletics Academic Services’ rules regarding working with tutors and/or keeping the Study Hall rules, the student-athletes will be subject to sanctions as noted in the Study Hall Accountability Policy & Sanctions.

Academic Area, Stewart StadiumThe Stewart Stadium academic and computer area has been provided for student-athletes to enhance their academic success. It is intended to be a place for quiet study where ALL students feel comfortable and can accomplish their academic goals. To provide a positive study environment for all students, the following rules will be enforced:

• Study quietly, independently, or in small groups.• Use positive and non-offensive language and/or comments.• Profanity, vulgar comments, and/or ethnic or gender-based comments will NOT be tolerated.• Computers and the printer are for academic use only. Please print only items needed for academic purposes. Student-athletes using email, social networking, and other non-academic websites will be asked to give up their computer if someone needs it for academic purposes. If computers are not currently needed for academic purposes, students are welcome to use them for personal access.• Banned substances are NOT ALLOWED in ANY academic area at this university. Substances may include, but are not limited to tobacco products, alcohol, and illegal drugs.

twice a semester. All information received through this process from the instructors regarding your class status will be shared with Athletic Academic Advisor(s), compliance staff, coaches, and other applicable parties.

Academic Awards BanquetEach spring, the Wednesday after spring break, the Department of Athletics holds a luncheon to honor those student-athletes who have distinguished themselves in the classroom during the previous calendar year. Please plan to attend if you have received a 3.2 cumulative GPA or above during the two previous terms. Invitations will be sent out to those invited student-athletes.

• We also recognize those who have achieved a 4.0 cumulative GPA in the previous two semesters. Their pictures will appear on the Wildcat Wall of Academic Achievement.• The team GPA awards are distributed to two teams in each of the previous two semesters that received the highest cumulative GPAs.• The Big Sky Scholar Athlete Award is given to the male and female student-athlete whose athletic eligibility has been exhausted and who has the highest cumulative GPA.• The Wildcat Club awards out the Wildcat Club Academic Achieve ment Award. This award is based solely on academic achievement and is given to a male and a female senior student-athlete with the highest cumulative GPA.

The Department of Athletics recognizes the faculty and university administrators as key components in the continued academic excellence of the student-athletes. The academic community sees the level of importance the WSU Department of Athletics places upon the student-athletes’ academic success.

Therefore, student-athletes are welcome to submit names of faculty members they wish to be invited to the Banquet. Names can be submitted to the Academic Coordinator at any time during the spring term, prior to the Awards Banquet. Please submit names early enough that faculty members can be invited.

The names of those student-athletes who receive the 4.0 Awards, the Big Sky Scholar Athlete Awards, and the Wildcat Club Academic Achievement Awards will be placed on the Academic Award Plaques posted in the academic area of the stadium. The names of the teams that received the Team GPA Awards will

also be placed on a plaque posted in the Academic Area.

Academic ResponsibilityJust as no one can perform your responsibilities on the court/field except you, no one can make and keep you eligible for competition EXCEPT YOU!! Weber State University and the Department of Athletics provide all student-athletes with academic support to help them pursue and complete a degree. Student-athletes are expected to contribute as much to their academic success as they do to their athletic success. Success or failure in anything in life is a personal choice.

Priority RegistrationStudent-athletes receive PRIORITY REGISTRATION and are able to register the first day of fall and spring registration periods. To obtain classes at the best times and with choice instructors, student-athletes need to register the day registration begins. Student-athletes can meet with their Athletics Academic Advisor as early as 2-3 weeks prior to registration to put a schedule of classes together. Upperclassmen are required to meet with department advisor(s) BEFORE meeting with Athletics Academic Advisor(s). Students should make appointments EARLY with department advisors so as to get their schedules prior to Priority Registration.

REMINDER: Meeting with an advisor to put a schedule together DOES NOT REGISTER YOU for courses!

Registration dates are issued to new and continuing students the week before registration begins. Student-athletes may access their eWeber Portal to obtain their time to register for classes. Student-athletes should aim to be enrolled in a minimum of 15 credit hours, at least 12 of which MUST BE countable towards meeting NCAA eligibility rules. The Director of Athletics requires that student-athletes meet with an Athletic Academic Advisor during each registration period to verify that classes taken will maintain athletic eligibility. Student-athletes are encouraged to prepare their schedules beforehand so they are prepared to meet with their advisor.

Registration can be accomplished in the following ways:• Internet – Go to www.weber.edu . Enter your Wildcat mail username and password. Click on Student Services tab. Then, click on Register for classes Add/Drop. If the Course Registration Numbers (CRNs) have already been obtained, you need only to enter them; otherwise, you will need to look up the information in the Class Schedule.• In person – You may register at either the Ogden or Davis Campus Registration Windows. Ogden campus hours are Monday – Thursday 7:30am – 5:30pm, Friday 7:30am – 4:30pm. CRNs are required for registration at the Registration Windows.

Student-Athlete Attendance PolicyThe Department of Athletics is committed to the goal of regular, effective class attendance. To this end, a student-athlete is expected to attend all class sessions for each and every class they may have. Regular class attendance sends a message to the faculty that the student-athlete is taking the course seriously. It is important to not only attend all class sessions but to arrive on

time and remain for the entire length of the class. Student-athletes should apply the same dedication to team practices and meetings to their academic success.It is impermissible for a student-athlete to miss class, leave early, or arrive late to a class in order to prepare or attend practice. There is a NCAA mandated practice time penalty for violations of this rule

Missing Class for CompetitionIt is mandatory that instructors be notified ahead of time when a class will be missed due to an athletics event. At the beginning of each semester, student-athletes will receive a letter from Athletics Academic Services that will contain Weber State University’s policy regarding students missing class to represent the university. On the back, it will contain a travel schedule for the team. It is each student’s responsibility to make arrangements prior to being gone to make up any missed work or tests. This will be at the discretion of the instructor. It is the student’s responsibility ot make up work in a timely manner, according to the instructor’s request. You must take this letter to each instructor during the first week of the semester. These letters can be obtained from Academic Services Staff if they were not made available.

Missing Class due to Illness, or other Reasonable GroundsIf you are unable to attend class due to illness, other medical situations, or reasonable circumstances it is YOUR RESPONSIBILITY to notify the appropriate people as noted below. A personal phone call and/or email is best. 1. Contact Instructor – Give them your full name, class name, and time. Let them know the reason for your absence. 2. Contact Athletic Academic Advisor - If leaving a message, state your full name and sport and reason for absence.3. Contact Coach – Coaches are banned from contacting faculty and campus personnel. DO NOT expect them to relay a message for you.4. If ill, contact your athletic trainers to seek medical attention or treatment.

Laptop CheckoutsThe Department offers student-athletes laptop computers to check out on a first-come, first-served basis. Laptops may be checked out for team road trips, to use in study hall, or for special circumstances. The laptops can be checked out in the Compliance Office, which is in room 204 of Stewart Stadium. Student-athletes will be responsible for any damage and loss that occurs to alaptop and may be subjected to fines if laptops are not turned in by the required date.

NCAA ELIGIBILITY REQUIREMENTSThe NCAA, Big Sky Conference, and Weber State University have multiple specific criteria each student-athlete must meet to be eligible for practice and competition during each year. It is critical that you understand these requirements and work closely with your Athletics Academic Advisor to ensure you are meeting them. ALL student-athletes must be enrolled in 12 credit hours at all times to practice, receive athletics aid and compete.

Student must complete the Student Success Center (SSC) Online Module by going to the link below.

Student is required to take an AccuplacerExam at Lampros Hall. Cost is $10 per test.

Faculty Athletics RepresentativeCraig ObergMicrobiology Department Chair/Professor

A native of Oregon, Dr. Craig J. Oberg graduated from Weber State in 1979 and received his Ph.D. at Utah State University in 1985. He then joined the Department of Microbiology at Weber State and for the past 21 years has served as depart-ment chair. Designated Presidential Distinguished Professor of Microbiology in 2006, Dr. Oberg has also received the John S. Hinckley Fellow (1998), the Alumni Association’s H. Aldous Dixon Award (1999) and the Spencer L. Seager Distinguished Teaching Award (2001). He was also the Cortez Honors Professor (2000) and the College of Science Endowed Scholar (2004-07). An active researcher, he has published

more than eighty articles, delivered over a hundred scientific presentations, received several patents, and edited a book reconciling science and religion.

His eclectic interests have led him to such diverse projects as a book and honors course on fly-fishing, a photographic essay on the microbiology of Yellowstone National Park’s thermal features, and a book on Mozzarella cheese. In addition to all of this, Professor Oberg has devoted considerable energy to service both on campus and in the professional community. He has served as president of the Utah Academy of Sciences, Arts, and Letters and of the Intermountain Branch of the American Society for Microbiology.

He and his artist wife Lynette have five children, five grand children, and live in Liberty, Utah. Four of their children have graduated from Weber State. Besides fly-fishing, his hobbies include running marathons, photography, training bird dogs, and more fishing.

NCAA STUDENT-ATHLETE AFFAIRSThe NCAA Student-Athlete Affairs unit (formerly known as CHAMPS/Life Skills) provides life skills support in the areas of academics, athletics, personal development, career development, and service through the distribution of accessible resources, strategic partnerships and customized programming to member institutions. Student-Athlete Affairs programs, resources and events are designed to promote the well-being and development of student-athletes. Weber State’s Department of Athletics is committed to the growth and development of student-athletes in the areas of academics, athletics, personal development, service and career development.

ACADEMIC EXCELLENCE ensures that student-athletes will have the opportunity to excel in their chosen field of study and that athletics programs will provide services to support and enhance the academic success of student-athletes.

ATHLETICS EXCELLENCE ensures that student-athletes will be provided facilities, coaching staff, and support services that will enable them to excel in a broad program of sports sponsored by the University. The fundamental principles of these programs will be based on a commitment to sportsmanship, equity, fair play, and integrity.

PERSONAL DEVELOPMENT ensures that each student-athlete participating in a CHAMPS/Life Skills program will be provided opportunities to focus on personal growth issues such as values clarification, goal setting, fiscal responsibility, decision making, and stress management. Each of these components will be focused toward developing a healthy lifestyle.

SERVICE is our challenge to student-athletes to give back to communities and individuals who are in need. With a clearly defined program of service, our student-athletes will be given opportunities to develop the foundation for a lifelong commitment of volunteerism.

CAREER DEVELOPMENT places a priority on preparing for life after college sports. The program acquaints students with the job search process, provides networking opportunities, and ultimately assists with job placement. As our programs work toward the development of the total person, the ultimate goal is to develop individuals who will have rewarding careers and productive lifestyles.

Student-Athlete Advisory Committee (SAAC)The Student-Athlete Advisory Committee (SAAC) is made up of student representatives from each sponsored sport and the spirit squad. The group meets bi-monthly to discuss events, procedures, and current issues, in addition to coordinating fundraisers, community service and social events. The Committee serves as a vehicle to promote communication between the athletic administration and the student-athletes. The Committee addresses issues relevant to student-athletes in general, such as gender equity, financial aid, changes in NCAA regulations, etc. Representatives serve as liaisons between the Department of Athletics and their teams. The Committee is also instrumental in organizing community service projects and serves as a vehicle to enhance leadership qualities and characteristics.

This includes agreements for representation in the future. Moreover, if a student-athlete or their relatives or friends accepts any type of benefit from an agent or anyone who represents an agent, the student-athlete shall lose his or her eligibility for competition. AmateurismA student-athlete is not eligible to participate in intercollegiate athletics in a sport if he or she has ever:

• Taken pay, or the promise of pay, for competing in that sport;• Agreed, through a signed contract, to compete professionally in that sport;• Played on any professional athletic team in that sport; OR• Used their athletics skill for pay in any form in that sport.

Student-athletes become ineligible if they promote, in any way, a commercial product or service or allow their name to be used in promoting a commercial product or service.

Athletically-Related Financial AidA student-athlete may receive an athletics scholarship for up to 5 years within a six year period. To be eligible for an athletics scholarship, a student-athlete must be registered in a full-time program of study. An athletics scholarship may cover all or part of the following elements: tuition, student fees, class fees, room, board, and required course textbooks. Athletics scholarships are renewed on an annual basis. During the term of the award, an athletics scholarship may be canceled or reduced if (1) the student-athlete renders themselves inelgible for competition, (2) fraudulently misrepresents any information on an application, letter of intent, or financial aid agreement, (3) engages in serious misconduct, (4) voluntarily withdraws from the team, or (5) violates any other term on the aid agreement.

Additionally, the Office of Financial Aid, which administers athletics financial aid, has other restrictions that may limit the ability of a student-athlet to receive their athletics scholarship. Specifically, the Office requires student-athletes to maintain a cumulative 2.00 WSU GPA. If a student-athlete is on scholarship and fails to maintain a 2.00 WSU GPA, the student-athlete may be able to submit an appeal to the Office of Financial Aid to have their athletics scholarship reinstated.

Book Purchases - For all student-athletes who receive a book scholarship, the Compliance Office will notify the bookstore which student-athletes are on a book scholarship and what books are needed for their classes. Book orders should be available for pickup in room 115 of the bookstore on the first day of classes. Photo ID is required to pick up your books. Please verify that your order is complete and all books are in the box before leaving the bookstore.

Book Returns - During finals week, all student-athletes must return all textbooks purchased with book grant-in-aid money to the Department of Athletics. The Compliance Office will notify coaches of the times for book returns. Student-athletes will be billed at the return value (usually 50% of the new book price) for any books not returned, even if the books have been lost or stolen. If the textbook is required for an additional term, students can complete the Additional Term Book Request Form. Student-athletes are still responsible to return that book at the end of the additional term.

Employment Earnings – All SportsA student-athlete’s earnings from on- or off-campus employment is exempt from, and is not counted in, determining the full grant-in-aid or in the individual’s or institution’s financial aid limits. If a student-athlete works while he or she is a member of a team, the student-athlete and the employer must sign a statement prior to beginning the employment (to be kept on file in the Compliance Office). The written statement must verify:

• The student-athlete’s compensation does not include any remuneration for value or utility that the student-athlete may have for the employer because of the publicity, reputation, fame, or personal following that he or she has obtained because of athletic ability;• The student-athlete is compensated only for work actually performed; AND• The student-athlete is compensated at a rate commensurate with the going rate in the locale for similar services

Ethical ConductAll student-athletes must conduct themselves with honesty and sportsmanship at all times so that intercollegiate athletics at Weber State shall represent honor and dignity of fair play. A student-athlete will lose their eligibility if they knowingly take part in NCAA violations, receive extra benefits as a student-athlete, or refuse to cooperate in any investigation of possible violations.

Extra BenefitsAn extra benefit is any special arrangement by an institutional employee or a representative of the institution’s athletic interest (booster) to provide a student-athlete or their friends or relatives a benefit not available to the general student population and not authorized by the NCAA.

Examples of “extra benefits” include, but are not limited to, the following:• Use of Department of Athletics’ photocopier without being charged;

• Use of the Department of Athletics’ telephones for long distance phone calls;• Typing of course papers by someone other than the student-athlete without being paid the going rate for such a service;• Receipt of cash or loans by a student-athlete;• Payments of parking tickets by the Department of Athletics;• Use of a Department of Athletics’ staff member’s automobile; • Discounts or credits to local businesses (i.e. reduced cost of rent, movie tickets, phone cards, sports apparel including athletic shoes other than the ones provided by the university, etc.);• Receipt of more than the allotted number of complimentary admissions allowed for an athletics contest; OR• The selling of complimentary admissions for an athletics contest. Receipt of any of these items will cause a student-athlete to become immediately ineligible pending reinstatement by the NCAA.

GamblingThe NCAA strictly prohibits any form of sports wagering. Student-athletes may not place bets on any sport, collegiate or professional, in which the NCAA conducts a championship. This policy is strictly enforced because the NCAA and its member schools believe that gambling activities undermine the spirit of amateur athletics. Student-athletes are prohibited from engaging in the following activities:

• Providing information concerning intercollegiate athletics competitions to individuals involved in organized gambling activities;• Soliciting a bet on any intercollegiate team;• Accepting a bet on any team representing the institution; AND• Participating through a bookmarker, a parlay card, the Internet, a “pool”, or any other method employed by organized gambling in any gambling activity that involves intercollegiate athletics or professional athletics.

The following may render a student-athlete ineligible:• Providing any information (i.e. team injuries, team morale) to individuals known to be involved in organized gambling activities;• Posting any information via social networking platforms that could be advantageous to an individual known to be involved in organized gambling activities;• Accepting a bribe or agreeing to throw, fix, or illegally influence the outcome of any intercollegiate contest; OR• Failing to report any bribe of offer or attempt to throw or fix the outcome of any contest.

Playing and Practice SeasonsThe NCAA mandates that student-athletes cannot participate in countable athletically related activities for more than four hours per day and 20 hours per week during their playing season. These activities include practice, weight training and conditioning, watching film, meetings, and competitions. Coaches must also give student-athletes one day off during the week while in their playing season.

The NCAA also mandates that student-athletes sign the NCAA Student-Athlete Statement, Buckley Amendment Consent Form, and the Drug Testing Consent Form before they officially begin practice (whether it is during the playing season or during the off-season). Additionally, student-athletes are required to complete the Big Sky Conference Historical Questionnaire.

Outside the regular playing season, student-athletes may not participate in the following activities for more than eight hours per week:

• Required weight training and conditioning;• Participation in a physical fitness class conducted by a member of the athletics staff, unless such a class is a regular physical education class listed in the catalog and open to all students.;• In sports other than football, two hours of skill-related instruction with a member of the institution’s coaching staff at the request of the student-athlete. The two hours of skill instruction must be included in the eight hours.

No activities may occur during the week before finals and the week of finals if a team is out of season.

RecruitingEnrolled student-athletes may have a limited role in the recruitment of a prospective student-athlete (PSA) for Weber State University. An enrolled student-athlete may RECEIVE telephone calls from a PSA made at the expense of the PSA. An enrolled student-athlete may serve as a student host to the prospect, their parents, legal guardians, or spouse on official visits. Before a student-athlete may host a PSA on an official visit, they must attend a training and certification meeting offered by the Compliance Office. The University may provide a student host with $30.00 per day to entertain the prospect. This entertainment does NOT include the meals and admissions to home athletic contests. Student host entertainment money MAY NOT be used to purchase souvenirs such as WSU t-shirts or other university mementos. If

a student host entertains more than one PSA, they may receive an additional $15.00 per day for each additional PSA.

The student host may receive complimentary meals during the official visit, on or off campus, as long as the host accompanies the prospect during their official visit. Also, complimentary admissions may be given to the PSA and the host to home, on-campus athletic contests, provided the host accompanies the prospect. The PSA MAY NOT be given complimentary admissions to conference or NCAA Championship events. The PSA MAY NOT be recognized (e.g. on the scoreboard, or by a public address announcer) during an athletic contest.

The student host MAY NEVER use the automobile of a Department of Athletics staff member or coaching staff member for transporting the PSA during their visit. Entertainment of a prospect must occur within 30 miles of the WSU campus. The PSA and the PSA’s parents or spouse may receive complimentary admissions to home athletic contests. These admissions must provide seating in the general seating area (i.e. not on the bench, special seating behind the bench or in the press box).

Student-Athlete Opportunity FundThe NCAA created the Student-Athlete Opportunity Fund (SAOF) to support direct benefits to student-athletes. As a guiding principle, the fund shall be used to assist student-athletes in meeting financial needs that arise in conjunction with participation in intercollegiate athletics, enrollment in an academic curriculum, or to recognize academic achievement. A student-athlete may receive money from the SAOF only up to the full cost of attendance (Athletics Scholarships included). The following are categories Weber State has identified to utilize the SAOF funds.

• Student-Athlete Advisory Committee• Academic Services Enhancement• Emergency/Hardship*• Summer School*• 5th year aid*• Other Educational Expenses and Fees

* see below for guidelines to the awarding of SAOF funds for summer school, 5th/6th Year Aid, and Emergency/Hardships:

Summer School** The following benchmarks must be met for a student-athlete to be eligible to receive summer school funding from the SAOF:

• Complete application turned into Associate Athletics Director/SWA by deadline***;• 3.0 cumulative GPA; AND• Deemed eligible based on NCAA, Conference, and institutional guidelines.

Summer school funding derived from the SAOF can be applied only to tuition, fees, and/or books and will not to be used for stipends.

Student-Athletes 5th/6th Year Aid**Student-athletes will be able to receive SAOF funding and it is limited to tuition and fees and can not exceed the previous year’s equivalency. The following requirements must be met for a student-athlete to be eligible to receive 5th/6th year aid funding from SAOF:

• Complete application turned into Associate Athletics Director/SWA by deadline***;• Currently receiving athletics aid (scholarship), student-athlete cannot have already received 10 semesters of athletics aid;• In academic good standing;• Have 18 or fewer credits remaining to graduate and must enroll in 12 credits applicable toward degree, unless less are required for gradua- tion;• Agree to work 20 hours/week within the Department of Athletics during the semester in which they are being awarded aid.

Emergency/Hardship**Each request for an emergency or hardship formulated with supporting documentation from the student-athlete’s respective head coach will be evaluated on an individual basis to assess the level of need and the nature of the emergency. A committee comprised of the Associate Athletic Director/SWA, Athletics Financial Officer, and Compliance Coordinator will review all requests for approval or denial.

** These categories are subject to an application and/or referral process from the Head Coach.*** Deadlines will be strictly enforced.

Financial AidStudent-athletes should reapply each year for federal financial aid, even if they did not qualify for aid in the past, due to annual changes in requirements. The priority filing deadline for federal financial aid is March 1. You can still apply for financial aid after this date, but your money may not be available in time for the beginning of classes. The WSU Financial Aid and Scholarship Office suggests that students apply as early as possible as award offer processing time may take 8 to 10 weeks. If a student is requesting loan assistance, additional processing time will be required.

Dream Weber ProgramTo apply please complete the following steps:

1. Apply for admission to WSU;2. Complete the Free Application for Federal Student Aid (FAFSA) by March 1 — this application takes about 60 minutes; • WSU offers free FAFSA application help during the month of February. 3. Complete the WSU Financial Aid Application by March 10. Go to the eWeber portal to complete this step — it takes 5-10 minutes.

• Approximately 30 percent of all students are randomly selected for “verification” (which means you must submit income tax returns, W-2 forms, etc. used in the FAFSA application). If you are selected, all verification documents must be submitted to the WSU Financial Aid Office by March 15.

4. Complete the WSU Scholarship Application by March 10. Go to the eWeber portal to complete this step - it takes 15-20 minutes.

You’ll automatically be considered for the Dream Weber Program when the WSU financial aid office receives your results directly from FAFSA and processes your other applications.

Athletic-Related Injuries Policy & Procedures

POLICYThe WSU Insurance Policy is designed to educate every student-athlete, parent, trainer, coach, team physician, and healthcare provider of the procedure for reporting, processing, and payment of medical claims for athlet-ic-related injuries.

A) An athletic-related injury is defined as any accident or injury sustained during a WSU supervised practice or scheduled event. A practice or scheduled event must be supervised by a coach, trainer, or strength coach.

• Any injury not directly related to participation in a WSU supervised practice or scheduled event is not covered by the WSU insurance plan.• Any medical conditions such as dehydration, fatigue, staph infection, or an illness is not covered by the WSU insurance plan.

B) The WSU Student-Athlete Insurance Plan is secondary to a student-athlete’s primary insurance plan. All student-athletes are required to have primary insurance that covers athletic-related injuries and is billable within the United States.

• All injury claims must be processed through a student-athlete’s primary insurance plan before they can be submitted to the WSU secondary insurance plan.• The student-athlete’s primary insurance Explanation of Benefits (EOB) must be received by WSU before the claim can be submitted and processed through the secondary plan.It is the responsibility of each student-athlete to make sure medical billings are processed through his/her primary insurance plan. • WSU is not authorized to obtain information from a student-athlete’s

primary insurance plan regarding specifics of any billing.

PROCEDUREA) ALL athletic-related accidents or injuries must be reported to a certified athletic trainer within 48 hours.

1) The trainer will meet with the student-athlete to evaluate the extent of the injury.2) During the evaluation, all information will be entered into Sportsware (medical injury software) and an Injury Report will be generated.

B) If the student-athlete needs medical attention by a physician or treatment at a medical facility, the trainer will give the student-athlete a copy of the Injury Report/Referral Form.

1) The student-athlete will take the Injury Report/Referral Form to the Department of Athletics’ Insurance Coordinator (Kerri Thompson, Stadium Training Room). 2) The Athletic Department Insurance Coordinator will verify primary insurance coverage and have the student-athlete fill out the required Secondary Insurance Claim Form.3) The student-athlete will take the Injury Report/Referral Form and attached primary insurance information to the physician or medical facility they have been referred to.

C) The student-athlete will meet with the physician for treatment of the athletic-related accident or injury. The physician may recommend that the student-athlete come back for a follow up visit, visit a specialist, or continue treatment at another facility. NOTE: The student-athlete must have all visits approved through the WSU Department of Athletics’ Insurance Coordinator. D) The physician may recommend treatment with drugs.

1) All drugs must be prescribed by a physician. The physician may give the student-athlete a hand written prescription or call in the prescription to Smith’s Food and Drug, 40th and Harrison in Ogden. The student-athlete will then contact their athletic trainer regarding instructions on picking up the prescription.

E) The physician may recommend treatment with durable medical equipment (braces, crutches, etc.).

1) The attending physician will fit the student-athlete for all durable medical equipment (DME) and issue a prescription with the exact specifications and type of brace, etc. 2) The student-athlete will return the prescription to the Department of Athletics Insurance Coordinator. 3) The Insurance Coordinator will place the order for DME with instructions

to the vendor to bill the student-athletes primary insurance. F) The physician may recommend surgery.

1) The physician will contact the Head Athetic Trainer with a recommendation for surgery. If it has been determined that the student- athlete will need surgery, the Head Athletic Trainer will immediately contact the Department of Athletics’ Insurance Coordinator.2) The Insurance Coordinator will verify primary insurance coverage and inform the trainer/physician or student-athlete of any stipulations (i.e., surgery pre-approval, or specific facilities that must be used due to the type of insurance the student has). If the student-athlete has an out-of- state insurance policy that will not cover surgery at a Utah facility, the student-athlete will be sent home by the Department of Athletics for surgery. 3) Surgery must not be performed until steps 1 and 2 above have been performed.

G) All providers need to directly bill the student-athlete’s primary insurance carrier first and the WSU Athletic Department second. Any bills sent directly to a student-athlete should be brought to the attention of the WSU Insurance Coordinator.

The Weber State University Athletics Media Relations office is a student-athlete’s link to the media of Utah and the nation. The media relations office is primarily responsible for any media-related items, including interviews, articles, and any noteworthy events which involve you as a student-athlete. The office also produces all publications, media guides, and game programs. We are also responsible for statistics and scoring for all games and events. All conference and national honors for student-athletes are promoted and nominated by our office.

In addition, we operate WeberStateSports.com, the official website for Weber State Athletics. We are also responsible for social media for the department and individual teams on Facebook, Twitter, and YouTube.

Department of Athletics Grievance Procedure

The Weber State University Department of Athletics is dedicated to the health, safety, and welfare of its student-athletes. Therefore, the following grievance procedures are designed to help student-athletes resolve conflicts, including such things as mental abuse, physical abuse, suspension, dismissal, deviations from team policy, or where an existing team policy is considered unfair. There may come a time when a conflict arises. If this should happen, the following procedures are to be followed in an effort to resolve the complaint, grievance, or problem.

1. The student-athlete is encouraged to first meet with the specific Department of Athletics employee (i.e., coach or staff) involved to see if the situation can be resolved. During that meeting, work to identify and accomplish the following:

• Completely define the situation;• Present a solution; AND• If necessary, negotiate how the situation will be solved.

2. If step one is not feasible or does not result in a satisfactory resolution, the student-athlete is encouraged to discuss the situation with a senior-level Department of Athletics administrator (i.e., Director of Athletics or designee, Senior Woman Administrator) One or both of these individuals will attempt to mediate your problem. If your complaint is against the AD or SWA, you have the right to speak with the other individual who is at this level. If you still feel there has been no resolution to your issue, there are two more committees that have been developed to resolve any issues that are outlined below.

In cases where NCAA Bylaws specify, a hearing must be conducted by an entity outside the Department of Athletics (e.g. the denial of a transfer request). In this case members of the University Athletic Board will comprise the hearing entity.

Financial Aid Appeals CommitteeIf your WSU Athletic Grant-in-Aid has been reduced or totally canceled in comparison to the previous year and you want to appeal this decision, your appeal will be heard by the Financial Aid Appeals Committee.

• Alert the Director of Athletics in writing that you wish to appeal your athletic financial aid problem. The AD will contact the Director of Financial Aid.• The Director of Financial Aid will attempt to set up a hearing with you and their committee. You are advised to present your case in writing with as much documentation as possible.• You will be provided a hearing before the committee. Their decision is final

and will be totally implemented by the Department of Athletics.

Department of Athletics Grievance CommitteeThe committee will handle all conflicts, problems, and accusations brought by student-athletes other than the renewal of NCAA administered financial aid. It will meet on an as-needed basis. The composition of this committee will include at least 3 of the following:

• Chair of the Athletic Board• Faculty Athletics Representative• Representative from the coaching staff• Senior Woman Administrator• Academic Coordinator• Compliance Coordinator(s)• Member of the SAAC Board• Faculty Member• Office of Civil Rights Officer• University Legal Council

Any athlete may have a hearing before these two committees if they have followed the proper procedures outlined in steps one and two. If your appeals within the Department of Athletics have been exhausted, inform the administrator you have been working with that you wish to appeal to one of the committees, and you will have a scheduled hearing. The decisions of the committees are final and will be implemented by the Department of Athletics.

PLEASE NOTE: Any and all charges, complaints, grievances, and/or accusations must be in writing with as much detail (e.g. time, date, and incident) as possible. For example, “I was treated unfairly” without documentation details is not acceptable.

Some Valuable Suggestions:• Whenever possible, use face-to-face discussions to solve problems.• Get your facts straight prior to any discussion(s) and/or accusation(s).• False accusations tend to backfire.• If needed, seek confidential advice from someone who is able to rationally analyze the situation (i.e. team captain, coach, advisor, etc.).• Attempt to recognize potential conflicts immediately and bring it/them to the attention of your coach.• If there is a problem, address it immediately by defining it and bringing it to the attention of someone who is in a position to help (i.e. coach, AD, SWA, Compliance Coordinator, Academic Coordinator, etc.).

All Weber State University parking is by permit only. You can buy a $20 pass and always find a place to park in one of the 3,000 available spaces at the Dee Events Center. The shuttle runs from the Dee Events Center to the bus stop in the loop south of the Stewart Library and back to the Dee Events Center.

The shuttle runs at 7-minute intervals from 6:30 a.m. until 2 p.m. and about every 14 minutes from 2 p.m. until 8:30 p.m.
