Injury and Illness Prevention

South Bay Medical Center’s Culture of Safety

All injuries and all safety incidents are preventable: the goal is to be injury free.

All employees are accountable for working safely.

Prevention is more effective and sustainable than post-injury management.

Safety feedback and observation are everyone’s responsibility.

Employee involvement is critical.

Managers are responsible for ensuring that the systems, equipment, training and support allow employees to work safely.

When alerted to an injury/illness, all managers must call the WPS pager @ 310-797-2409.

Facility Disaster Plans can be found in South Bay Policy Manager, 6900 Series

South Bay’s Emergency Codes:

Code Red – Fire

Code Orange – Disaster

Code Secure – Infant Abduction

Code Gray – Combative/Assaultive Behavior

Code Silver – Active Shooter

The Hospital Command Center (HCC) is located in ACR-1. The HCC phone number is 8-340-4186.

The highest risks for SBMC based on the 2013 Hazard Vulnerability Analysis (HVA) are:

Earthquake, Severe Thunderstorms with Flooding and External Chemical Release

If you are at home when a disaster occurs:

Ensure the safety of you and your family

Come in on your next scheduled shift unless instructed otherwise

Call 3300 to report any/all emergencies.

Fire Safety, Response and Extinguishers

South Bay’s 4-Step Fire Plan is RCAE.

RESCUE those in immediate danger

CONFINE the fire by closing doors

ALERT people to the fire by activating the nearest manual pull station and calling 3300

EXTINGUISH the fire if its safe to do so and you have been trained

Otherwise, consider evacuation (same floor is preferred)

Fire Evacuation is in the order:

Those nearest the threat

Horizontal to next safe smoke compartment, then

Vertical, then

Out of the building

Hazardous Materials and Waste

The Facility Hazard Communication Plan, now called Globally Harmonized System (GHS) can be found in South Bay Policy Manager, Policy # 6060

Each department at SBMC is required to maintain an updated copy of its MSDSs, now called SDSs (Safety Data Sheets) and chemical inventory. You can access MSDSs by going to South Bay Policy Manager and click on MSDS or use the link MSDS On Line.

EVS related chemical MSDSs are located in each EVS closet in the hospital.

Pursuant to GHS requirements, all employees at SBMC must be trained on the new label elements and SDS format by December 1, 2013.

SBMC chemical spill procedures:

Assess, limit traffic in the area of the spill and call 3300.

Remember, before handling any spill, locate and wear the appropriate personal protective equipment (PPE) and locate the departmental spill kit.

Infection Control

Bloodborne Pathogen (BBP) Exposure:

On Campus: Report to Supervisor: Monday through Friday 8:30 AM – 5:00 PM, go to Occupational Medicine (ext. 3739) in Normandie North B Building for initial care, follow-up with Employee Health (ext. 2384). After hours, weekends and holidays, go to the Emergency Department for initial visit.

MOBs: Report to Supervisor and designated physician for initial visit and call Employee Health (ext.2384) for follow-up.

Accessing the BBP Exposure Control Plan:

Access the South Bay Policy Manager, click Browse manuals, click Medical Center Wide manual, click Infection Control manual and click on Policy # 7601.

Accessing the ATD Exposure Control Plan:

Access the South Bay Policy Manager, click Browse manuals, click Medical Center Wide manual, click Infection Control manual and click on Policy # 7605.
