Hubungan Cardiovascular and Respiratory System

DO2 = [1.39 x Hb x SaO2 + (0.002 x PaO2)] x CO

BP = CO x Systemic Vascular Resistance

Cardiac Output = Heart rate x Stroke Volume

Stroke volume is determined by Preload, Afterload and Contractility

Preload = volume akhir diastolik volume pada pembuluh darah vena

Afterload = tegangan dinding ventricle pd akhir sistolik tegangan/tahanan pembuluh darah

- The systolic wall tension (which follows the:

 law of Laplace = TMP x r / 2 h [transmural pressure x radius / 2 x wall thickness]).

Starling konsep hubungan preload dengan stroke volume

Stroke volume = EDV-ESV

Ejection fraction = EDV – ESV x 100% / EDV

Ventricular stroke volume (SV) = ventricular end diastolic volume (EDV) - the end

systolic volume (ESV)

EDV = 120 ml of blood ESV = 50 ml of blood SV = 70 ml EF = 70:120 x 100%

Ditingkatkan dengan inotropic agents:

Adrenaline / Nor adrenaline Dobutamine / Dopamine Calcium

Menurun pada kondisi:

Acidosis Hypoxia Hypocalcaemia

Arterial Pulse Pressure

Perubahan tekanan aorta maksimal selama systole (pada saat katub aorta membuka sampai dicapai tekanan aorta tertinggi)

Pulse Pressure = Systolic Pressure ─ Diastolic Pressure

Ditentukan oleh compliance dari aorta juga oleh stroke volume.

Makin kaku aortanya, pulse pressure makin melebar. 

Makin “compliant “ aortanya, makin menyempit pulse pressurenya

Ditentukan oleh cardiac output (CO), systemic vascular resistance (SVR), dan central venous pressure (CVP)

Didasarkan pada hubungan flow, pressure and resistance

Tanda – tanda gagal sirkulasi

Pompa: pulsus alternan, suara gallop, bi-basal rales, peningkatan tekanan vena jugularis, penurunan MAP

Isi Pipa (defisit): tanda-tanda dehidrasi, CRT > 2 detik, produksi urine menurun, PP << Isi pipa (overload): distensi vena leher, rales

Pipa: edema, ascites, akral dingin
