The Amityville Union Free School District includes the Village of

Amityville and the unincorporated areas of the Town of Babylon and the

Town of Oyster Bay. The total population of the district is approximately

20,000. Twenty-eight hundred pupils attend the public schools of the

district, including three elementary schools: Northeast Elementary School,

Pre-K, K; Northwest Elementary School, grades 1, 2; Park Avenue Memorial

Elementary School, grades 3, 4, 5, 6; Edmund W. Miles Middle School,

grades 7, 8, 9; and Amityville Memorial High School, grades 10, 11, 12.

THE BOARD OF EDUCATIONThe Board of Education consists of seven members, each elected for

a three-year term. These nonpaid representatives of the community set policy

and maintain legal authority for the operation of the school district.

During the 2009-2010 school year, the Board of Education will meet

in the Park Avenue Memorial Elementary School Auditorium or the Park

North Administration Building as noted on the calendar. Unanticipated

changes of meeting dates or locations will be publicized in the AmityvilleRecord and Massapequa Post (time permitting) and notices will be posted in

the display cases at the west and east doors of the Park North Administration

Building and on the district website (www.amityville.k12.ny.us).

The public is invited to attend these meetings and is afforded the

opportunity to address the Board, in appropriate public sessions.

RELEASE OF STUDENT INFORMATION UNDER THE “NO CHILD LEFT BEHIND ACT”

Pursuant to the “No Child Left Behind Act,” all public schools are now legally required to release the names, addresses and telephone numbers ofjuniors and seniors to military recruiters or institutions of higher learning upon request. Parents have the right to request that such information not bereleased without prior written consent.

PHOTOGRAPHY/VIDEO POLICYIt is the Amityville UFSD policy, as in most Long Island public school districts, to film or photograph students and staff as they go about the

gratifying business of learning and succeeding in school. If you do not wish to have your child photographed and/or videotaped, you must submit yourrequest in writing to the Director for Pupil Personnel Services, Special Education and Central Registration, no later than September 1, 2009. Group orfull-class photographs and videotapes should not be included in your request.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT OF 1974 (FERPA)The Amityville Union Free School District encourages parents/guardians and eligible students to exercise their rights under the Family

Educational Rights and Privacy Act of 1974 (FERPA). FERPA defines “eligible students” as those who attain the age of 18 or who enroll in apostsecondary school. The list of rights guaranteed to parents/guardians and eligible students includes the following:

1. The right to be informed about FERPA2. The right to inspect and review the student’s educational record3. The right to seek to correct a student’s record4. The right to exercise a limited control over access to the student’s record5. The right to report alleged violations of FERPA to the U.S. Department of EducationParents/guardians/eligible students who wish to inspect educational records may obtain applications for inspection of records from the school

principal. Copies of the district’s policy pertaining to FERPA are available for parents/guardians/eligible students in the principal’s office of each schooland in the Superintendent’s Office.

FINDING ANSWERS

Under no circumstances will the school district or Board of Educationconsider or take action on any letters or communications that are unsigned.Anonymity of individuals registering complaints or allegations in writingwill be respected if requested, but such communications must bear thename, address and signature of the writer.

When parents/guardians have a question or complaint about a child’sclasswork or progress in a specific course, they should contact the teacher.The teacher, who works with the student, knows the child and can generallyhelp to resolve the difficulty.

At the elementary level, a parent/guardian should contact the buildingprincipal if the teacher does not offer a satisfactory resolution. At thesecondary level, the parent/guardian should schedule an appointment withthe student’s guidance counselor. If satisfaction is not achieved, then theparent/guardian should call the building principal.

If a resident’s concern is of a more general nature and involves theeducational program or activities of a school, the building principal shouldbe contacted directly.

If the question still remains unresolved, the parent may choose to contactthe office of the Assistant Superintendent for Curriculum and Instruction,and then the office of the Superintendent of Schools.

In the case of a complaint against a professional staff member,parents/guardians are urged to:1. Speak to the staff member involved. If the situation is not resolved:2. Speak to the immediate supervisor/administrator of that teacher or staff

member. If the situation is still not resolved: 3. Speak to the building principal. If the situation is still not resolved:4. Contact the office of the Assistant Superintendent for Curriculum and

Instruction. If the situation is still not resolved:5. Contact the office of the Superintendent of Schools. If the situation is

still outstanding without resolution:

6. Contact the President of the Board of Education.

PESTICIDE NOTIFICATIONInitial Notification to Persons in Parental Relation and Staff Pursuant

to Section 409-H of the State Education Law.New York Education Law Section 409-H, effective July 1, 2001,

requires all public and nonpublic elementary and secondary schools toprovide written notification to all persons in parental relation, faculty,and staff regarding the potential use of pesticides periodically through theschool year.

The Amityville School District is required to maintain a list of personsin parental relation, faculty, and staff who wish to receive 48-hour priorwritten notification of certain pesticide applications. The followingpesticide applications are not subject to prior notification requirements:a school remains unoccupied for a continuous 72 hours following anapplication; antimicrobial products, nonvolatile rodenticides in tamper-resistant bait stations in areas inaccessible to children; silica gels and othernonvolatile ready-to-use pastes, foams, or gels in areas inaccessible tochildren; boric acid and disodium octaborate tetrahydrate; theapplication of EPA-designated biopesticides; the application of EPA-designated exempt materials under 40CFR152.25; the use of aerosolproducts with a directed spray in containers of 18 fluid ounces or lesswhen used to protect individuals from an imminent threat from stingingand biting insects including venomous spiders, bees, wasps and hornets.

The Amityville School District includes the services of a contractedfirm that is an expert in pest management. This firm conducts monthlyinspections of all district facilities and takes appropriate remedialmeasures to eliminate or reduce the presence of insects or other pests.

While the Amityville School District does not anticipate any situationwhere there will be a need to provide notification of a pesticideapplication 48 hours in advance, if you would like to review such notification if one were required, please contact the AmityvilleUFSD Buildings and Grounds Office at 150 Park Avenue, Amityville,NY 11701.

USE OF SCHOOL BUILDINGS

The Board of Education willinglyconsiders specific requests for the use ofschool buildings and/or facilities by civic andcommunity organizations. Interested partiesshould complete and submit a “Request forUse of School Facilities” form availablethrough the Business Office (598-6524) atthe Park North Administration Building, 150Park Avenue, Amityville, NY 11701, at leastthirty (30) days prior to the next scheduledBoard meeting.

CHILD FIND

All children are entitled to a free andappropriate public education, with specialservices for those with disabilities who haveneed of extra support. Parents/guardians whosuspect or have verified that their child has adisabling condition that may require specialassistance are asked to contact the Office ofPupil Personnel Services, Special Education,& Central Registration at the followingaddress:

HEALTH SERVICESA registered nurse, who provides a

variety of health screening services andadministers first aid, staffs each school in the Amityville UFSD. School nursesmaintain health histories for all children in the building. It is important thatparents/guardians keep the health officeinformed of any changes in a child’s health.

MEDICATION

School nurses are able to administermedications, including over-the-countermedications, only with the receipt of adoctor’s prescription. If it becomes necessaryfor a child to receive medicationduring school hours,parents/guardians must also providethe medication in its originalcontainer with affixed instructions.No school professional can makeformal recommendations formedication. Please see your ownhealthcare professional for formalrecommendations.

PROOF OFIMMUNIZATIONS

New York State and Federallaw established regulations governingimmunizations to protect school

children from communicable diseases. Allstudents must have these immunizations tocontinue to attend school and school-sponsored events. Parents/guardians arerequired to provide the school nurse withwritten proof of a child’s immunizationagainst polio, rubella (german measles),mumps, diphtheria and hepatitis B (3 dosesfor students born on or after 1/1/94). *A lackof an original doctor-stamped immunizationrecord may result in the exclusion of a studentfrom school. Additionally, all enteringkindergarten students must have a secondmeasles immunization given after 15 monthsof age, to be considered fully registered intheir home school.

PHYSICAL/DENTAL EXAMINATIONS

New York State law requires medical and dentalexaminations for all new entrants and children inkindergarten, second, fourth, seventh and tenth grades. InSeptember, proof of a physical and dental examinationmust be submitted to the school nurse. Children for whomthere is no documented proof will be scheduled for aphysical examination by the school doctor after the firstweek in October. Furthermore, students participating onan athletic team in grades 7-12 must receive a sportsphysical before tryouts or practice for any team.

HOMELESSNESS

In keeping with the McKinney-Vento Act and otherapplicable New York State statutes on the homeless, aparent/guardian of a homeless child should contact CentralRegistration for assistance and due process rights, at 225-4615.

PLEASE NOTE:*Lack of immunization documentation for

homeless students should be discussed with Central

Registration. In some cases, homeless students are

protected from exclusions stated in the immunization

guidelines.

PUPIL PERSONNEL SERVICES,

SPECIAL EDUCATION AND

CENTRAL REGISTRATION

For families of children believed to have a disability: Inkeeping with the mandates of the Individuals WithDisabilities Education Act (IDEA) and New York StateCommissioner’s Regulations, please note the following:

The Department of Pupil Personnel Services, SpecialEducation and Central Registration provides assistance to thestudents and families of the Amityville UFSD in the areas ofspecial education, ages 2-21, psychological and social workservices, nursing and health services, and CentralRegistration. If you have questions regarding these services,or your child’s eligibility for these services, please call225-4610, or Central Registration at 225-4615.

A District Committee on Special Education and aCommittee on Preschool Special Education are appointed bythe Board of Education for the purpose of reviewing theindividual educational requirements for all students in thedistrict who have been diagnosed as having disabilities thatwould interfere with their educational progress. Thesecommittees have the responsibility of makingrecommendations to the Board of Education concerning anappropriate program or services, if needed, for each childwith a disability. Parents/Guardians who have questionsconcerning children with disabilities should contact theDirector of Pupil Personnel Services, Special Education, andCentral Registration at 225-4610.

WORKING PAPERS

The Amityville School District cooperates with the New YorkState Labor Department in the issuance of working papers required bylaw for minors, ages 14-19, who seek part-time employment. Thestudent must secure the working paper application in person and signit in the presence of a school official. All applicants must submit proofof age and proof of a physical examination within the previous year.For information about working papers contact the Main Office atAmityville Memorial High School at 598-6550.

FOOD SERVICE

The Amityville UFSD operates a food service program that provides nutritionally balanced meals that encourage good eating habitsand supplies proper nutrition to help students be fueled for learning. Meals are offered daily at a reasonable cost in all schools. The purchaseof discounted prepaid meals is encouraged.

Please note: Families who meet specific financial criteria could be eligible for either free or reduced-price breakfast and lunch.Applications are mailed to each household in early August. A new application, one per family, must be returned to the Food Service Officeby September 30, 2009. Please provide money for lunch or a lunch from home until the eligibility letter is received. Please note that eachfoster child must have a new individual application each year. Applications are accepted throughout the school year. Menus and free andreduced-price meal applications can be found on the Amityville School District website (www.amityville.k12.ny.us).

SPECIAL NOTE TO PARENTS/GUARDIANS AND STUDENTS

You should not expect privacy with respect to student lockers, desks, and other school storage places. School officials have control overlockers, desks, and other storage places. Where reasonable cause exists, these areas are subject to search at any time without notice to you andwithout your consent.

EMERGENCY CLOSING OF SCHOOLS

If inclement weather causes Amityville Schools to close, inaddition to the district website (www.amityville.k12.ny.us), thefollowing radio/TV stations will carry the district’s closinginformation:

INTRODUCTIONThe Amityville UFSD has established a

computer network and is pleased to offer Internetaccess for student use. Students will be issued avisible form of identification, recognizing them asInternet users. This will provide students access toa variety of Internet resources. In order forstudents to use the Internet, students and theirparents and/or guardians must first read andunderstand the following acceptable use policies.

ACCEPTABLE USES:1. The computer network has been set up in order

to allow Internet access for educationalpurposes. This includes classroom activities,research activities, peer review of assigned workand the exchange of project-related ideas,opinions and questions via email, messageboards and other means.

2. Students will have access to the Internet viaclassroom, library and lab computers. Studentaccess is limited to times deemed appropriate bythe adult in charge.

3. Network users must respect resource limits andmust remain within an allotted disk space.Users are responsible for deleting old emails orother files that may take up excessive amountsof storage space.

4. Student use of the Internet is contingent uponparent/guardian permission in the form of asigned copy of this Acceptable Use Policy.Parents/guardians may revoke approval at anytime.

5. Material created and/or stored on the system is

not guaranteed to be private. Network andschool administrators may review the systemfrom time to time to ensure that the system isbeing used properly. For this reason, studentsshould expect that emails, material placed onpersonal web pages and other work created onthe network may be viewed by a third party.

6. Network users must keep their passwordsprivate. Accounts and/or passwords may not beshared.

7. Network users are expected to adhere to thesafety guidelines listed below.

UNACCEPTABLE USES:1. The network may not be used to download,

copy, or store any software, shareware orfreeware without prior permission from thenetwork administrator.

2. The network may not be used for commercialpurposes. Users may not buy or sell products orservices through the system without priorpermission from the network administrator.

3. Use of the network for advertising or politicallobbying is prohibited.

4. The network may not be used for any activityor to transmit any material that violates UnitedStates or local laws. This includes, but is notlimited to, illegal activities such as threateningthe safety of another person or violatingcopyright laws.

5. Network users may not use vulgar, derogatoryor obscene language. Users may not engage inpersonal attacks, harassment, or post privateinformation about another person.

6. Network users may not log on to someone else’s

account or attempt to access another user’s files.“Hacking” or otherwise trying to gain access toanother person’s or organization’s computersystem is prohibited.

7. Network users may not access websites, newsgroups or chat areas that contain material thatis obscene or that promotes illegal acts. If a useraccidentally accesses this type of information,he or she must immediately notify a teacher,librarian, and/or network administrator.

8. Network users may not engage in “spamming”(sending an email to more than ten people atthe same time) or participate in chain letters.

SAFETY GUIDELINES FORSTUDENTS IN SCHOOL AND

AT HOME:1. Never give out your last name, address or

phone number.2. Do not include your school name or town in

your screen name or profile.3. Never agree to meet in person with anyone you

have met online unless you first have theapproval of a parent or guardian.

The Amityville Parent Teacher Council (A.P.T.C.) extends a cordial welcome to all parents and guardians to join our organization and attend ourmeetings, which are listed in this calendar.

Our goal is continued communication between the community and the public school. “We attempt to involve parents, teachers andadministration in the mutual aim of a good educational system for all of the students.”

Mission Statement:** To promote unified efforts among the school, home and community in attaining the highest advantage of the physical, mental, cultural andsocial development of every child in the Amityville Public School District.** To function as advocates of public education for the children of the Amityville Public School District.
