EMERGENCY RESPONSE POLICY

Tri-Hi Pressure Ltd. intention is to ensure any risks arising from work activities are eliminated or reduced to a minimum. However, the company acknowledges that despite these measures it cannot be assumed that a major incident will never occur. Although such an incident is highly unlikely if all risks are adequately controlled, the consequence could be catastrophic. Emergency procedures are in place to ensure injury and damage limitation.

In the event of an emergency occurrence the emergency response plan is to be activated. All employees must safety shut down all work activities and vacate the work area immediately, and wait for instructions from management.

Emergency contact radio and phone numbers are placed at every exit in the shop and office and in each vehicle. There shall be adequate communications with all employees within the site area.

The organization provides information and training as often as is necessary to all employees (and other persons, such as contractor and visitors) to enable a better understanding of these matters. Any concern employees may have regarding the company's emergency procedures should be reported to a responsible person immediately. The company will then take the necessary measures to investigate and remedy the situation.

All personnel responsible for carrying out the emergency response plan must be aware of their responsibilities. Employees without specific responsibilities must be aware of how to evacuate the area safety.

The emergency plan shall be practiced and documented annually. If any shortcomings are found in the system, it will be reviewed immediately.

All injuries must be reported to Management or Supervisor.

EMERGENCY RESPONSE - ResponsibilitiesPLANNINGEach employee at Tri-Hi Pressure Ltd. has a responsibility to know his/her role should an Emergency occur. Copies of the Emergency Response Plan and list of employees assigned to specific roles are posted throughout the worksite for regular review. Field personnel will follow the emergency evacuation procedures posted at the worksite.

RESPONSIBILITIESIt is the responsibility of Management to: Reassess potential hazards at worksite. Recognize the possible consequences of each hazard. Ensure the availability of warning systems and communications. Define key responsibilities, and allocate needed resources. Test the plans for accuracy. Detail emergency response procedures. Detail evacuation procedures. Detail medical response procedures. Assess procedures and plans to determine effectiveness. Test plans and procedures against measurable standards.

It is the responsibility of the Supervisor and Employee to: Know your role during the emergency. Follow your supervisor’s instructions. Participate in emergency response drills Address unrecognized hazards at safety meetings.

FIELD EMERGENCIESTri-Hi Pressure Ltd. will discuss emergency response procedures with the client representative on site. Employees will take action to prevent emergencies from occurring, and follow all emergency response plans in place. Field emergencies will be recorded on an Accident & Incident Report form and forwarded to the nearest safety team member.

HOW TO EVACUATE A JOBSITE Know and follow the procedures outlined in the emergency response plan. During evacuation proceed calmly, but quickly. Close the door if you are the last out of a building. Do not lock any door during and

evacuation. If there is a build up of smoke, keep low to the ground. If possible cover your nose and mouth with a damp cloth. Once outside, report immediately to the muster point, so others will know you are safe.

Emergency Response Plan Tri-Hi Pressure Ltd.

1.0 INTRODUCTION

This Emergency Response Plan provides operating guidelines to meet those foreseeable emergencies, which may arise during the operation of Tri-Hi Pressure Ltd. in Whitecourt, Alberta. All worker’s help establish and maintain the emergency response plan.

Tri-Hi Pressure Ltd. shall, after consultation with the work place committee or the health and safety representative, prepare emergency procedures: (a) to be implemented if any person commits or threatens to commit an act that is likely to be hazardous to the health and safety of the employer or any of his or her employees; (b) if there is a possibility of an accumulation, spill, or leak of a hazardous substance in a work place controlled by the employer, to be implemented in the event of such an accumulation, spill, or leak; (c) to be implemented in the event of a failure of the lighting system; and (d) to be implemented in the event of a fire.

The objectives of the Emergency Response Plan is to enable Tri-Hi Pressure Ltd., to respond to all emergency situations, and to minimize Tri-Hi Pressure Ltd. exposure to loss and any impact on the environment from such situations. The objective will be met by providing for:

1. The safety of employees, contractors, visitors, customers, and the public.2. Continued security of the facility.3. An effective incident reporting system.4. Minimizing damage to the environment.

3.0 LOCAL AUTHORITIES

The RCMP can respond to the Whitecourt office within 15 to 20 minutes for incidents involving death, sabotage, fire or explosion, or criminal offences.

The ambulance can respond to the Whitecourt facility within 15 to 20 minutes. They can provide 5 ambulances to the site at a time and provide advanced life support for transfer to the hospital in Whitecourt. They also have an air ambulance and can call in more ambulances from the surrounding areas.

The Whitecourt Fire Department can respond to the Whitecourt facility within 5 to 10 minutes.

The Whitecourt Hospital in Whitecourt can provide triage and stabilization for injured workers. If necessary, they can coordinate transfer of casualties to Edmonton Hospitals for advanced medical aid. This would be done by air ambulance or ambulance.

The Poison Control Centre can provide information over the phone concerning how to deal with poisoning.

The Spill CO-OP is a group of companies and equipment to clean up a major spill.

Alpine Environmental Ltd. is a company that completes spill cleanup on and off site.

7.0 EMERGENCY RESPONSE PROCEDURES INTRODUCTION

Tri-Hi Pressure Ltd. policies regarding releases, fire and injury are to limit damage to persons and property to the fullest extent possible.

Every employee shall be instructed and trained in: (a) the procedures to be followed by him/her in the event of an emergency; and (b) the location, use, and operation of fire protection equipment and emergency equipment provided by the employer.

The emergency procedures shall contain: (a) an emergency evacuation plan, where applicable, or a plan for evacuating employees who require special assistance to be implemented in the event of a fire; (b) a full description of the procedures to be followed. For off-site locations, evacuation procedures should be identified and reviewed with workers prior to commencing work activities.

Given these policies, emergencies will be declared if any of the following occur: A major leak or spill; Fire or explosion; Serious injury or loss of life; Major H2S release.

Minor incidents typically involve incidents where no danger exists outside of company or contractor property, and where the situation can be handled entirely by Tri-Hi Pressure personnel.

Major incidents usually involve situations where safe operating control has been lost, resulting in or potentially resulting in fatalities; serious injury to Tri-Hi Pressure personnel, or contractors or the public; serious property damage; serious impacts to the environment; or major impact to surrounding communities. Emergency response plans will require implementation.

Appropriate emergency procedure must be initiated immediately after discovery that an emergency exists.

The main person responsible for dealing with any emergency will be the Manager of Tri-Hi Pressure Ltd. In the event that the Manager is not on duty, or available, the individual discovering the emergency must notify the next available individual on the following list:

7.1 SITE EMERGENCY CONTACT

The supervisor on site at the time of any emergency is charged with the evaluation and immediate response to rectify the situation or activate the Emergency Response Procedures.

In the event that the first Site Emergency Contact leaves the site for any reason he must notify the next available individual to assume the responsibilities of Site Emergency Response Contact.

7.1.1 SPILL RESPONSE

7.1.2 MINOR SPILL

The Operator(s) Shall:

Determine if the spill has a high hazard potential (i.e. vapour release, chemical reaction.)

Start up equipment and/or processes if it helps to contain a spill.

Shut down equipment and/or processes if it’s the cause of a spill.

Contain leakage and isolate source. Contact Site Emergency Contact.

The Site Emergency Contact Shall:

Take appropriate action in event of an injury or fatality (Sections 7.3 or 7.4).

Organize response team, contain spill and clean up spill. Confirm the identification and volume of spill.

The Safety Supervisor shall:

Complete Incident Report and contact management. Contact authorities within 24 hours or next working day (if

necessary) (Section 3.2). Put on the necessary personal protective equipment. Contain spill/leak with appropriate equipment (Section 13.2). Recover liquids and contain effected debris.

7.1.3 MAJOR SPILL

The Operator Shall:

Take appropriate action in the event of an injury or fatality (Sections 7.3 or 7.4).

Shut down operating equipment and/or processes. Contact the Site Emergency Contact person.

The Site Emergency Contact Shall:

Confirm the identification and volume of the spill. Proceed with Crisis Communication Schedule (Section 8.0). Contact Authorities (Section 3.2). Organize a Response Team.

The Safety Supervisor Shall:

Continue the Communication Schedule (Section 8.0). Notify residents of crisis (if necessary) (Section 5.0). Complete Incident Report and contact management if

necessary. Put on the necessary personal protective equipment. Contain the spill or leak if possible with appropriate equipment

(Section 13.2). Recover liquids and contain any affected debris. Arrange for the spill to be cleaned up.

7.2 FIRE RESPONSE

7.2.1 MINOR FIRE

The Person(s) Shall:

Shut down equipment in location of fire if possible. Attempt to extinguish fire with available extinguishers. If needed call for help from available personnel to help extinguish fire. Contact Site Emergency Contact. If fire is getting out of control, evacuate immediately.

Note: Do not attempt to extinguish truck containers, vessels or tanks.

No person shall, in a fire hazard area, (a) use any equipment, machinery, or tool of a type that may provide a source of ignition; or (b) smoke or use an open flame or other source of ignition.

Signs shall be posted in conspicuous places at all entrances to a fire hazard area: (a) identifying the area as a fire hazard area; and (b) prohibiting the use of an open flame or other source of ignition in the area. For off-site locations, fire hazard areas should be identified and communicated to employees prior to commencing work activities.

The Site Emergency Contact Shall:

Assess if authorities are required. If needed, organize a response team to extinguish fire. Take appropriate action in event of an injury or fatality (Sections 7.3

or 7.4). Organize a Response Team.

The Safety Supervisor Shall:

Complete Incident Report and contact management. Contact authorities within 24 hours or next work day if necessary

(Section 3.2). Put on the necessary personal protective equipment

7.2.2 MAJOR FIRE/EXPLOSION

The Site Emergency Contact Shall:

Initiate Evacuation Plan (Section 9.0). A person must not enter or work at a work area if more than 20 percent

of the lower explosive limit of a flammable or explosive substance is present in the atmosphere. Atmospheric testing results should be assessed before a worker is exposed.

Take appropriate action in the event of an injury or fatality(Sections 7.3 or 7.4).

Organize a response team. Contact Authorities (Section 3.2). Proceed with Crisis Communication Schedule (Section 8.0). Preserve and protect the accident site until investigations are complete,

following the end of the emergency.

The Safety Supervisor Shall:

Continue the Crisis Communication Schedule; (Section 8.0). Notify residents of crisis, if required (Section 5.0). Complete Incident Report and contact management. Put on the necessary personal protective equipment. Shut down operating equipment and/or processes. Secure area to extent possible. Shut off power. Flammable substances stored or used at a work area (that is considered

not a hazardous location) will not be in sufficient quantity to produce an explosive atmosphere if inadvertently released, are not stored within 30 meters of an underground shaft, are not stored in the immediate vicinity of the air intake of ventilation supply system, an internal combustion engine, or the fire box of a fired heater or furnace, and are stored only in containers approved to CSA, or ULC Standards. CSA B376-M1980 R1998), "Portable Containers for Gasoline and other Petroleum Fuels." ULC Standard C30-1995, "Containers, Safety".

7.3 INJURY RESPONSE

7.3.1 MINOR INJURIES

Injured Person Shall:

Report to Site Emergency Contact immediately. Apply first aid as necessary.

The Site Emergency Contact Shall:

Help the injured person by applying first aid. Take injured person to medical help, if necessary.

The Safety Supervisor Shall:

Complete incident and first aid report and contact management if necessary.

7.3.2 MAJOR INJURY

The Site Emergency Contact Shall:

Organize a response team. Contact Authorities (Section 3.2). Contact Crisis Communication Schedule (Section 8.0). Ensure unobstructed access for emergency response personnel. Receive and direct emergency response personnel to the accident site.

The Safety Supervisor Shall:

Continue the Crisis Communication Schedule (Section 8.0). Notify injured person’s family (Section 4.0). Complete Incident Report. Contact management. Put on necessary personal protective equipment and first aid

equipment. Shut down operating equipment and/or processes in location of injured

person or persons. If required, contain source of leak or extinguish small fire. Move injured person or persons to safety and start first aid treatment.

7.4 FATALITY

The Site Emergency Contact Shall: Organize a response team. Contact Authorities (Section 3.2). Contact Crisis Communication Schedule (Section 8.0). Preserve and protect the accident site until investigations are complete. Receive and direct emergency response personnel to the accident site. Continue the Crisis Communication Schedule (Section 8.0).

Note: RCMP to be notified immediately. RCMP will notify family members.

The Safety Supervisor Shall: Put on the necessary personal protective equipment. Shut down equipment and/or processes in location of fatality. If required, contain source of leak or extinguish small fire. Do not move person or persons, if it is not necessary. Complete incident report. Notify management.

7.4 H2S OR GAS RELEASE, LEL CONDITION

Upon discovery of release, staff is to acknowledge the condition. All persons to take appropriate action and clear work area and proceed to meeting point upwind away from hazardous area.

The Site Emergency Contact Shall:

Take appropriate action in the event of an injury/fatality (Sections 7.3 or 7.4). Organize a response team. Contact Authorities, if required (Section 3.2). Proceed with Crisis Communication Schedule, if required (Section 8.0). Protect the accident site until investigations are complete.

The Safety Supervisor Shall:

Continue the Crisis Communication Schedule, if required (Section 8.0). Notify residents of crisis, if required (Section 5.0). Complete incident report. Contact management. Put on the necessary personal protective equipment. Shut down operating equipment and/or processes, if possible. Contain source of leak. Monitor site and surround area outside of site.

7.6 MAJOR PROPERTY DAMAGE

The Operator Shall:

Shut down operating equipment and/or processes in the effected area, if safe to do so.

Stay clear of effected area. Contact Site Emergency Contact.

The Site Emergency Contact Shall:

Take appropriate action in event of an injury or fatality (Sections 7.3 or 7.4). Organize response team to stabilize effected area to allow for continued

operation, if possible.

7.7 NATURAL DISASTERS

7.7.1 LIGHTENING STORM

Prepare:

Shut down equipment and processes if working outside.

Action:

During an electrical storm, work activity (i.e. worksite) immediately around worksite should be temporarily stopped until danger has passed.

7.8 BOMB THREAT

First Actions:1. Listen carefully.2. Be calm and courteous.3. Do not interrupt the caller.4. Obtain as much information as you can include background noises,

speaker’s tone, accent, gender, manner of speaker (excited, calm, etc.), and mannerisms (educated, stutter, etc.), and the exact wording of the threat.

5. Questions to ask:a. What time will the bomb explode?b. Where is it?c. Why did you place the bomb?d. What does it look like?e. Where are you calling from?f. What is your name?

6. Notify management and the Safety Supervisor immediately after the caller hangs up.

7. Management will then contact the R.C.M.P.

Search Guidelines:1. Do not touch anything – report any suspicious objects.2. Cooperate fully with the R.C.M.P.

Evacuation Guidelines:1. Remain calm.2. Follow R.C.M.P. instructions.3. Remove your personal effects, such as purses, briefcases, etc., when

evacuating.

8.0 CRISIS COMMUNICATION SCHEDULE

Each person notified will discuss the situation with the person providing notice, and they will jointly decide if the notification process shall escalate, and who will be responsible for proceeding with further notification.

9.0 EVACUATION PLAN

In the event of an emergency that requires evacuation, all employees, customers, visitors, and contractors must immediately leave the work area and report to the facility evacuation meeting point. The main office evacuation meeting point is, (office) on the main road in front of the office. Field sites, are as directed by the site superintendent, in the event of an alarm or announcement of evacuation.

The Site Emergency Contact Shall:

Take all work permits; Report to the road in front of the office; Account for all personnel; Organize a response team, if required; Contact Authorities, if required; (Section 3.2); Proceed with Crisis Communication Schedule, if required. (Section 8.0).

The Employees Shall:

Advise all visitors, customers, and contractors of alarm, if possible; Shut down operating equipment and/or processes, if possible; Report to the road in front of the office, (field sites) - as directed by the site

superintendent.

No person shall be allowed to return to his or her respective work areas until an “all clear”, has been given by the Site Emergency Contact or designate.

10.0 TRAINING AND PRACTICE DRILLS

All employees will receive orientation on their first day of work. All training records will be filed in an employee file by the Safety Supervisor. Annual ERP drills will be conducted to ensure awareness and competency of all Tri-Hi Pressure Ltd.staff.

10.1 PERSONNEL TRAININGAll personnel will have the following training: H2S Training Transportation of Dangerous Goods Act and Regulations TDG (if applicable) WHMIS PST Training First Aid Review of all Company Safety Policies, Rules and Safe Work Procedures Driver Evaluation Job Hazard review

The courses the new employee and contractors may need (depending on his/her job duties) is as follows: Fire Training Confined Space Training Air Brake Course Defensive Driving Course Petroleum Safety Training Forklift Training

10.2 PRACTICE EXERCISES AND DRILLS

Exercises will be conducted at least once every (12) months. These exercises may be of a significant scope involving appropriate outside agencies (minimum telephone contact to verify emergency contact numbers). The scenarios for the exercises will be both relevant and realistic, based on current activities.

Each exercise will be reviewed upon completion for the effectiveness of the planned response(s). The minutes of the review will be recorded at a safety meeting and the findings will have action items assigned and follow-up documented.

Drills

Fire drills/facility evacuation will be carried out at least (12) months, prior to a Safety Meeting. Each drill will be reviewed for effectiveness of plan and execution and with the review recorded in the minutes of the meeting. Any action plans arising from this review will be recorded and implemented.

11.0 PLAN EVALUATION AND UPDATES

Based on actual incident reports and practical drills, revisions of the plan will be promulgated, reviewed and implemented. Amendments will be dated, manuals updated and reviewed with personnel.

EMERGENCY PROCEDURE FOR MAN-DOWN (SOUR LOCATION)

All personnel entering a sour gas H2S location must be aware of the properties and toxicity of Hydrogen Sulphide Gas. They must be able to recognize its pressure and now how to protect themselves and co-workers from its lethal effects. All people on-site must be familiar with H2S and its properties either through the H2S Alive course, or a similar recognized H2S Awareness course. In the undesirable event of a person(s) being knocked down by sour gas, these seven steps should be followed as outlined by the H2S Alive course and are as follows.

If a workers clothing/and or skin is contaminated with a flammable or combustible liquid, the worker must: a) avoid any activity where a spark or open flame may be created or exists, b) remove the clothing and, c) ensure the clothing is decontaminated before it is used again. If a workers skin is contaminated the worker must wash the skin at the earliest possible time.

Step 1: Evacuate If you see someone collapse in or around your area, get out of the area.Under NO circumstances should anyone initiate a rescue without putting on the proper respiratory protection.

Step 2: Alarm Call for help or have others call for help.

Step 3: Assess Try to determine the amount of people involved, where the victim(s)are and any potential injuries.

Step 4: Protect Protect yourself by putting on the approved respiratory protection. i.e.SCBA or SABA before attempting to rescue and have backup.

Step 5: Rescue Remove victim(s) to safe area.

Step 6: Revive Administer first aid as required. If the person is not breathing, beginArtificial Respiration, if required, start CPR.

Step 7: Medical Aid Arrange transportation for victim(s) to the nearest hospital or medicalfacility. A doctor must evaluate anyone who has been knocked downby H2S.

An internal combustion engine in a hazardous location has a combustion air intake and exhaust discharge that are equipped with a flame arresting device, or located outside the hazardous location. Whenever possible, internal combustion engines should be located outside the hazardous location.
