Discipline: Our summer enrichment will follow the same core rules as listed in the student handbook. Students are expected to listen to teachers’ instructions, be respectful, and behave responsibly at all times. Should students choose to not follow rules, they will be sent to the office. The site coordinator will assign consequences and contact the parents. Students will be dismissed from program if behavior is a problem. Please remember, summer enrichment is a privilege offered to students who want to prevent summer learning loss and enjoy the activities offered.

Nutrition: We will offer a nutritional snack midmorning, but we will not offer breakfast or lunch. Please ensure your child eats breakfast before getting on the bus. Students may bring bottled water (ONLY) to school. No candy, sodas, Kool-Aid, chips, etc. should be brought to school.

Pictures: We regularly share pictures of our students’ accomplishments on Facebook, the school website, and the newspaper. If you DO NOT want pictures of your child published, please see your site coordinator.

PERMISSION FOR PARTICIPATION IN OFF-CAMPUS ACTIVITIES

TO: Parents and Guardians of QSD 21st CCLC StudentsFROM: Tara Tucker, 21st CCLC Program DirectorRE: Permission for bus travel to off-campus sites As part of our after-school, summer-school, and enrichment programs, we will sometimes visit other facilities, campuses, parks, and buildings in our area. Most of our activities require an active, hands-on component that might be better learned in an off-campus setting. (Examples: Our tennis and soccer classes will travel to the city complex to use the public courts. Some classes will visit the Clarke County airport and Clarkco. Some classes will tour businesses in town.)

Please sign and date below to indicate that you have been made aware of this information and give permission for your child to participate in off-campus activities.

My child, ________________ _________________, has permission to travel to and participate in any off-campus activities under the supervision of the QSD teachers, bus drivers, and other staff. I understand that my child will be required to comply with all guidelines, ethical expectations, and school rules and that failure to do so can result in his/her being disqualified from participation in future school trips.

I also give permission for my child to be given medical treatment by the nearest medical care personnel or at the nearest medical care facility. I understand that the QSD employees are not responsible for any injuries or accidents that may occur during this trip or any expenses that may incur due to any accidents.
