The Clinical Impact of Intra-abdominal Infection (IAI)

Appropriate Antibiotic Therapy for IAI

• Antibiotic therapy is an important adjunct to surgery and supportive therapy.

• Antibiotic treatment should cover appropriate gram-positive and gram-negative, aerobic and anaerobic bacteria.

• Most of the available antibiotics for the treatment of IAI generally require multiple daily doses or combination multidrug therapy to provide adequate coverage of all important pathogens.

*Ertapenem is not appropriate for nosocomial infections where Pseudomonas and Acinetobacter spp. have been identified. Ertapenem had potent in vitro activity against the majority of the anaerobic clinical isolates tested; limited activity was noted against C. difficile. Methicillin-resistant staphylococci including Staphylococcus aureus (MRSA) and enterococci including Enterococcus faecalis and E. faecium are resistant.

Was as effective as pip/tazo in complicated IAI. Appropriate gram-positive and gram-negative, aerobic and anaerobic coverage, appropriate for community-acquired polymicrobial infection.14* Ertapenem is active against ESBL and AmpC beta-lactamase-producing Enterobacteriaceae.15

Ertapenem: An Advantage Over Current Therapy

Fluoroquinolones Limited anaerobic coverage. Increasing prevalence of ciprofloxacin-resistant gram-negative bacilli.8 May select for or promote multidrug-resistant gram-negative bacilli.9-11

Piperacillin/tazobactam Expanded coverage (e.g., Pseudomonas) may not be advantageous in IAI; more useful in higher-risk patients.12-13

Aminoglycosides Not widely used due to risk of toxicity (e.g., ototoxicity, nephrotoxicity) and difficulty of obtaining therapeutic drug levels within a narrow therapeutic range.14
