To whom this may concern,

I am writing on behalf of the Southeastern Utah Jr. Livestock Show to Thank-you for your

continued support.

The Southeastern Utah Jr. Livestock Show is held each July in Ferron, Utah. As one of only five livestock

shows open to aii4-H and FFA youth in the State, the Stock show attracts over 300 exhibitors from

throughout Utah. The show is the culmination of their efforts to produce the perfect lamb, steer, goat

or hog. Approximately 400 market animals are exhibited at the show each year.

Events at the Livestock show include a Beef Rate of Gain Contest, Written Test, Judging Contest,

Record book Contest and Showmanship Contests in addition to judging of breeding and market animals.

Every year the Livestock show awards over $24,000 in prizes and awards. Your support is needed to

provide prizes for e>shibition and to support the sale of market animals. .. ' The Southeastern Utah Junior Livestock Show is a 501 C-3 nonprofit organization.

Thanks again for your support in 4-H and FFA youth, in both Emery and Carbon County. If you

have questions or need more information, please contact, Cody Jensen 435-820-6824, Paul Gilbert

435-609-1266 or Cindy Fillmore 435-820-0742.

Sincerely,

SOUTHEASTERN UTAH JUNIOR

LIVESTOCK SHOW July 10-13, 2019 FERRON, UTAH

Each year youth from around our state enthusiastically choose 4-H and FFA Junior Livestock projects to compete at their local shows always hoping to find that certain animal to win a blue ribbon.

In addition to the monetary costs, these projects entail hours of labor and patience through feeding, training, and preparing their animal for the show. The youth, in the meantime, learn important and essential life skills such as communication, cooperation and compassion for the humane treatment of animals.

" Proceeds from the sale of their projects most often help fund a college education, further enabling these youth to become contributing members of society.

The real purpose of 4-H and FFA livestock programs is to provide opportunities for young people to develop character.

Please support the 2019 Southeastern Utah Junior Livestock Auction.

HOW THE SALE WORKS An animal mar, be purchased and kept for private use or

may be "boosted ; with a local packer taking the animal at the market price. The buyer pays the boost price, which is the difference between the selling price bid by the buyer and the actual market price.

If you do not want to purchase the meat for private use but wish to support the youth, a donation of $25, $50, or any amount you wish will mean a great deal to the exhibitors.

If you are unable to attend the sale, a member of the Auction Committee will be glad to take care of bidding for you. Please fill out the attached Buyer's Order Form and return it to the Stockshow office. Any support you can give this worthwhile endeavor will be

appreciated.

TERMS & CONDITIONS OF SALE Animals will be sold on a per head basis. The base price

for each animal will be noted on the sale order. Payment must be received on all animals before being

released to private individuals or packers, unless prior arrangements have been made with the Show President.

Buyers are required to mals.e immediate payment upon receipt of invoice. Interest of 1.5% will be charged monthly on any amounts owing 30 days after date of first notice. Southeastern Utah Junior Livestock Show reserves the right to refuse bids from or sales to any buyer.

The sales committee, auctioneer, and staff of the Southeastern Utah Junior Livestock Show accepts no liability for injury or death of sale animals. Animals are the responsibility of the exhibitor until packer or individual buyer accepts ownership.

All sales are final. There will be no re-sale of animals. In the event of a tie or dispute, the auctioneer's decision

will be final. All bids are to be made by bona fide bidders or by

acceptance. When an animal is turned to the packer or the buyer

in the sale ring, it becomes the property of the packer or that buyer. Any changes must be done by private treaty with the packer or buyer and not involve the Southeastern Utah Junior Livestock Show. Packers have the right to refuse a sick or injured animal at the time of loading and ownership of the animal reverts to the exhibitor.

May 22nd City Council Meeting 1 message

Amanda Mcintosh <[email protected]> To: [email protected]

Good Afternoon, Sherrie!

Mail - May 22nd City Council Meeting

Sherrie Gordon <[email protected]>

Man, May 6, 2019 at 4:15PM

As per our conversation last week, I am hoping to be put on the City Council Meeting for Wednesday, May 22, 2019. I will be proposing the following:

My name is Amanda Mcintosh. I am the Suicide Prevention Specialist for the South East Utah Health Department. I have been involved in suicide prevention and awareness since July 21, 2014, when my husband died by suicide in front of me. I have been an active member of the HOPE Squad of Carbon/Emery/Grand County, and advocate for prevention and awareness, and have recently left a career in banking to take on my current position. The month of September is slated for Suicide Prevention Month. More specifically, the week of September 8-14 -2019 is recognized as Suicide Prevention Week. As one of the leading counties for suicides in the state of Utah and Utah being ranked 5th nationwide for suicide completions, it is time for more prevention and awareness.

I will be proposing/asking permission for a number of things: 1. The HOPE Squad of Carbon/Emery/Grand Counties would like to use a portion of the Price Peace Gardens

to "plant" 100 wooden, faceless angels to symbolize the number of community members we have lost to suicide since 2013 (as of Monday, May 6, 2019, that number is 91.) The angels are 6 inches high by 4 inches wide by quarter inch thick. They will be mounted to wooden dowels and hammered into the grass and would sit approximately 18 inches off the ground. This section will be roped off to help keep people from walking on them. This memorial would be set up on Saturday, September 7, 2019, and would be taken down on Saturday, September 13,2019. Square footage of the area can be provided at a later date. I would like to ask for help promoting the showcase by way of advertisement/donation.

2. The HOPE Squad would like permission to display a banner in the peace garden explaining the angels and bringing attention to the HOPE Squad, the partners involved, and the Suicide Prevention Hotline. The size of the said banner has not been determined (waiting for quotes and partnerships) but we are looking at 8'x4' to be the largest size. This banner would also be pegged into the ground.

3. The HOPE Squad would like permission to set up a night display/slide show against the wall of the museum that would run on a loop until approximately 10:00 PM each night. This slide show would include pictures of the loved ones lost to suicide (should the survivors participate and are comfortable sharing pictures.) If participation is slim with pictures, we would like to project the Suicide Prevention Hotline number as well as the HOPE Squad information.

4. The HOPE Squad would like permission for a Candle Light Vigil to take place at the Peace Garden with the Angels on Tuesday, September 10, 2019, at 7:30-8:30 PM. This is World Suicide Prevention Day and to show support, it is asked to light a candle at 8:00 PM in remembrance.

5. The HOPE Squad would like permission to use the Peace Gardens for our 6th Annual Suicide Prevention Walk slated for Friday, September 13, 2019, at 6:00PM to be finished by 9:00PM. We will have food, inspirational speakers, and a walk around the block.

We are hoping to start advertising for these events at the beginning of August. I will be getting in contact with Kathy soon to discuss possible entryway signage. I am open to hearing your thoughts, suggestions, and ideas with regard to bringing awareness to suicide in our community.

Misc. Information: A similar garden will be put together in Emery County. Details to follow if needed. If the survivors of suicide would like to sponsor an angel in memory of their loved one, they will be able to purchase the angel from the HOPE Squad of Carbon/Emery/Grand County for around $5.00 and in exchange will be able to decorate the angel and receive a shirt with HOPE Squad logo and inspirational message. They will need to have the decorated angel back to the SEUHD no later than Wednesday, September 4, 2019.

Is this what would need? Did I miss anything?
