Don’t Let This Happen To You! 013 What Happened: A bucket worker using a top handle saw for ground cutting on a very steep bank, leaned forward facing the slope, when a kickback occurred. The saw contacted a piece of re-bar and kicked; striking the operator in the neck. Damage/Injury: Cut to the throat. So lucky the cut was not deeper nor hit the major vein and artery just a few inches away!! Human Performance Trap – Physical Environment – Change/Off Normal – the steep bank put the saw work closer to the operator, increasing the “line-of-fire” hazard. Banks of this steepness are not an everyday occurrence in most areas.

Human Performance Error Prevention Tool – Use proper PROCEDURE – S.T.A.R. – the proper procedure is to stay out of the line-of-fire at all times. When faced with a different or change of environment, Stop – Think – Act – Review. Cutting with the saw angled/tipped to the right would have changed the line-of-fire!

Follow PROCEDURE and this will not happen to you!

No Permita que Ésto le Suceda a Usted! 013 Lo que sucedió: Un trabajador de cubo utilizó una sierra con el mango arriba para hacer cortes en una cuesta muy empinada, se inclinó hacia adelante de frente a la cuesta, cuando ocurrió el contragolpe. La sierra se atascó con algo y dio un contragolpe que golpeó al operador en el cuello. Daño/Lesión: Cortadura en el cuello. El operador fue muy afortunado que la cortadura no fuese más profunda y que no cortó las arterias ni venas principales por muy poca distancia!! Trampa del Desempeño Humano – Ambiente Físico – Cambio/Fuera de lo Normal – la cuesta empinada puso al trabajo de sierra más cerca del operador, incrementando la “línea de fuego”. Cuestas tan empinadas no son comunes en la mayoría de las áreas.

Herramienta de Prevención de Errores del Desempeño Humano – Use el PROCEDIMIENTO apropiado – S.T.A.R. – el procedimiento apropiado es mantenerse alejado de la línea de fuego en todo momento. Cuando se enfrente a un ambiente cambiado o diferente, Pare – Piense – Actué – Revise. Cortando con la sierra en un ángulo inclinado hacia la derecha hubiera cambiado la línea de fuego!

Siga el Procedimiento y ésto no le sucederá a usted!

Don’t Let This Happen To You! 016 What Happened: A climber using a rear handled saw to trim a spruce tree and cut vines, unknowingly cut through the tail of his climbing line, below the friction hitch. As the climber descended several feet, he pulled the friction hitch off the end of the cut rope; falling 38’ to the ground. Damage/Injury: Impact trauma to the face and head, bleeding on the brain, a collapsed lung, and Doctors induced a coma. Human Performance Trap – Over Confidence – Time Pressure – Operating a saw too close to a climbing line. Not checking the climbing line prior to descending.

Human Performance Error Prevention Tool – Use proper S.T.A.R. – Stop – Think – Act – Review. The rope is your life line. Check to see it is in good condition; make sure it is free from tangles, limbs and debris. Place one hand on your knot and the other around the tail end of the rope as a brake hand and discover rope damage before it is too late!

No Permita que Esto le Suceda a Usted! 016 Lo que sucedió: Un trepador cortando pequeños árboles y enredaderas usando una sierra con mango, sin darse cuanta cortó a través de la punta de su línea de trepar, debajo del enganche “friction hitch”. A la que el trepador descendía varios pies, él halo el enganche pasando el extremo de la cuerda cortada. El trepador cayó por 38 pies al suelo. Daño /Lesión: Trauma a la cara y la cabeza, sangrado en el cerebro y un pulmón colapsado. El trepador estuvo en coma inducida médicamente. Trampa del Desempeño Humano – Exceso de Confianza – Operando una sierra muy cerca de una línea de trepar. No verificando la línea de trepar antes de descender.

Herramienta de Prevención de Errores del Desempeño Humano – Use S.T.A.R. – Stop (Pare) – Think (Piense) – Act (Actúe) – Review (Revise). La cuerda es su línea de vida. Verifique para asegurarse de que está en buenas condiciones, asegúrese de que no está enredada, ni atascada a ramas ni a escombros. Coloque una mano en su nudo y la otra alrededor de la punta de la cuerda para actuar como un freno y descubrir daño a la cuerda antes de que sea muy tarde!
