Goal # 1 Health and Wellness
I will learn how to keep fit and eat right at school.
This goal is important because I would like to be healthy and fit so I don’t feel so tired.
I will work on this goal by eating well at lunch time at school and I will work to be fit.
Who can help me myself and my mom and dad and some other people.
Goal # 2 School work
I will try to work independently with out an S E A when I know I can do it .
This goal is important because I have to be independent for the future.
I will work on this goal by challenging myself to not have an S E A.
Who can help me: Me , teachers and S E A.
Goal #3 Transitions Planning
I will get my grad photos taken.
This goal is important to me because I would like to get them done so I can have
some when I leave the school.
I will work on this goal by going to get them done.

Goal #4 Transitions Planning
i will look into college programs (early childhood education)
Kwantlen college – Douglas college – Surrey/Delta continuing education.
Goal #5 Self - Determination
I will learn about my learning needs by
reviewing my IEP and to explain my disability.
This goal is important because it is apart of my life and what I go throw.

I will speak with a clear voice and use active listing skills
This goal is important because I talk all the time and I’m just so outgoing

I will cook simple meals independently gets a drivers license volunteer in the
community.
This goal is important because I would like to do those things.
