Founder / Director – Operations and Head of HSSE & Q
BACKGROUND
Mr. Lester Ifill vision and passion for inspection services over the past fourteen (14) years, resulted in the
organization known as RQS-Asset Integrity Services Limited. He brings to this vision, a wealth of sixteen
(16) years experience in the NDT Industry and HSSE & Q Management Systems documentation and
implementation. Also Mr. Ifill holds the position of Director, Operations and HSSE&Q.
RESPONSIBILITIES
Mr. Ifill is / and would be responsible for the planning, implementing, and execution of any assigned
contract and is the designate Company Representative for the purpose of observing and inspecting
the services being carried out for our Clients. He would also be responsible for adhering to and
carrying out all safety advices as instructed by Clients in connection with the scope of works of ANY
contract and provide the adequate and suitable protective clothing & devices of an approved
standard and instruction in its used, as well as information on instruction, training and supervision. Also, all employees (directors; contractors and sub-contractors) shall be required to used,
maintained and inspect the PPE provide to them by management.
He would specifically oversee and ensure:
1) Overall responsibility for the planning and operation of performing Integrity Inspection, as stated
within the required scope.
2) Delegate responsibilities to NDT Team through the channel of the NDT Supervisor.
3) Coordinate with supervisor and client.
4) Establish or adjust work procedures to meet schedules

Mr. Berkeley holds the position of Administration & International Business Development Director at RQS-
Asset Integrity Services Limited (RQS-AISL). He brings a wealth of experience and knowledge in his area

I am writing this letter to thank RQS-Asset Integrity Services Limited (RQS-AISL), as a general contractor, for a job well done on supplying the highest level of Inspection Professionals – Level II &III for our June 7, 2011 – July 23, 2011 Turn-A-Round (TAR) Project.
Within the seven weeks of the project, the inspectors/inspections comprised of:- 1-off Piping Engineer. 2-off Piping Inspectors (570) 2-off Certified Welding Inspectors (CWI) 4-off Non-Destructive Testing Inspectors (NDT)
All provided technical services to support the piping works as requested. These included the preparation of a materials take off sheet and associated specifications for the piping and fittings for the proposed slurry line change out. Works, also carried out, included installation of slurry polymer PVC piping system and modification to the associated slurry piping system.
Works were also done on the design and supervision of the fabrication of the 2 belt press slurry tank platform extensions and installation of a man hole on one of the slurry tanks.
The inspectors provided technical services on the NDT of some of the welding activities on the plant as directed and were also associated with the compilation of the Quality Assurance / Quality Control documentation preparation associated with the NDT activities.
Even though there were several challenges during the course of the TAR project, the inspectors were able to maintain efficiency within the project. Their attention to the specifics of the project has been laudable, and general performance met our high standards.
Again, I would like to express sincerest thanks to all your teammates for the safe and efficient completion of work on our TAR project.

installation at Heli-Deck; Main-Deck; Mezzanine-Deck; Production-Deck; Cellar-Deck and Working
Point (Jackets), both topsides and undersides.

Within the eleven weeks of the project, the inspector / inspection comprised of:-
1 – Level III Nace Certified Coating Inspector.
Technical support services were provided to ensure that proper PFP was install and intact to prevent a fire
risk from breaching critical areas on platforms should there be a need for an emergency evacuation of
personnel’s on platforms.
Such critical areas are:-
Exposed fire water ring main supports and supporting structure.
Ensure blast wall is protecting living quarters and items behind the blast wall from fire to
facilitate a platform evacuation if made necessary.
Ensure PFP is on Accommodation Building; Electrical Building; Generator Control Room and
Compressor Control Room.
Ensure PFP is on ESD Valves, located on the pig receivers to allow for a minimum time period
of 10 minutes to permit the ESD Valves to close in an emergency.

Conduct AUT – Phased-Array Welding Inspection on 5” O.D. Butt Weld Joints on Outlet Pipe from
S1’s West Boiler Feed Pump.
Within the project, the inspectors / inspection comprised of:-
1 - Project Supervisor / Reliability Engineer.
1 – NDE / NDT Level II Tech.
Welding was inspected using AUT Phased-Array on process piping to eliminate zero loss of on site
production; reduce overall project cost, duration and increase welding productivity while increasing safety.

June 14th – August 30th, 8,000 BBL Storage Tank Construction Project for SOL EC LTD.

Within the project, the inspector / inspection comprised of:-
1 – API-653 NDE / NDT Project Manager.
1 – Certified Welding Inspector (CWI).
1 – Level III Nace Certified Coating Inspector.
The NDE / NDT Project Manager responsibilities entails verifying all Inspection activities applicable to the
Inspection and Test Plan, Procedures, Codes and Standards of all Contractors on site.
This Includes :- Welding Inspections; Vacuum Box Testing; Pad Plate Leak Testing; Diesel Oil or Unseed
Oil Testing; Painting & Coating Inspections etc.

Furnace Area Package 1 & 4 Bustle Line and Cricital Items Package.

SCOPE OF WORK
Conduct Inspection of all Off-Shore Box Boom and Lattice Boom Cranes on Central Processing
Platform; Gas Export Platform; Canteen 1 Platform; Kairi 1 & 2 Platforms and Aripo Platform.
Within the four weeks of the project, the inspector / inspection comprised of:-
1 – Certified Mobile Crane Inspector.
Technical support services were provided to carry-out detailed visual checks on BOOM, PEDESTAL &
other key STRUCTURAL ELEMENTS.
Such as:-
Boom Tip; Boom Welds; Swing Circle Assembly; Luffing Cylinder Attachment Points; Crane
Hook and Safety Latch; Vertical Support & Foundation; Safety Devices; Overhaul Ball & Hook;
Operator’s Cab & Station and Load Charts just to name a few.

March 1st – April 25th , Structural Platforms Project

SCOPE OF WORK
Conduct Level I Visual Inspection of all Critical 1 Structural Joints & Members on Central
Processing Platform; Gas Export Platform; Canteen 1 Platform; Kairi 1 & 2 Platforms and Aripo
Platform.
Within the eight weeks of the project, the inspector / inspection comprised of:-
1 – Level III Nace Certified Corrosion Technician.
Technical support services were provided to identify any existing cracks; signs of excessive corrosion;
bent, missing or damaged members; missing or damaged bolts; and any indications of obvious overloading,
design deficiencies and any operational usage that is inconsistent with the original design intent of the
installation at Heli-Deck; Main-Deck; Mezzanine-Deck; Production-Deck; Cellar-Deck and Working
Point (Jackets), both topsides and undersides.

March 1st – April 25th, Passive Fire Protection Project

SCOPE OF WORK
Conduct Level III Passive Fire Protection (PFP) Visual Inspection on all Piping Supports; Blast
Walls; Buildings; and ESD Valves and Actuators on Central Processing Platform; Gas Export
Platform; Canteen 1 Platform; Kairi 1 & 2 Platforms and Aripo Platform.
Within the eight weeks of the project, the inspector / inspection comprised of:-
1 – Level III Nace Certified Coating Inspector.
Technical support services were provided to ensure that proper PFP was install and intact to prevent a fire
risk from breaching critical areas on platforms should there be a need for an emergency evacuation of
personnel’s on platforms.
Such critical areas are:-
Exposed fire water ring main supports and supporting structure.
Ensure blast wall is protecting living quarters and items behind the blast wall from fire to
facilitate a platform evacuation if made necessary.
Ensure PFP is on Accommodation Building; Electrical Building; Generator Control Room and
Compressor Control Room.
Ensure PFP is on ESD Valves, located on the pig receivers to allow for a minimum time period
of 10 minutes to permit the ESD Valves to close in an emergency.
