FAA Wings Program – Safety Seminar
DEATH IS NOT AN OPTION
By Jeremy R.C. Cox – APIA FCC + Radar/CPL IR SE ME
This presentation uses a ‘shock-factor’ to make you all understand that …
‘Death is Not an Option’
…Just as long as you always ‘think safety!’
Failure to follow procedures might result in Death!
Common Themes of Accidents: Flawed Assumptions – Human Error – Organizational Lapses
– Pre-existing Failures – Unintended Effects
Death is not a component part of a formal risk management assessment.
Accident Threat Categories:Bird Hazards – Cabin Safety/Hazardous Cargo – Fuel Exhaustion
– Flight Deck Layout/Avionics Confusion – Crew Resource Management– Fuel Tank Ignition
Ask yourself…”Am I ready to undertake this flight, and have it conclude both safely, and successfully?”
Ask Yourself, “do I have…”“Enough Oxygen?” “Enough Fuel?” “Enough Rest?” “Enough Visibility?”
“Enough Payload Margin?” “Enough Information?” “Enough Training?”
Ask yourself…”What is the Risk Likelihood of My Flight Today”
Ask Yourself, “do I have…”“A Proper Weight & Balance Report?” “An Airworthy Aircraft?”
“The Latest Weather Report?” “Up-to-date Charts?”“A Safety Pilot?” “An Exit Strategy?”
Ask yourself…”Have I managed to Identify all possible Hazards that I
expect to encounter on My Flight Today?”
When was the last time that you: Had Recurrency Training? – Read Your Flight Manual ?
– Used a Checklist? – Did a Weight & Balance…For Real?
Generally Flying is not overly Dangerous…
…Hitting the Ground Definitely Is!!
Safety is no accident
Please get trained, stay current, and most importantly please develop the proper mental attitude towards your flying
