CHAMP-Path Envelope

Inside the envelope:Two allocation labels identifying patient’s diagnosis and randomly-assigned case number and care group (UC=Usual; PC=Pathway)

Use one label for the Patient Registration Form. If PC, keep the other label for the Pathway Care Referral Form, otherwise leave ‘UC label’ in envelope.

Return Form and Envelope to ED Pharmacy, for pharmacy staff to sign.

If patient has been assigned to Pathway Care (PC)…..

Initiate Pathways through QuadraMed

Fill Out Pathways Referral Form

Give to Nurse Manager or Nurse-in-Charge to fax to the Medicine Pathway Team

This form can be found at:(1)ER Counters(2)Dept of Med office door

Affix the Patient Label.
