Inside the envelope:Two allocation labels identifying patient’s diagnosis and randomly-assigned case number and care group (UC=Usual; PC=Pathway)

Use one label for the Patient Registration Form. If PC, keep the other label for the Pathway Care Referral Form, otherwise leave ‘UC label’ in envelope.

Give to Nurse Manager or Nurse-in-Charge to fax to the Medicine Pathway Team

A: False; Pathways Referral Form is only for those patients who have been assigned to pathway care, not usual care.
