In support planning you control the process not the answers……

Things you’ll need to find out before planning……….

What part(s) of the planning does the person feel confident to do themselves?

Who does the person want to help with the plan?

Where do they want the planning to happen?

What kind of planning session(s) does the person feel are best for them?

Do they want to make any sort of decision-making agreement?

Does the person need support with communication - Is a communication chart needed?

Does this person have a learning disability, if so, do they have a person centred plan?
