PRINTING TEMPORARY

ID CARDS

Members of our 2018 medical plan may get a copy of their ID card through our new

MyQHealth website www.hmhealthplan.com . Register on the MyQHealth website using

your Houston Methodist email address as your USER name.

Once registered look for the REQUEST ID CARD tile at bottom right:

1) Click on the DOWNLOAD link to Request ID CARD

2) Click on the Physician Specialty drop down arrow (yes – this is correct)

a. Scroll down and select: “None – Need Replacement Card”

b. Select “DOWNLOAD” (give it a minute)

c. Open to PRINT or Save the ID card to your PC or phone
