Physical Assessment

The patient's hair is blackin color and is evenly distributed. His hair is short and is styled with "military cut". There were no foreign materials on his hair nor any lesions. There were no other hair present on his body.IV. HE!The patients head"skull is symmetrical and normocephalic with smooth muscle contour. #o nodules and lumpsor pain noted. He has symmetric facial features and facial movements. Edema and lesions were also absent.V. E$E%The eyebrows are symmetrically aligned with e&ual movement. Eyelids are intact with no discoloration nor discharges. Eyelids close and open symmetrically. The sclera appears white with small capillaries. The cornea is transparent and shiny. 'upil is black( rounded and e&ual in si)e.VI. E*%.uricles on both earshave the same color as the facial skin without tenderness. It recoils after it is folded. There were no lesions nor edema. In the ear canal( there were few sticky cerumen noted. The patient has normal sense of hearing without complaints of earaches.VII. #+%E #! %I#,%E%The nose is symmetric and uniform in color. !ischarges and tenderness were absent. The nostrils onb both sides are patent where air moves freely as the patient breathes through the nares. The sinuses are not tender and swellings are absent. #o mucus noted.VIII. -+,TH #! TH*+T'atient's lips are reddish brown. He can purse his lips upon command. Inside( the gums are firm with no swelling noted. He has tooth decay but does not complain of toothache. His tongue is lightly pink with tinywhitespots. It moves freely from up to down and right to left with no signs if difficulties.
