I am happy to write the foreword of the June 2012
issue of Chalak Patra. I have observed that Chalak Patra
has been a regular tool for upgradation of the technical
and safety related knowledge of the electrical running
staff of this division.
So far this division has maintained high standard
in the safety front. I would like that all electrical
running staff of this division including the newly
recruited ALP through RRB should take interest in study
of Chalak Patra and should also contribute for its further
development.

(i) On 18.04.12, LP working with Loco- 27779+27832 /WAG-7/MU failed the loco in between RNG-MTPS due to failure of MP/EEC with LSB glowing. Fresh loco- 27237+27193/WAG-7/MU/BNDM O/L at 09.05, PR at 09.25 and arrived at 2nd hopper at 09.50. CLI K.Mahato attended the loco and found J-2 on improper position. He set it normal and then loco become normal. This was a case of improper trouble shooting of loco.
02 sets p. pass withheld. (ii) On 09.05.12 ELP (M/Exp)/ASN while working loco no: 22737/WAP-4/HWH
train no: 18450 Dn stopped the train at DPX/DNR div. due to External fault of SIV. He failed to trouble shoot the same properly even after advice of CLI/ASN and subsequently loco got another problem of smoke emission from MVMT-2 after JMT. This was caused detention of the train.
Increment stopped for 01 year(NC). (iii) On 22.05.12, ALP (E)/ASN while working train No: 13019 up with electric
locomotive no: 21389/WAM-4/ASN failed to drop the token at token dropping point near BJU and as a result path was blocked and train no: 15631 & 15281 detained in repercussion.
One set p.pass of LP stopped and SF-11 of ALP has not yet finalized.
(iv) On on 28.05.12 ELP (G)/ASN ,while working train No: EC/GHTL with Elect
Loco No: 31166/WAG-9/LGD, train stopped near BRR/Adv/signal ( middle of the bridge) at 21:00 hours due to application of penalty brake by VCD as he failed to acknowledge VCD switch in proper time. As a result train no: 23319 up and 12259 up detained in rear at BRR and ULT respectively

(v) On 30.05.12 ELP (G)/ASN went to Chanch siding with coupled light engine no: 23330/WAG-5 +27878/WAG-7 from KMME and after detaching the engine no: 23330/WAG-5 at chanch siding, while standing with loco no: 27878/WAG-7 loco was rolled down from mast no: EC-61 to EC-69 and entered into the panchet road due to not getting any track and derailment of the loco no: 27878 was occurred.
DA enquiry of SF-5 of LP is under proceeding and SF-11 of ALP has not yet been finalized.

(i) On 03.04.12, Sri Asoke Barua worked Train No-Ec/BOBR/E, hauled
by Loco No-23815+23801/WAG-5/ASN.After completing unloading at DPL
factory, he was drawing 59 BOBR/E. When the train was passing DCOP shunt
signal No-45 from 19:50hrs, he observed that OHE bracket tube was
displaced from its alignment. He stopped his train and lowered the
pantograph. But pantograph was entangled without damaging it. He informed
TLC & section control immediately. Thus Sri Barua saved possible major
disruption of TRD, which could have been massive and claimed heavy toll of
damage.
DRM/ASN has conferred award of Rs.500/-
(ii) On 10.06.12 LP B.Saha/ASN was working with loco- 21323/WAM-
4/MGS, train no- 13008 Dn. He left JAJ at 19.21 hrs. After passing neutral
section JAJ-NRGO he observed ICDJ with BA-voltage “0’. So he stopped the
train at KM-363/10 at 19.25 hrs. On checking, he found one terminal of BA
smoked in BA box-2. He by passed the concerned BA. Now BA-voltage shown
80V. He closed DJ successfully but observed non functioning of CHBA. He
informed TLC and started from the spot at 20.00 hrs. He drove the train to
MDP and arrived at 21.12 hrs successfully. Loco was changed at MDP.
SrDEE/OP/ASN has conferred cash award of Rs. 400/-
(iii) While working train no- 63522 on 12.06.12 Sri R.K.Sengupta
LPP/ASN got badly injured due to sudden opening of look out glass of driving
unit no. 21005(H/E) of MEMU rake (21012+21032+21005 base-ASN) in
between GLI and KAN at about 17.50 hrs. His right hand was badly
injured and also bleeding heavily. However, he was not mentally upset
and continued to work alone up to BWN and reached the destination
safely with his train in right time. He was treated at BWN and had to
undergo stitches on his arm. He has shown exemplary dedication in his
work in the interest of railway.
DRM/ASN has conferred cash award of Rs.500/-

3. Malfunctioning of ZSMS/SMGR
LP/ASN took the charge of loco – 21384/WAM-4/ASN with train no- 13106 Dn at 19.15 hrs. LP found MP/EEC failed at the time of starting at 19.20 hrs with message on MPCS “TLTE due to P-2 OFF”. He tried with Manual control of GR also but unsuccessful. In the mean time CLI/JAJ also attended the loco but he was unsuccessful also. So LP failed the loco at 19.55 hrs. CLI/JAJ and LP continued to trouble shoot further and found SMGR air cut out plunger spring pressed inside and entangled. They pulled it anyhow and given fit the loco at 20.30 hrs. Same loco worked the train further. Reamrks: ZSMGR might have operated and it s plunger reamained in pressed even after bringing ZSMR in normal condition. So LP should focus on this point also in case of MP/EEC failure.
4. Failure of loco- 30042/GZB-12004Dn/NDLS-LKO Shatabdi Express due to speed not increasing more than 1kmph on 06.06.12

Train stopped after starting from GZB due to ACP in the coach. It again left after resetting the ACP but LP stopped the train due to speed not increasing more than 1kmph although tractive effort was 200KN. Since abnormality occurred after ACP, driver presumed the problem being related to brake binding and the TXR staff released the whole train. The problem continued even after release of train. Loco was detached from the load. M/Staff isolated bogie-2 but did not succeed. Thereafter they isolated bogie-1 and loco became normal. Fresh loco – 22656 worked the train. On down loading the DDS details of locomotive, it is noticed that driver after ACP had experienced speed not increasing more than 1 kmph and loco not moving even though tractive effort was 200KN. As per TSD, in such an event driver is required to see the message in DDS. In the present case DDS had message of error Taco generator 3 i.e. Traction Motor-3 speed sensor having some problem. As per TSD driver is required to isolate bogie on by one and trouble shoot the loco accordingly. However, from DDS it is noticed that message of techo generator error appeared once, though the driver has tried to move the loco repeatedly. It appears the driver got confused since this problem of creep was followed after ACP and driver paid full attention to ACP ignoring the possibility of loco having
some abnormalities.

1. Mal functioning of VCD in rear of Multi loco
On 26.04.12 LP/UDL was working with Loco- 23879(H/E)+23818(K/E)/WAG-5/MU/ASN Train NO- SEP/NH/Crack. Loco -23818 was dead as load was permissive. After passing neutral section OYR-DCOP, train stopped due to failure of MP/EEC. LP tried to trouble shoot as per his best but unsuccessful. So he started the train with manual control of GR and arrived at DGR at 02.25 hrs. CLI Sri B.K.Singh/UDL attended the loco and found VCD (H/E) of rear dead loco- 23818 s get activated automatically. Lamp was flashing and buzzer was sounding also. He put OFF/ON HBA and put HVCD on isolate position. Now front loco became normal. Train started from DGR at 02.50 hrs, arrived PAJ at 03.50 hrs. Here again CLI normalize rear loco VCD and found normal behavior of loco. LP worked the train up to NH without any trouble further. Remarks: As per working system VCD of rear loco remains in sleep mode. But in this case it get activated and caused failure of MP/EEC of front loco.
2. Mal functioning of VCD
On 18.05.12 LP Nausad Ansari/ASN was working with loco- 27193(H/E) +27273(K/E)/WAG-7/MU/BNDM, train no- MTPS. As per LP train left RNG at 03.22 hrs and stopped at KM 14/1 at 04.25 hrs due to failure of MP/EEC. He checked both loco separately and found notch regression after 1st notch. There was a previous booking in log book about wedging of QVCD due to notch not progression. But it was made normal by sucessive LP and entry was also logged about normal behaviour of loco. Now LP again wedged QVCD and Suceeded to normalise the loco at 05.45hrs. Train arrived at MTPS at 07.10 hrs. Reamrks: Some times notch regresses after 1st notch due to mal functioinig of VCD system in SIV fitted loco. So LP may try to trouble shoot it by wedging QVCD in de-enrgised condition also.

Board, vide letter under ref (iii), have advised Zonal Railways to ensure that the Drivers do not carry Mobile phones on person while working train but keep them in the bag /box for personal stores. The mobile phone should remain switched off/or in silent mode during the entire journey and should not be used during train run except in case of emergency to call for assistance in the event pf accident or failure.
It has been reported that the discipline is not being adhered to and the mobile phones are being used on duty by crew for their personal communication as well as other purpose including enquiring of the signal aspects ahead and train running status.
This is being reviewed seriously and therefore, Zonal Railways are advised to establish practice to get the call records of CUG phone of running staff checked at random to identify their usage while on run and take sever action against the defaulters.
