“NursingHomeTransition(NHT)isaprocessofassistingandempoweringresidentswhowanttomovefromaanursingfacilitybacktoahomeoftheirchoiceinthecommunityThroughtheNursingHomeTransitionProgram,ouragencyassistsconsumersundertheageof60.LifeandIndependenceforTodayalsoprovidesoutreachandeducationtoresidents,nursingfacilitystaff,families,andcommunitymemberssothatconsumershavethenecessaryinformationtomakewell-informeddecisionsabouttheirtransition.
