“NursingHomeTransition(NHT)isaprocessofassistingandempoweringresidentswhowanttomovefromaanursingfacilitybacktoahomeoftheirchoiceinthecommunityThroughtheNursingHomeTransitionProgram,ouragencyassistsconsumersundertheageof60.LifeandIndependenceforTodayalsoprovidesoutreachandeducationtoresidents,nursingfacilitystaff,families,andcommunitymemberssothatconsumershavethenecessaryinformationtomakewell-informeddecisionsabouttheirtransition.

ATAisobservingits40thAnniversaryYearin2016.Thesixcountytransportationagencywasformedin1976.ThetransitauthorityandLIFTsharethesamegeographicserviceareaandcoordinatemanyactivitiesforthedisabledandjointadvocacyforADA,nowinits26thyear.
