Action Required: Please review the attached list of users who have accounts on the New MMIS who have never logged in. This spreadsheet is a screenshot and is not searchable any other way than listed (alphabetically). If a staff person does not need access to the new MMIS, please email David C. Hutchings (Dhutchin@dhs.state.or.us) to request their account be revoked. Staff must log-in to the new MMIS by February 28, 2009, to keep their account open or a new IUP will need to be resubmitted to re-open their account. Reason for Action: Staff must log-in to the new MMIS by February 28, 2009, or their rights will be removed. Field/Stakeholder review: Yes No
