Happy New Year 2014! In 2013 Ahora y Siempre, Inc. (AyS) had some great accomplishments and things to be proud of. At the end
of 2013, we secured office space in Del Rio, TX where we will open our first satellite office on February 1, 2014. To say that we are
excited about this expansion is truly an understatement.
Much progress was made over the last year on many fronts. Our growth continued, and we reached over 100 clients in 2013. To
give some perspective, when we started 2013 we had about 60 clients and by the end of 2013, we were well over 100 clients with
five other clients waiting to be placed. As a result our Client Care Coordinator teams are now better defined. We have team Mariela
and Hector and we have team Jai and Genesis. Mariela and Jai serve as lead coordinators while Hector and Genesis serve as backup
coordinators. Your team of nurses continues the same; Glenna is the Registered Nurse while Mary is the Licensed Vocational Nurse.
The Registered Nurse is responsible for initial assessments as well as annual assessments. The RN will also provide delegation for
medication administration as needed. The LVN will do focused assessments as needed and will provide education about diagnoses
and medications as needed.
As we continue to grow and implement new systems to better serve you, we will use our Quarterly Newsletters to share these
systems with you. In this newsletter we will explain to you what happens to your paperwork when it reaches our office so you can
have a better understanding about what is going on in our office.
We wish you prosperity but most importantly happiness and health in 2014. Remember that we are here to serve you. If there is
anything we can do to help you, we are only a phone call, text message, email or Skype message away. We hope to see you at our
Quarterly Advisory Committee Meeting later this month.

Quarterly Advisory Committee Meeting Wednesday, January 22, 2014

As a new year begins, we have many new goals and
changes to share with you. Please join us for this
quarterly meeting that provides very important details
regarding the services provided at AyS. Clients, parents,
guardians, caregivers, advocates, friends, everyone will
benefit from the details shared. Please bring your
questions, comments, or concerns for us to address.
All Advisory Committee Meetings take place at our main
office in downtown Dallas in the Adolphus Tower,
located at the corner of Akard & Main St. We provide
FREE PARKING in the garage directly across from our
building. Please call us for directions if needed.
Advisory Committee Meetings are in English & Spanish.
Light snack & beverages are provided.
Appointment Sheets When you visit a clinic, we ask that you request the clinic staff to complete an
Appointment Sheet as record of your visit. This form lets us know the results of
your visit. Important information such as follow up appointments, medication
changes, lab tests, etc. is noted on this form. If the clinic staff will not complete
the form please ask them for documentation of your visit, such as a printout of
their consultation or progress notes.
Always get a doctor’s signature on any prescriptions or medication changes.
Please do not complete Appointment Sheets yourself. If you have further
questions or need additional assistance, please call our office.

Always call the nurse if…
Client falls or gets injured. Client refuses medication.
Client starts a new medication. Client goes to the ER.
Client is admitted to the hospital. Client has fever, nausea, vomiting, or diarrhea.
Client seizure disorder changes. Client behaviors require staff to call 911.
Fire Drill Reports are due on
Friday, January 31.
Remember to always…
Conduct drills Awake & Asleep.
Practice 1st & 2
nd escape routes.
Complete drills in 3 minutes.
Practice every other month.

(Forwards logs to Client Care Coordinators & Nurses.)
Client Care Coordinators & Nurses
Review and follow up with client, family, therapist.
If you provide transportation for a client, we must have a copy of your driver’s license
and auto insurance coverage on file (current & valid). Please remember to resubmit any
expired documents once they are renewed. If you do not have a driver’s license and
auto insurance coverage, you are not eligible to transport the client(s). Please contact
Human Resources if you have any questions.
Do you know someone looking for part or full time work?
If someone you know is interested in working with the Special Needs
community please have them contact our office and request to speak
with our Human Resource Department. We are actively seeking SHL,
respite, and foster care providers. Applications are on our website.

Service providers, below is a flow chart of the various departments that review, approve, and process service
delivery logs and invoices before payment is made. As paperwork comes in each week we must verify that all
forms are meeting DADS requirements and billing guidelines. It is very important to fill out forms entirely.
Always include your signature on each form and avoid leaving anything blank. If you any questions or need
help, please call our office.
