A 41 year female with a large left thigh mass.She also has a small mass on the back of the right shoulder which has a remarkable increase in contrast uptake on nuclear scan. She has a previous history of an excision of a thyroid mass done at a private institution and the biopsy result was that of a Chondrosarcoma..

The gross, histological features and immunohistochemical profile of this tumour are those of a extraskeletal mesenchymal chondrosarcoma.

An incisional biopsy of the left thigh mass was performed and the result was a mesenchymal chondrosarcoma.

Then an excisional biopsy was done.

 Immunohistochemistry –

S100 - Positive

Vimentin - Positive;

Cytokeratin AE/13 - Negative. .

Microscopic findings –

The sections show a diffuse proliferation of pleomorphic small, round to oval cells.

In some areas, a haemangiopericytoma-like growth pattern is present.

Cartilagenious areas with mild pleomorphism of chondrocytes in the lacunae and calcifications are present.

Results

IntroductionMesenchymal chondrosarcomas constitute less than 2 % of all chondrosarcomas. Approximately two thirds of cases of mesenchymal chondrosarcoma occur in bone while the rest occur in places outside of the bone—i.e.in extraskeletal locations.They are aggressive lesions with a high propensity for metastases.Mesenchymal chondrosarcoma is a fast growing tumor that spreads more often. They can remain dormant for long periods of time.

This case illustrates multi-focality, recurrence and metachronous nature of a extraskeletal mesenchymal chondrosarcoma.
