How Wellness Points Bank Works Accumulate points for participating in Wellness Points Bank activities between

January 1 and August 31, 2012

Earn the minimum number of points by August 31, 2012 300 points for employee only or employee plus child(ren) 400 points for employees who cover a spouse/Same-sex domestic partner

Keep track of your points at StayWell Online using the “My Points Bank” progress tracker

Receive either a $300/$400 reduction in your 2013 UPlan Medical Program rates

Covered spouses/partners can contribute up to 100 of the 400 total points required, but are not required to do so

Biometric Health Screenings – 100 points Boynton-delivered health screenings

Available in the Twin Cities, Duluth, Crookston, Morris, and Rochester campuses Appointments and walk-ins accepted 15-20 minutes complete full screening No additional charge to UPlan members, including spouses/partners

Screenings available: Fasting cholesterol* Fasting glucose* Body composition and Body Mass Index (BMI) Blood pressure

Health Care Provider Form also available for those who choose not to attend a Biometric Health Screening on campus

*Fasting requires that you consume nothing other than water for at least eight hours prior to the health screening.

Fit Choices – 50 points Visit a participating fitness center eight times per month and receive a $20

credit toward membership dues Receive the $20 credit at least five months between January 1 and July 31 to qualify

Bike Commuter Program – 50 points Get a free tamper-proof tag installed on your bike

In the Twin Cities have your tag installed at the Campus Bike Center in the Oak Street Ramp

If you bike to a campus or workplace that does not have an RFID station, please contact [email protected] for your options

Ride past one of the RFID stations located on campus to have your commute tracked electronically RFID stations will be located on Crookston, Duluth, Morris, and Twin Cities

campuses (East and West Banks and St. Paul) Participants not located on one of the campuses can submit their commutes

manually Bicyclists are required to make 40 round trips by August 31, 2012 to

earn 50 points Only one ride past a station per day will count

Self-reported Pledges – 25 points each Annual flu shot pledge – 25 points

Sign and submit a pledge that declares you have received a flu vaccination within the last 12 months

Non-tobacco user pledge – 25 points Sign and submit a pledge that declares you are currently tobacco

free and have been for at least the last six months This pledge does apply to people who have always been tobacco-

free Both pledges are available at StayWell Online in the “My

Points Bank” tab To submit your pledge, simply enter the date that you are

reporting your pledge – “today’s date”

Get Fit and Reduce Stress (Twin Cities only)Two reimbursement programs still available:

Group Strength Express (GSX) 8-week fitness series to improve strength and endurance $199 reimbursement for individuals who attend 13/16

classes, and complete a pre- and post-assessment Mindfulness Based Stress Reduction (MBSR)

8-week class to reduce stress and provide coping techniques $200 reimbursement for individuals who attend 6/8 sessions

and a day-long retreat No wellness points are available for these programs at

this time.
