FMEA – what is it?

Failure mode and effects analysis

FMEA- Methodology

List all usages, functions,

requirements, process steps.

Which failure can happen?

What are the effects?

How to detect or prevent these

failure?

How much it impacts the product normal

behavior?

How often it can appear?

How good these methods are to

detect or prevent such failure?

What are the improvements we can make to lower

the risk?

What are the causes?

FMEA Key Management AspectsPart/Process Integration

•FMEA is about part and process. Changes on these items can impact the FMEA.

Centralized hosting•All the FMEA in one organization should have the same format and be hosted in the same place to facilitate analytics and reports

Standardized entries•To get a real value out of your FMEA reports you need to be able to have standardized entries. Two similar causes have to have the exact same name.

Fill Collaboratively•Creating an FMEA is a lot of work and people specialist on each area should write their section of the FMEA.

Send Actions•An FMEA’s target is to lower risk by running preventive or corrective actions based on a risk analysis. Actions are run by multiple peoples, you can’t just send emails.

Consume actions’ outputs•Once the action are run. The output should be a change of the risk values. These should be easily updated in the FMEA.
