Map of Medicine (MoM)
In the West Midlands there are various local healthcommunities using the MoM to assist them withvarious transformational projects:
• North Staffordshire and Stoke-on-Trent have localised a number of Musculoskeletal (MSK) and Chronic Obstructive Pulmonary Disease (COPD) pathways in order to support their ‘Fit for the Future’ Programme
• A web based visual representation of evidence-
based patient care journeys
• Provides doctors with the information they need to make informed decisions based upon national guidance and also local processes
• Accessible for patients to see through the NHS Choices website
• Empowering patients to give them choice and control

Lorenzo Podiatry
• Clinical notes: record consultations, upload photos of wounds, treatment plans and facilitates work flow.
• Forms: monitor wounds, assess foot risk, record nail surgery assessments and treatments.
• Clinical documents: letters back to referrers and referrals onto consultants. • Very stable and reliable, even when used out and about in patient’s homes via 3G mobile broadband.

Accident and Emergency Department
• Able to view Ambulance ECS patient information on the Webex Application within in the A&E department at University Hospitals Coventry and Warwickshire (UHCW);
• Able to improve efficiency and safety in the A&E department:

• Technology updates should allow for data to be assimilated into NHS Care Record Service compliant A&E records.

• Enabling clinical images to be stored electronically and viewed on screen;
• Access at the touch of a button, patients can benefit from much speedier diagnoses and more informed consultations;
• PACS enables the comparison of old and new patient images - measuring the effectiveness of their patients' treatment and monitor their progress;
• Images are easily accessed and can be shared;

Electronic Discharge Summary(EDS)
• NHS Hereford has developed Electronic Discharge Summary system with all its GP practices using DOCman.
• EDS significantly speeds up communication between clinicians. Able to share EDS with wider team. Community Matrons and District Nurses are also able to access the information.
• Improving the quality and quantity of information sent to a patient's GP.

Using Choose and Book to book a physiotherapy appointment
• A GP or patient can book physiotherapy
appointments using Choose and Book at
University Hospitals Birmingham and NHS Hereford;
• Offers the patient an appointment at a time and place convenient to them.

information A secure means of sharing clinical data:
• SMS text reminders for appointments
• Electronic Discharge Summaries taken from NHS Mail straight into patients GP records (via Docman)
