Mr. Williams,I believe a good course of action to take with Ms. Rider would be to request a copy of your own forms. This should not be a problem as they contain your information. In addition to this, perhaps Ms. Rider would not mind creating some “SAMPLE” forms. These could be filled out by her staff with plausible, but not personnel specific, information. These ideas will help get you the information you are looking for without stepping over any confidential boundaries.
