• The PSCI Population State‐of‐Health Analyzer utilizes EMR, claims and pharmacy data to perform risk stratification of patients by chronic conditions such as ― diabetes, coronary heart disease, asthma, osteoporosis and others.

• Using sophisticated but clinically proven algorithms, the module computes state‐of‐health scores and segments patients into high, medium and low risk pools and color codes them for easy visualization.
