Nancy Clay, a 31 year old production manager died while on the phone with Chicago 911. Firefighters didn’t know she was on the 20th floor office on One North Wacker Drive.

May 13, 1987


Kaafee Billah, age 39, called 911 due to chest pains. 911 responded within minutes, but left after finding no one. Mr. Billah was found deceased, 10 hours later in his office, across the street by the cleaning staff.

Dr. Bivort, age 77, was hiking in the Massif du Mont-Blanc where he suffered a leg injury. He called for help and remained online for more than an hour trying to detail his position. Dr. Bivort’s remains were found 2 years later.

July 2009


Mark Fletcher, a 48 year old Telecommunications Engineer, suffered a brain aneurysm at his home in New Jersey. His daughter used his VoIP phone to call 911. A Dallas 911 operator directed her to find another phone as she could not transfer the call. Fletcher survived, after brain surgery and a 6 week coma.

October 12, 2009


Deanna Cook, age 32, can be heard choking, screaming and begging for her life on the 911 call. Police did not come for 50 minutes after she called and left when no one answered the door, leaving her family to find Cook dead in a bathtub two days later.

August 2012


Kari Hunt, a 31 year old mother of 3 was brutally murdered in a Marshall, Texas hotel room. Her 9 year old daughter knew to dial 911 – and DID 4 times - but only got a fast busy signal. She was never told to dial 9 for an outside line on a PBX.

December 1, 2013


Randy Palmer, suffered a heart attack in a Midvale, Utah auto parts store. 911 responded to the Salt Lake City store due to a location reporting error. Doctors say Mr. Palmer could have survived had EMS gotten to the right location sooner.

January 22, 2014


Shanell Anderson, age 31, crashed her vehicle into a lake. She called 911 and told dispatchers exactly where she was. Because she was just outside of the map boundary there was confusion locating her for 20 minutes. She never regained consciousness and died 12 days later in the hospital.

December 29, 2014


Jose Beltran, a 13 year old student collapsed in gym class in Delano California. A call from the school was routed to a 911 center in Ontario, Canada. The call was then sent to the Delano Police administrative lines, and had to be transferred again to County for the ambulance. Beltran was pronounced dead at the Delano Regional Medical Center.

January 26, 2015


Police in Dortmund, Germany searched for a woman’s car that crashed and plunged down a slope. She was trapped in the car, but was still able to call 112 with her cell phone. The exact location was not known and the phone connection dropped. After about an hour the woman was eventually found in her vehicle by fire fighters who had searched the road on foot
