Fatigue• A worker who is sleep-deprived can be just as hazardous as

Outdated Safety Records• Are your training records current? • How about the safety data sheets? If you’re relying on

someone to manually update them each time there’s a change, you could be opening yourself up to errors, not to mention inefficiencies.

Disorganized Policies and Procedures• Do you have a step-by-step process outlined for safely

locking out or tagging out equipment to prevent the release of hazardous energy?

• What about your other safety procedures? If your employees don’t have consistent guidelines to follow, it’s more difficult for them to act responsibly.

Poor Communication• Paper-based processes and inconsistent

communication procedures can create a disconnect between departments.

• If your workers aren't quickly adapting to new policies —or if you're seeing policies that aren’t uniformly enforced—you may need to implement more consistent systems for relaying information across your workplace.

fear of something—whether it’s missing regulatory deadlines, being injured or getting slapped with an OSHA violation—they won’t be working safely or efficiently.
