The ONE and ONLY

• The only time you would MOVE anything on site is to assist or make safe the area to assist to give a worker first aid or remove the PATIENT to get medical treatment. If the subject is dead NOTHING MOVES

Not all incident are earth shattering

• But where large scale property or workers become seriously hurt, you have to start collecting proper data and calling the proper agencies into to do there work!

As a Supervisor YOU must know

• As a Manager or supervisor you must know the provincial/state and company rules on reporting serious incidents at work and start the investigative procedures IMMEDIATELY

Basic Steps Secure the scene Control hazardous conditions Provide care Call EMS and law enforcement if necessary Isolate the scene Notify OH&S, Supervisor, Corporate Office,

Insurance carrier Gather evidence Identify and interview witnesses

Accident Scene Preservation

• Secure the area• Photograph the area from various angles– Use an item of known length to give perspective

to objects• Draw a picture or diagram the scene– Details are critical

• Collect samples, if applicable

Accident Scene Preservation

• Collect written documents– JSAs, log books, work permits, contracts, training

records• List eye witnesses and contact information

(interview ASAP)• Interview emergency personnel about the

conditions upon arrival• List all equipment and materials involved

Accident Scene Preservation

• List environmental factors– Wind, temperature, light, dampness, time

• Determine the original location of all items• Obtain copies of law enforcement report and

any photographs taken

Think and Know The Four P’s of Evidence

• Physical evidence– Tools, equipment, spills, leaks, debris

• Paper evidence– JSAs, procedures, training records

• People evidence– Written statements ASAP

• Picture evidence– Photos and diagrams

Guarding Floor and Wall Openings

• Among the violations cited were:• Unguarded floors, platforms or runways• No stair railing• Unguarded chutes in walls

Electrical

• No strain relief• Switchboards and panelboards not in dry

locations• Covers missing from junction boxes• Power strips used in lieu of fixed wiring or not

UL approved• Bushings not used when cords pass through

openings

Electrical

• Equipment not listed or labeled for the purpose for which it is being used.

• Electrical free from recognized hazards• Sufficient working space around equipment• Guarding of live parts• Identification of disconnecting means

Eyewash facilities

• No eyewash facilities• Using non-potable water for the eyewash

station

No PPE worn when dumping caustic

Fire Extinguishers

• Fire extinguishers were not charged for immediate use

• Not mounted, identified• Inspection and maintenance• Annual training

Confined Space

• Informing outside contractors of the hazards of the host employer's confined spaces • Confined space determination

• Posting• Positive measure to prevent entry• Written program

Hazard Communication

• No written program• Copies of all SDS’s not available in the

workplace
