1. How have healthcare providers taken time to understand your personal, cultural and faith background?– i.e. medical interpreter, male/female physician

option, dietary restrictions, etc.

2. How are your children treated? Please describe some notable differences between your care and theirs—such as quality of care, time spent with the doctor during a visit, level of accommodation for dietary/cultural/faith based needs, etc.

3. Has there been a time when you left an appointment unsure of how to manage your medication or maintain your health? – What could providers or the healthcare system

do differently for you to manage your health?
